About this file

This document is a Disclosure Statement for Events DC, a convention and sports authority located in the District of Columbia. The form requires principals (likely potential contractors) to disclose any past or present business, familial, or personal relationships with Events DC's Board of Directors and Key Staff Members. The document provides a comprehensive list of board members including Leif Dormsjo (Chairman), Cherrie Doggett (Treasurer), and key staff such as Angie M. Gates (President and CEO), and provides a template for principals to certify the accuracy of their disclosure.

The disclosure statement is part of a procurement process for Art Curator Services, offering a mechanism for transparency and potential conflict of interest identification. Principals are instructed to either submit a signed statement indicating no relationships exist or, if relationships are present, to detail the nature of those connections. The form requires signatories to certify their disclosure to the best of their knowledge, with space provided for name, date, and title. This document appears to be a standard component of the vendor vetting and selection process for Events DC's contractual opportunities, ensuring ethical and transparent business practices.
