STATEMENT OF WORK

A. GENERAL GUIDANCE

1. Title of Project: Dock Levelers Service Contract for VA North Texas Health Care System (VANTHCS) and other locations under VANTHCS, listed below.

VA Dallas 4500 S. Lancaster Rd Dallas TX 75216.

VA Garland 2300 Marie Curie Blvd Garland, TX 75042

VA Grand Prairie 2737 Sherman Street Grand Prairie, TX 75051

VA Bonham 1201 E. 9th St.

Bonham, TX 75418

2. Scope of Work: Organized preventive maintenance program for VANTHCS loading dock levelers conducted on a semi-annual basis. Repair and/or Replace Dock Levelers as needed Emergency Repairs.

• Technicians shall thoroughly clean equipment and components to ensure longer life.

• Remove dirt and debris from clogging working parts and causing premature wear.

• Technicians apply correct lubricants in proper places to ensure equipment works efficiently. To reduced components from wear, moisture, and corrosion.

• Make all necessary component adjustments to make sure equipment works smoothly and consistently.

• Technicians shall thoroughly inspect equipment and bring small problems to government's attention before they become big issues. Provide predictive maintenance solutions as needed.

• Provide documentation to show service work that has been completed and to indicate areas of problems, and potential maintenance issues down the road.

a. CONTRACTOR RESPONSIBILITIES: The Contractor shall exercise extreme care to avoid damaging Government property such as docks, buildings, and equipment. Any damage to Government property by the Contractor shall be repaired by the Contractor to its original condition at no additional cost to the Government. The Contractor shall ensure that all materials are new, free from defects, imperfections and asbestos free (NO ASBESTOS CONTAINING MATERIALS SHALL BE USED ON THIS PROJECT). If replacement dock levelers are required vendor shall provide in like kind (USA Made)

I. Contractor shall immediately, but not later than 24 consecutive hours after discovery, notify the CO and COR, (in writing), of the existence of the development of any defects in, or repairs required to, the scheduled equipment which the contractor considers he/she is not responsible for under the terms of the contract.

b. SCHEDULE OF WORK: The Contractor shall notify the Contracting Officer’s Representative (COR) Charles Cole, Engineering, phone: 214-857-4161, prior to start of work. The Contractor shall schedule work through the COR. The Contractor shall coordinate work with COR NLT 3 days before starting work. Work shall be complete with-in 40 hours of work start. Other contractors may be working in the same area performing other work. The contractor shall execute work in a manner so as not to impede other contract work.

3. Hours of Work: All work shall be performed between 6:00am and 5:00pm (CST) M-F due to where dock levelers are located, and surrounding areas shall be congested with traffic of the hospital.

Weekend only (not to exceed 40 hours) Work shall be performed during weekend work hours only. Work to be performed in other than weekend hours shall be approved by the Contracting Officer’s Representative.

Federal holidays observed by VANTHCS:

New Year’s Day Martin Luther King's Birthday Presidents Day Memorial Day Juneteenth Day Independence Day Labor Day Columbus Day Veterans Day Thanksgiving Day Christmas Day

Emergency / Unscheduled Maintenance:

a. Contractor shall maintain the equipment in accordance with manufacturer’s standards.

b. The contractor shall provide repair service, which may consist of calibration, cleaning, oiling, adjusting, replacing parts, and installing parts.

c. All parts will be covered in this agreement.

d. The Contracting Officer, COR and designated Alternate have the authority to approve/request a service call from the contractor.

Response time:

a. Contractor’s FSE shall respond by phone during normal business hours or within

(30) minutes after normal business hours.

b. If the problem cannot be corrected by phone or by remote diagnostics the contractor will dispatch a technician to arrive not later than 4 business hours after the call is placed. Uptime commitment shall be 95%.

Scheduled Maintenance (Performed on a on a semi-annual basis):

a. Contractor shall perform PM service to ensure that equipment listed in the schedule performs in accordance with the manufacturer's recommendations as published in the Manufacturer’s Maintenance Manuals for the system.

b. Contractor shall provide and utilize procedures and checklists with worksheet originals indicating work performed and actual values obtained (as applicable) provided to the COR at the completion of the PM.

c. PM services shall include, but need not be limited to, the following:

• Cleaning of equipment to include filters, gearing, and other parts.

