STATEMENT OF WORK

BOILER OPERATIONS AND MAINTENANCE

NATIONAL INSTITUTES OF HEALTH, OFFICE OF RESEARCH FACILITIES

ROCKY MOUNTAIN LABORATORIES, HAMILTON, MT 59840

DATED: 12 APRIL 2017

C.1. STATEMENT OF WORK

Independently and not as an agent of the Government, the Contractor shall be required to furnish all the necessary services, qualified personnel, material, equipment, and facilities, not otherwise provided by the Government, as needed to perform the Statement of Work.

C.2. GENERAL INTENTION. Contractor shall provide operation, maintenance and repair services for the Central Boiler Plant and distribution systems at Rocky Mountain Laboratories (RML) by means of a firm fixed-price. The central boiler plant shall be manned 24 hours per day, 7 days a week, for the life of the contract.

C.3. GENERAL REQUIREMENTS. The Contractor shall furnish licensed boiler operators, mechanics, supervision, tools, materials, and equipment specified herein and as needed;

incidental engineering; transportation; and management necessary to operate, maintain, and repair the central boiler plant and associated distribution systems, facilities, and equipment in accordance with the requirements specified herein. (Exhibits E-C1 and E-C2 describe the facilities and equipment to be maintained in this contract.) Work includes the performance of service work, preventive maintenance, troubleshooting and repair of equipment, and other services as described herein.

C.4. DEFINITIONS - TECHNICAL. As used throughout this contract, the following terms shall have the meaning set forth below. Additional definitions are in the "DEFINITIONS" clause in Section I.

a. Where "as shown", "as indicated", "as detailed", or words of similar import are used, it shall be understood that reference is made to this document and the exhibits and attachments accompanying this document unless stated otherwise.

b. Where "as directed", "as required", "as permitted", "approval", "acceptance" or words of similar import are used, it shall be understood that direction, requirement, permission, approval, or acceptance of the CO is intended unless stated otherwise.

c. Contractor. The term Contractor as used herein refers to both the prime Contractor and any subcontractors. The prime Contractor shall be responsible for insuring that his/her subcontractors comply with the provisions of this contract.

d. Maintenance. Work includes both preventive, repair and routine services.

e. Facility. An establishment, structure, or assembly of units of equipment designated for a specific function.

f. Contracting Officer Representative (COR). The person(s) whom the Contracting Officer will designate by name to conduct liaison between the Contractor and the Contracting Office on matters pertinent to this contract and to be his/her authorized representative. An Assistant or Deputy COR may also be assigned.

g. Quality Assurance (QA). A method used by the Government to provide some measure of control over the quality of purchased goods and services received.

h. Quality Assurance Evaluator (QAE). The Government representative that is responsible for the daily monitoring of Contractor performance. For this contract, the Contracting Officer Representative (COR) or his/her designee will perform the function as the QAE.

i. Quality Control (QC). A method used by the Contractor to control the quality of goods and services produced.

j. Regular Working Hours. The Government's regular working hours are from 8:00 AM to 5:00 PM, Mondays through Fridays except (a) Federal Holidays and (b) other days specifically designated by the Government.

k. Repair. Repair integral to this contract is the restoration of a piece of equipment, a system, or a facility to such condition that it may be effectively utilized for its designated purposes. Repair may be overhaul, reprocessing, or replacement of constituent parts or materials that have deteriorated by action of the elements or usage and have not been corrected through maintenance.

l. Response Time. Response time is defined as the time allowed the Contractor after initial notification of a work requirement or repair to be physically on the premises at the work site with appropriate tools, equipment, and materials, ready to perform the work required. The Contractor shall generally have two (2) working days to accomplish the work requirement or repair.

C.5. BOILER PLANT OPERATION, MAINTENANCE, AND REPAIR. The Contractor shall be responsible for the effective and efficient operation, maintenance, and repair of the central boiler system 24 hours per day, 7 days per week. This system includes the boilers, plant, and related equipment including fuel storage and handling, propane system, water treatment equipment, associated pumps, components, controls, the steam distribution system including steam lines, condensate return pumps, and related equipment as identified in the Exhibit E-C1 and E-C2. When work requires an outside certified mechanic/welder, the Contractor shall arrange for those services, notify the COR and supply the proper license/certificates.

a. Plant Operation. Operation of the central boiler plant includes the start-up and shutdown of boilers, associated boiler equipment, operator maintenance, troubleshooting, repair and inspection, and the efficient and economical production of steam to assure its availability to the Government at the lowest possible cost. This work also includes record keeping of operations and conditions; analysis of records to correct non-optimal practices; water treatment; monitoring warranties; testing operations and capabilities of the central boiler plant; periodic operation and inspection of idle equipment; touch-up painting; and cleaning, preservation, lubrication, and adjustment of plant equipment.

