PRODUCT DESCRIPTION

1. REQUIREMENTS:

The Contractor shall provide Local Exchange Carrier Services and Features as defined in the National Local Exchange Carrier Services for the Service Delivery Points (SDPs) listed in the attached workbooks. The SDPs will include locations across the Enterprise.

No changes to services or features may be applied to this requirement except as approved by the Contracting Officer’s Representative (COR) or designated Office of Information & Technology

Solution Delivery/Infrastructure Engineering and Provisioning representative.

6. INFORMATION SECURITY CONSIDERATIONS: See the basic contract Performance Work Statement Section 6.0 for applicable security requirements.

7. SPECIAL CONSIDERATIONS:

As defined in PWS 6.1 POSITION/TASK RISK DESIGNATION LEVEL(S) AND CONTRACTOR PERSONNEL SECURITY REQUIREMENTS

The Contractor(s) shall comply with all personnel security requirements included in this contract and local level organization security requirements described in each individual task order.

The position sensitivity of contractor work performed under any contract or task order resulting from these requirements is designated as low risk and the level of background investigation will be commensurate with the required level of access.

A. All Contractor personnel shall always be escorted by facility OIT staff while on station. Contractor staff will not require PIV badges due to being escorted, however will be badged by the facility as a visitor. Contractor staff must check in with facility OIT Area Manager or LCON as identified in TO prior to performing any work or installations.

B. The Contractor shall be responsible for the actions of all personnel provided to work for VA under this contract. In the event that damages arise from work performed by Contractor provided personnel, under the auspices of this contract, the Contractor shall be responsible for all resources necessary to remedy the incident.

TRANSITION OF SERVICES AND LOCATIONS

i. In the event that the Contractor is not already the incumbent location service provider, the Contractor shall perform a site survey and coordinate with the VA facility. The Contractor shall have 30 calendar days to develop and deliver to the VA COR an Installation Plan and Cutover Plan including a listing of tasks identifying the installation and cutover approach. The Contractor shall not commence installation and cutover until the Contractor receives acceptance of the service installation and cutover plan from the VA COR. Deviations from the VA approved Transition Plan shall be coordinated with the VA COR prior to any work being accomplished in the affected area. The Contractor shall coordinate with the government to ensure all circuits and services provided by the incumbent, under the prior contract, are disconnected/discontinued upon successful cutover. In the event that the TO award is made to the incumbent Contractor, no installation/cutover plan shall be required.

ii. No service shall be transitioned from the incumbent service provider to the awarded service provider without due consideration of incumbent’s contracted period of performance. No physical transition of services may be effected without government approval. Where the awardee of this task order is the incumbent provider, the contractor may initiate transition to the National LEC Services Next Generation Contract immediately with the understanding that they will waive any early termination fees from the previous contract.

C. INSTALLATION PLAN - The Installation Plan shall contain the following site specific information:

i. Description of equipment to be installed.

ii. Location of equipment to be installed including placement of equipment and floor plan.

iii. Installation schedule.

iv. Site Survey and Support Requirements – specific to the individual site. The Government will provide floor space for the installation of the Contractor’s owned and provided transmission equipment. The floor space shall be limited to accommodate Contractor-provided relay rack(s), 19 to 23 inches wide by 7 feet tall, as ceiling height permits at each designated demarcation. The Government will provide adequate wall space should the Contractor not require floor/rack space. The Government will provide electrical power and room environment for the installation of the Contractor equipment. When additional rack space, floor space, or wall space is required to support Government requirements, the Contractor shall coordinate with the VA Area Manager (AM) or designee. The Contractor shall provide a listing of all discrepancies that do not meet local electrical codes, seismic codes, and also identify Government deficiencies in regards to floor loading and environmental conditions to perform all requirements of individual Orders.

D. CUTOVER PLAN – The Cutover Plan shall contain the following information:

i. Support required.

ii. Actions and events for cutover.

iii. Contractor responsibilities.

iv. Incumbent Contractor responsibilities and expectations.

v. Government expectations.

vi. A back-out approach which restores service to its original state.

vii. Porting and retention of all existing telephone numbers.

viii. Letters of Authorization (LOA) for transfer of telephone numbers from incumbent LEC.

ix. Simultaneous operation of telecommunications services to minimize down time. The Contractor shall coordinate cut-over of telephone service with the VA COR and local technical contact to reduce down time.

E. INVENTORY OF LINES AND CIRCUITS

i. Upon TO completion, the Contractor shall complete an inventory of all circuits. The inventory shall include the demarcation binding post assignments, the local and long haul circuit identification, the Network Interface (NI) location, and monthly recurring charges for each item in the inventory at each demarcation point for the location specified in the TO. The Contractor shall submit the baseline inventory data to the designated COR no later than 15 business days after the completion of the installation.

ii. The development of the existing services inventory provided by TO Contractor will be initiated under a separate TO. The Contractor shall provide account information and will maintain the master list of circuits and inventory as well as the site locations identified in Addendum C. VA sites not currently listed will be added by a modification to the base contract.

F. During the base period of performance of this task order and any option periods effected, the quantities and types of services and features will fluctuate routinely with regard to SDP activations/deactivations.
