Statement of Work Genetic Analyzer 3130xl #1205-034 and 1604-019

Maintenance Contract Renewal

Background

The capillary genetic analyzer ABI 3130xl performs capillary electrophoresis and detection of DNA fragments for sequencing and fragment analysis.

The separation of amplified and labeled microsatellite fragments by electrophoresis on the genetic analyzer ABI 3130xl is used as an internal quality assessment procedure for confirming the identity and quality of progeny samples (DNA and Blood) generated from the original whole blood received in OMGS.

In order to allow OMGS to carry out its essential functions, the 3130XL DNA Analysis Machines must be covered by the technical services department of Life Technologies. This coverage is essential for prompt and accurate resolution of technical issues or malfunction, as well as appropriate software/hardware upgrades. We therefore are required to purchase the service contract to comply with federal law and to provide consistent and high quality health care results to those who submit specimens to our laboratory.

Scope Contractor (Life Technologies (LT)) is to provide all labor, material and equipment to provide preventive maintenance for government‐owned 3030xl Genetic Analyzer for the period of 2015‐2016, with renewal for the 2016 ‐2017(base year), 2017‐2018(option year1) ,2018‐2019(option year 2) and 2019-2020(option year 3). All maintenance services shall be performed in accordance with the manufacturer’s standard commercial maintenance practices.

Preventive Maintenance Planned maintenance visits are intended to minimize the need for service calls. LT may perform more than the number of planned maintenance visits indicated in LT's quotation, at LT's discretion. Customer will not be charged for any planned maintenance visits made during the plan period, except for visits that are in addition to the number indicated in LT's quotation that are requested by customer.

Scope of Work

1. Parts, labor and travel for remedial repair.

2. One planned maintenance visit at no additional charge.

3. Guaranteed priority response time of 2 business days after receipt of a service call for instruments. If LT fails to arrive at the instrument location within 2 business days for reasons other than customer's failure to provide access to LT or causes beyond the reasonable control of LT, LT will provide customer a service plan renewal credit in an amount equivalent to one day's pro‐rated charge for each day LT's response is late.

4. Priority telephone and email access to instrument technical support.

5. Telephone and email access to application technical support.

6. Remote Monitoring and Diagnostic Service, which provides for notification to customer of instrument failures or errors that are reported by AB's Remote Monitoring software.

Payment Terms of payment are net 30 days from invoice date for each of the three service contracts.