(1) Operation Procedures. The Contractor shall maintain complete control of the boiler plant operation by preparation and adherence to written operating procedures. The operating procedures shall be prepared by the Contractor in accordance with the following, in order of precedence: (1) manufacturer's instructions, (2) industry standards and national codes. As a minimum the procedures shall include:

(a) Plant systems (boilers, etc.) operating instructions including a detailed description in correct sequence of the observations and adjustments to be made, the minimum frequency of the observations and adjustments, and who shall perform them.

(b) Plant equipment operating schedule.

(c) Boiler water treatment plan.

(d) Systems maintenance, inspection and repair.

(e) Emergency procedures for steam production/distribution and disruptions

(f) Safety/accident procedures.

(g) Boiler overhaul.

(2) Procedures. These procedures shall be submitted to the Contracting Officer through the COR for approval within fifteen days after contract award. The approved procedures shall be instituted upon approval by the CO. The Contractor shall update and revise all plant operation, distribution, and maintenance procedures on a continuing basis as required by physical or operational changes and re-submitted to the COR upon any changes made for re-approval by the

CO.

(3) Steam Generation. The central boiler system shall be operated continuously to meet industrial/production and steam demand requirements. The steam pressure shall be maintained at a minimum of 100 PSI and 338 degrees F.

b. Fuel Oil. The Government shall furnish fuel oil for boiler backup operation. The Contractor shall report monthly in writing to the CO through the COR the amount of usable fuel available for the plant. The Contractor shall maintain an accurate record of the amount of fuel received in each delivery. Tank measurements shall be taken and recorded before and after fuel delivery to verify quantities received. The Contractor shall initiate fuel orders as directed by the CO through the COR, receive fuel from tanker trucks, transfer to and among storage tanks, and make all operational fuel transfers. The Contractor shall maintain and repair all fuel oil handling equipment including storage tanks, pumps, piping, and heaters, and shall comply with all federal regulations pertaining to fuel operations. Boilers shall be tested and operated on fuel oil monthly.

Schedule for run hours per Boiler per month will be provided by the CO through the COR at the start of the contract.

c. Water Testing and Treatment. The Contractor shall daily collect feed-water, boiler water, and condensate samples from each operating boiler for testing. The Contractor shall perform the necessary tests on hardness, phosphate, sulfite, causticity (alkalinity as OH), pH, and total dissolved solids (conductivity). Results will be compared to applicable manufacturer requirements or local requirements and water shall be treated as necessary. The Government shall provide all water treatment chemicals required for plant operations. Boiler water shall be maintained within the following limits: Phosphate 40-60, Sulfite 20-40, pH 7.6 to 9.0, hardness

1.5 parts per million total hardness, causticity (alkalinity as OH) 1500 maximum, and total dissolved solids 3700 max, Boiler PH 10-12. Monthly, a water analysis report shall be forwarded to the CO through the COR. Changes to the approved water treatment must be in the operation procedures plan and must be approved by the CO through the COR. Contractor shall submit a Government request for purchase order form to the CO through the COR for water treatment chemicals and testing chemicals.

d. Operation Efficiency Standards. The Contractor shall be responsible for meeting the present operating standard of the boiler plant. This shall be based on present efficiency of the boilers.

Boiler efficiency shall be calculated by the ASME input/output method. The procedure for determining the efficiency used in the calculation shall be consistent throughout the term of the contract. The Contract shall maintain a minimum acceptable boiler efficiency level as recommended by manufacture. These standards are subject to revision based on changes in future operational conditions of the boiler plant. For example, improvements in the boiler plant that are accomplished at Government expense may require an increase in minimum acceptable efficiency.

e. Plant Reports and Logs. The Contractor shall provide and maintain operating logs on all operating equipment which will note operator checks, repairs and adjustments, and a record file noting normal or abnormal operating conditions, deficiencies or malfunctions, and corrective action taken. All recording charts and required logs shall be filed chronologically and kept in the location(s) as designed by the CO through the COR. A copy of the logs shall be submitted to the CO or COR on request. The types of reports and specific data required for logs and reports are indicated in Exhibit E-C6.

(1) Boiler Performance Report

(a) A boiler performance report will be prepared for each operating boiler on a weekly basis. The boiler performance report will include:

1. A plot of boiler combustion efficiency versus boiler load.

(2) A plot of the temperature difference between the boiler water and exit gas temperature versus load for steam boilers.