• Reviewing operating system software diagnostics to ensure that the system is operating as specified by the manufacturer’s specifications.

• Calibrating and lubricating the equipment.

• Performing remedial maintenance of non-emergent nature.

• Performing electrical safety inspection IAW NFPA 99.

• Documentation shall include detailed descriptions of the scheduled and unscheduled maintenance procedures performed, including replaced parts and prices required to maintain the equipment in accordance with manufacturer specifications. Test equipment used for calibration shall be listed on the service report by manufacturer, model number, serial number and date due calibration.

• Install Government Furnished Equipment (GFE).

Parts:

a. The Contractor will provide all replacement parts needed to keep the system in operating condition as originally designed by the manufacturer.

b. Parts may be repaired or replaced, as the contractor deems appropriate. No used parts will be used to repair this equipment.

c. Parts shall be shipped for arrival the next day by 1030 A.M.

4. Performance Period: Projected Base Year – 06-01-2023 to 05-31-2024.

5. Type of Contract: Firm-Fixed Price - Base plus 4 option years.

6. Invoicing: Contractor shall invoice per service call not for contract price in totality.

B. CONTRACT AWARD MEETING

Yes, "kick-off' meeting is required as to cover the basis of the contract.

C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES: Repairs within 24hrs pending availability of parts.

D. EVALUATED OPTIONAL FEATURES

1. Even though the contractor, at the Government's option, provides the requirements specified in this section; they are nonetheless treated with the same level of control as the mandatory specifications.

2. The contractor accepts responsibility for the equipment described in “as is” condition.

3. Failure to inspect the equipment prior to contract award shall not relieve the contractor from performance of the requirements of this contract.

E. REQUIREMENTS:

1. The Contractor shall visit the site to determine the full extent of the work. Failure to inspect the site will not constitute grounds for a claim after contract award. Charles Cole: POC 214-857-4161.

2. Disposal of materials removed under this contract at an approved disposal site in accordance with all local, state, and federal laws, regulations, and guidelines are the Contractor’s sole responsibility. The contractor shall adhere to all OSHA requirements concerning removal, transporting, and disposal of the material.

3. All fire and safety rules and regulations are applicable; NFPA and SBCCI dated 1999. The Contractor shall comply with EM385-1-1 dated 1987 for minimum safety requirements. Construction Safety Guidebook (VHA Center for Engineering & Occupational Safety & Health).

4. The Contractor shall be responsible for obtaining any licenses and permits and complying with any laws, codes, and regulations applicable to the execution of this work as required Federally and by the State of Texas.

5. The contractor shall warrantee work to be free of defects and workmanship for a period of 1 year from date of acceptance.

6. The Contracting Officer and Contracting Officer Representative reserves the right to conduct any inspections or perform any tests deemed necessary at any time during the execution of the contract to determine conformance with the requirements of theses specifications.

8. If chemicals are to be used in anywhere in this contract, the chemical must first be approved by the Safety Service here at the VA and MSDS must be obtained.

F. TRAVEL

Travel required to and from Dallas, Garland, Grand Prairie, and Bonham TX.

G. CONTRACTOR EXPERIENCE REQUIREMENTS:

1. Qualifications of Key Personnel (Standard ENGLISH mandatory language)

2. “Fully qualified” is based upon training and experience in the field.

For training, the FSE(s) has successfully completed a formalized training program for the equipment identified in this solicitation as required by the Original Equipment Manufacturer (OEM) and equal to what the manufacturer provides their own field service personnel.

3. . Contractor must provide, upon request, evidence of appropriate training of any FSE(s) providing services under terms of the contract. Subcontracting of any ensuing award of this solicitation will not be allowed without written permission of the Contracting Officer.

4. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from VA Police Service. VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions.

5. Smoking is prohibited inside all VA buildings.

H. CONFIDENTIALITY AND NONDISCLOSURE:

I. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS:

All Contractors are required to sign in with BLDG 9 (214-875-1005). Visitor’s badges will always be obtained and always worn while on VA property.
