In order to demonstrate your capabilities and ability to achieve the requirements of Section D, Attachment 1, Performance Work Statement, you must provide a list of three (3) recent and relevant projects that are the same or similar in scope as the RFP that you have performed within the last five (5) years. Include the following information for each contract and subcontract:

1. Contracts listed may include those entered into by other departments and agencies of the Federal Government; contracts with state and local governments; and contracts with commercial customers. An Offeror with no past performance information will receive a neutral rating, i.e. the rating will not add or detract from its Past Performance rating.
2. The offeror may provide information on problems encountered on the contract and subcontracts identified in paragraph 1 above and corrective actions taken to resolve those problems.
3. The offeror may describe any quality awards or certifications that indicate the offeror possesses a high-quality process for developing and producing the product or service required. If the award or certification is over three (3) years old, present evidence that the qualifications still apply.
4. Offerors should notify each of their private-sector (commercial) references that they may be contacted by the OPM and authorize them to provide the past performance information requested. References other than those identified by the offeror may be contacted by the Government, and the information received from them may be used in the evaluation of the offeror’s past performance.
5. The Past Performance Questionnaire in this attachment will be used by OPM to obtain past performance information from the references supplied by the Offeror and from any other sources available to OPM.
