About this file
This is an Office Automation Form B from the City of Philadelphia outlining equipment installation, configuration, and maintenance requirements for a Palo Alto Firewall installation at Simons Recreation Center in Fiscal Year 2027. The form specifies that all deliveries must be inside deliveries to a loading dock, with vendors responsible for providing costs associated with installation and configuration services. The product will be delivered pre-configured, with additional configuration conducted at the City's location as needed. Vendors must provide product literature upon request and complete all installation and debris removal activities.
The form establishes mandatory warranty coverage requirements with a minimum five-year warranty period for servers and IT appliances, four-year coverage for PCs and laptops, and three-year coverage for tablets, along with original equipment manufacturer warranty periods for peripherals including printers, copiers, and multifunction devices. All warranty coverage begins following a 30 to 120-day acceptance period. On-site maintenance is required with next-day response capability within 24 hours, including all parts and labor. The City offers both three-year and five-year maintenance options, with payment due after equipment delivery and City acceptance. Instruction and training by the vendor are requested as part of the scope. The form indicates vendor acceptance of refurbished equipment as an option, though specific platform details and vendor selections are noted within the document's configuration fields.
