VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under $150K

VHAPM Part 813.106 Page 1 of 1 Original Date: 08/30/17

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($150K)

Acquisition Plan Action ID: 36C249-20-AP-2708

1. Contracting Activity: Department of Veterans Affairs, VISN 9, Nashville Medical Center

626-20-3-4123-2095.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

Remove old vinyl sheeting and damaged portion of sub floor material. Then prepare and level sub floor for new vinyl sheeting. Lastly install new vinyl sheet flooring in OR Rooms 2, 5, and 7 at a cost of approximately $77,640.00

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The contractor has been used by the Nashville facility on occasion to repair /replace flooring and roofing. The contractor has worked in the OR suite at the Nashville campus, and has shown the ability needed to build containment walls and set up negative air that will be needed to contain dust and debris from entering other OR spaces. To my knowledge there is no other contractor that can perform duties necessary in the time frame needed for the repair of the OR floors.

4. Description of market research conducted and results or statement why it was not conducted:

This is an Urgent/special repair. As the exception states above this is an unusual and compelling urgency. Delaying this order by conducting market research or by sending it out for bid would not be advisable due to the patient and staff safety during surgery procedures.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief. In accordance with Federal Acquisition Regulation (FAR) 13.106-3, I have determined the pricing as provided to be both fair and reasonable. Fair and reasonableness determination for this procurement action is based off of a comparison with the Independent Government Cost Estimate

(IGCE).
