This contract will have a Base year and for optimal years
(vi) Description of Requirement (including a list of any attachments)
The objective of this contract is to secure audiometric testing to determine if employees are experiencing any loss of hearing associated with their jobs. The Offeror must have a mobile unit that has a testing lab inside. The unit should be able to test and teach 4-6 occupants per session. The Offeror must also have a local hearing center in Central Oregon or the ability to contract with a local vendor where we can send employees who may have missed the pre-scheduled dates for hearing tests at specified locations and times. Hearing conservation training is required also and needs to be provided at the hearing test appointment. The Occupational Safety and Health Administration (OSHA) regulations require employers to perform ongoing audiometric testing for employees who are exposed to a noise level at or above 85 decibels for 8 hours. The US Forest Service and BLM has approximately 300 employees who are, or may be, exposed to 85 decibels of noise at least one day during the year.
The contractor must be familiar with, have an understanding of and must comply with the various requirements in OSHA standards 29 CFR 1910.95. All technicians must be trained and certified by the Council for Accreditation in Occupational Hearing Conservation (CAOHC) and be under the supervision of a Licensed Certified Audiologist. All testing and calibration procedures as well as record keeping need to be in compliance with applicable OSHA and other regulatory requirements. The tests will support compliance with the applicable provisions of 29 CFR 1910.95, Occupational Noise Exposure with regard to personnel, testing procedures and facilities/equipment.
Audiometric Testing and Hearing Conservation Training
1. Inform employees being tested:
a. The effects of noise on hearing.
b. The purpose of hearing protectors, advantages and disadvantages of the various types, attenuation and instruct them on the selection, fitting, care and use.
2. Selected Offeror will provide the Forest Service/BLM with a letter for each employee who experiences a standard threshold shift (“21 day notification letter”). The selected offeror will provide the FS/BLM within two weeks of original testing of employee, a notification of employee who had a threshold shift. This letter will be used by the Forest Service/BLM to fulfill the notification requirements as listed in 29 CFR 1910.95. Conduct follow-up testing for employees who experience a standard threshold shift. This re-testing will be conducted at a date and location agreed upon by Selected Offeror and Forest Service/BLM. Selected Offeror will facilitate re-testing within 30 days of the annual test, if requested.
3. Explain test results to employees so they will understand what their hearing level is and what the test score means. Selected Offeror will provide a written letter for each employee explaining the test data, comparing test results to the baseline data, and advising them on how to protect their hearing from further loss.
4. Upon completion of testing at designated locations, Selected Offeror will submit bound hard copy reports to the Forest Service/BLM Safety Managers. The reports need to be separated by employee work locations, i.e. a report for Sisters Ranger District employees, a report for Crescent Ranger District employees, BLM Prineville District employees etc…The reports are due at the end of the testing cycle.
The reports will include the following:
• A summary report indicating how many employees were tested, a breakdown of the number of employees by hearing loss category and a roster of employees tested.
• A breakdown by employee indicating date tested and hearing loss classifications.
• A listing of employees showing a hearing test history.
• A list of employees who experienced a standard threshold shift.
• A listing of employees who show a recordable shift per the applicable OSHA standard.
• A roster of employees whose names were submitted for testing but did not report for testing.
• Documentation of hearing conservation training provided.
5. Selected Offeror will provide an electronic version of the report using MS Word or Excel formats.
6. Selected Offeror will provide consultation to Forest Service/BLM safety and health staff in regards to employee hearing tests.
The selected offeror will have a computerized registration system/program that allows all participants to schedule their own appointment and complete any paperwork needed prior to being tested. It should also allow them to cancel their testing if they have a conflict. The system must be operational before contract award. Tell us about the registration program and where it is located so we can test the viability/utility of the program.
(vii) Date(s) and Place(s) of Delivery and Acceptance/Location
