Quality Assurance Surveillance Plan (QASP)
1.0 Introduction
This Quality Assurance Surveillance Plan (QASP) has been developed pursuant to the requirements of the Performance Work Statement (PWS) in this contract. This plan sets forth procedures and guidelines that will be used in evaluating the technical performance of the contractor.
2.0 Purpose of the QASP
The purpose of the QASP is to describe the systematic methods used to measure performance and to identify the reports required and the resources to be employed. The QASP provides a means for evaluating whether the contractor is meeting the performance standards identified in the PWS. This QASP is designed to define roles and responsibilities, identify the performance objectives, define the methodologies used to monitor and evaluate the contractor’s performance, describe quality assurance reporting, and describe the analysis of quality assurance monitoring results.
3.0 Contractor and Government Responsibilities
3.1 Contractor Responsibility
The contractor is responsible for performing inspection services pursuant to the inspection clause and for providing and maintaining the inspection forms, approved by the government, to perform the contractor’s inspection services. The contractor has represented themselves, to be responsible for meeting the prescribed quality/performance standards in the PWS.
The contractor, and not the government, is responsible for management and quality control actions to meet the terms of the contract. The role of the government is quality assurance to ensure contract standards are achieved.
In this contract the contractor’s quality control plan is the driver for product quality. The contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a “self-correcting” contract is to ensure that the quality control plan approved at the beginning of the contract provides the measures needed to lead the contractor to success.
Once the Quality Control Plan (QCP) is approved, careful application of the process and standards presented in the remainder of this document will ensure a high-quality control program.
The contractor’s QCP must include all elements of the PWS. The COR will inspect services using a checklist containing all elements of performance and are met in accordance with the QCP.
3.2 Government Responsibility
3.2.1 Contracting Officer (CO)
The CO is responsible for administering and monitoring contract compliance, contract administration, cost control, and for resolving any differences between the observations documented by COR and the contractor’s performance. The CO may delegate various contract administration duties to the COR for performance management and administrative actions such as invoice approval and issuance of Contract Discrepancy Reports may be, and normally are, delegated by the CO to the COR. The CO shall approve any revisions to the QASP processes or standards.
3.2.2 Contracting Officer Representative (COR)
The COR, is designated in writing, by the CO. The COR responsibilities will be listed on their designations and may include the following: monitoring, assessing, recording, and reporting on the technical performance of the contractor on the schedule of inspections submitted by the contractor and approved by the government, witnessing the contractor’s performance of the inspections performed by the contractor or will evaluating the contractor’s inspection results as to documented findings relative to meeting the standards for each task/subtask within the statement of work, and primary responsibility for completing quality assurance monitoring forms which will be used to document the inspection and evaluation of the contractor’s work performance.
4.0 Types of Work to be Performed/Schedule of Inspections
This contract calls for the contractor to perform all work referenced in accordance with 14.0 Performance Tasks of Performance Work Statement in this contract and for COR to perform inspections on a minimum weekly basis, where work is performed.
Describes the minimum acceptable levels of service required for each task as referenced.
Performance Work Statement Task Section Performance Standard Biobased Products 4.0 All products used will be submitted to the COR, includes updated or change of products. Major deficiencies may result in termination.
Scheduling 8.0 Schedules are followed and updated as required.
Major deficiencies may result in termination.
Contractor Quality Control Plan 11.0 Checklists are completed. Contractor has properly documented inspections, deficiencies, and corrective actions. Contractor has complied with all aspects of their Quality Control Plan.
Major deficiencies may result in termination.
Basic Cleaning Services 14.1 No more than three periodic inspection defects or 2 validated customer complaints per month.
Restroom Cleaning Services 14.2 No more than two periodic inspection defects or 2 validated customer complaints per month.
Break Room/Conference Room Cleaning Services
14.3 No more than two periodic inspection defects or
2 validated customer complaints per month.
Periodic Cleaning Services 14.4 No more than one periodic inspection defect or 2 validated customer complaints per month.
4.1 Periodic Inspections
The COR will use the attached checklist to periodically check to see if services have been performed in accordance with the PWS and the contractors QCP. The contractor or on-site manager/supervisor will sign acknowledgment of completed checklist and make any comments and corrections of deficiencies.
A copy will be provided to the contractor through the CO. Periodic inspections may be reduced or increased depending on the contractor’s performance.
4.2 Customer Complaints
The COR will validate or reject any customer complaint and document the complaint with the PWS section reference. The contractor will be allowed to comment. Re-performance, if needed, shall be performed immediately. Any validated complaint will be sent to the contractor to file with their QCP.
4.3 The Contractor’s Quality Control Plan
The COR will periodically review contractor’s approved QCP to ensure the contractor is meeting all requirements of QCP. Deficiencies will be reported to the CO for a review of the plan. Review of the QCP may be rare if level of performance is consistently acceptable.
5.0 Methods of Government Surveillance
The COR will perform periodic, independent inspections. The contractor shall be required to rework/repair or replace any work that does not meet the performance requirements of the contract.
6.0 Quality Assurance (QA) Forms and Report
The COR will use a quality assurance monitoring form to document and evaluate the contractor’s performance under the contract. This can consist of a Form, a Memo for record or an entry in a daily or weekly diary. The form when completed will document the COR’s understanding of what the contractor was supposed to do, what was actually done, and the impact or consequences of what was done. The documentation will also identify the additional steps required of the contractor if there is a nonconformance.
The COR will judge each event in accordance with the performance standards and performance requirements stated within the QASP of the contract.
