Statement of Work:

1. Task Order Title: Large Standing Purchase Order to supply NIDCD animal research facility at 5 Research Court (5RC).

2. Background: QLP has supplied NIDCD animal facility at 5RC for over ten years now. QLP has been the sole source vendor since NIH stock has been undependable and lacking quality assurance (i.e. late deliveries and expired feed was delivered on numerous occasions). QLP is the only approved distributor of the specific feed and bedding given to NIDCD’s research animals at 5RC. This approval was predetermined by the feed and bedding manufacturers based upon 5RC’s location and region. NIDCD at 5RC has been using this specific kind of feed for many years and if it were to change this variance could affect the research animals’ digestive systems and the outcome of the research projects. 5RC does not have enough space to store all of this essential feed and bedding.

3. Objectives: The goal/expectations for this PO is to maintain a reliable delivery schedule of essential feed and bedding products to NIDCD 5RC.

4. Scope: QLP will supply, store, and deliver all feed and bedding to 5RC.

5. Specific Tasks: Task 1-IC contact QLP with delivery time and quantities.

Task 2- IC inspects delivery.

Task 3- IC receives delivery and signs invoice.

Task 4- IC records inventory and completed delivery.

6. Contract Type: Standing Order

7. Place of Performance: 5 Research Court Rockville, MD 20850

8. Period of Performance: August 14, 2013 to November 6, 2013.

9. Deliverables/Delivery Schedule: Delivery will be every Wednesday.

10. Security: N/A

11. Government Furnished Equipment: N/A

12. Packaging, Packing, and Shipping Instructions: Inventory Coordinator (Maria Publico) will call QLP (Kim Blair) every Monday or Tuesday to confirm Wednesday delivery time and quantities, and then confirm parts received.

13. Inspection and Acceptance Criteria: Inventory Coordinator (IC) will inspect (i.e. expiration dates, condition of packaging, quantities) and receive deliverables at the 5RC Loading Dock, room 1A-36. IC will then sign receiving invoice and file document in designated binder, as well as record weekly deliverables and remaining balances in Excel file.
