Description of Work

This agreement provides the maintenance service on facsimile machines located in CMS regional offices (plus all additional machines if/as needed to be purchased). Service will be performed by repair persons as designated by the Contractor and approved by CMS. Service includes parts, labor and travel time and repair expenses will be billed directly to the Contractor. Any fax machines listed on the attached inventory determined to be in need of replacement will be purchased by Contractor and drop shipped to the individual regional office only after CMS approves the purchase of the designated model and provides Contractor with the additional funds, not included in this contract vehicle. All maintenance of newly purchased machines would be covered under this contract for no additional cost.

Within 2 weeks of award, the Contractor must supply the list of chosen repair facilities for each regional office. CMS has the option of selecting an alternate repair facility other than the original chosen if their performance is found to be unsatisfactory. The Contractor must insure the Government is receiving best-value pricing.

Period of Performance: One year starting at date of award with 4 option years.
