PLEASE COMPLETE THE FOLLOWING:

1. Please indicate your business size for the North American Industry Classification System (NAICS) codes listed in the attached. (Please ensure that your certifications within the System for Award Management (SAM) (https://www.sam.gov/portal/SAM) reflect this NAICS code.)

2. Are you a large business or small business?

3. Are you a manufacturer or dealer?

4. Identify source of supply if dealer;

5. Do you operate through dealer / distributors or do you prefer to deal directly with the Government?

6. Do you carry these items in your inventory?

7. Are these items quantity sensitive?

8. Are these commercial items?

9. Are these commercial off the shelf (COTS) items with commercial sales history?

10. If the answer is no to question number eight and if these are not currently in the commercial market place; will these items be available in the commercial market place in time to satisfy the gov’t delivery requirements on this project?

11. If the answer is no to question number eight; are these items modified items of a type available in the commercial market place?

12. If the answer is no to question number eight; do these items have minor modifications of a type not customarily available in the commercial market place? (For example, minor modifications mean the item needs to retain predominance of non-governmental functions or essential physical characteristics.)

13. Are these catalog / price listed items?

14. Are you able to supply DLA with a copy of your price list?

15. Does this item contain hazardous material?

16. Can you provide standard commercial (STD-COM) packaging with military standard (MIL-STD) 2073-1D markings and provide / comply with radio frequency identification (RFID)?

17. If the answer is no to question number 12, are you currently in the process of obtaining the facilities to comply with RFID?

18. What is your best delivery for these items (including first article test (FAT) requirements if necessary)?

19. The Government prefers to utilize free on board (FOB) origin; is this acceptable?

20. Do you offer quantity / price breaks for these items and if so, please provide.