(c) Plots of combustion efficiency and temperature difference versus load should be reported for the full range of operating loads each week.

(d) The contractor will perform on a quarterly basis boiler stack analysis to include combustion efficiency. The contractor will measure stack emissions on all four boilers under both gas fired and oil fired fuels to include the following measurements:

1. Efficiency % Percent

2. Oxygen % Percent

3. CO PPM (parts per million)

4. CO2 % Percent

5. Excess Air % Percent

6. Draft inches of water column

7. NOX PPM (parts per million)

(e.) Water and flue gas temperature difference is an indicator of the cleanliness of the boiler watersides and firesides. A plot of this temperature difference shall be prepared by the Contractor after the boiler watersides and firesides have been cleaned. If the temperature difference rises by more than 80 degrees at several load points, deposits have probably formed and should be removed.

As a check, combustion efficiency should drop as the temperature difference rises.

f. Operational Emergencies. Operational emergencies such as ruptured mains, loss of boilers, etc., which reduce boiler pressure below 80 percent of normal operating pressure for a period extending beyond 5 minutes or which result in a change in the plant's reliability or capacity shall be reported within five (5) minutes of occurrence to the CO through the COR and RML shop personnel. The Contractor will identify the probable cause for the reduction and the estimated time to restore full steam capacity.

g. Plant Maintenance and Repair. All routine maintenance and repair of the central boiler plant is included in this contract and shall include but is not limited to: steam heating sources; fuel storage and handling, feed-water, condensing, flue gas, and air system equipment; miscellaneous pumps and plant instrumentation; electrical equipment and components; as well as associated appurtenances, necessary to generate and deliver steam to the distribution system external to the plant. During boiler safety inspections and/or certifications conducted by the Government or contracted inspectors, the Contractor shall provide a qualified operator to standby. Plant maintenance and repair shall be performed in accordance with the approved operating procedures as defined previously in this section. All major repairs and equipment replacement shall be limited to the boiler and any components up to the major steam header. Any repair or equipment replacement from the steam header outward, the contractor shall contact and notify the CO through the COR and RML shop personnel for repair.

h. Calibration. To maximize the newly installed O2 trim control system along with the enhanced variable speed drive combustion system; an ongoing calibrations routing is required to achieve the maximum boiler operating efficiency. This calibrations routine shall consist of the following:

1. The contractor shall provide quarterly calibrations of the attached instrument list (Exhibit E-C8).

2. The calibrations will be accomplished by the simulation of the process control signal at the transmitter. The contractor will simultaneously measure the output of the transmitter to the control or receiving control room device; this will constitute the loop check. The receiving device will be adjusted to accurately measure these inputs, the accuracy of which will be consistent with the manufacturers stated accuracies. The calibrations will consist of three discrete points to verify not only accuracy but linearity as well; these cardinal points will be at 25, 50 and 75% of scale. (8, 12, and 16ma.)

3. In addition to the specific calibrations described above, the contractor shall calibrate all pressure, temperature and instrument gauges annually and attach a self-adhesive seal on each instrument and gauge certifying that it has been calibrated. The date of the calibration and the initials of the person performing the calibration shall be noted on the sticker. A copy of calibrations shall be submitted to the CO through the COR annually. The gauges/instruments shall be calibrated using a standard dead weight tester or a calibrated master gauge, all test equipment will be certified annually and traceable to NIST. In addition to the normal annual calibration, the Contractor shall recalibrate any gauge or instrument which is suspected of being in error.

i. Boiler Overhaul

1. Maintenance work on boilers and direct support auxiliary equipment which cannot be performed while the boiler is in operation shall be accomplished as an overhaul item. Each boiler shall be overhauled at least once per year. The overhaul shall be performed in accordance with manufacturer recommendations and Section VII of the ASME Boiler and Pressure Vessel Code. All overhaul work shall be fully warranted against defects due to material or workmanship for a period of 180 days.

Historical information for past overhauls and future overhauls is listed in Exhibit E- C7.

2. The Contractor shall prepare a boiler overhaul schedule indicating the time and duration of the shutdown, which shall be submitted prior of scheduled work to the CO through the COR for approval. Within 10 days after completion of each overhaul, the Contractor shall prepare and submit a detailed report to the CO through the COR of the findings and work accomplished. Work affecting the structural or pressure integrity of the boiler shall be performed only when directed by the CO through the COR and in accordance with written procedures approved by a certified testing agency, these inspections will be conducted in accordance with the ASME boiler inspection requirements. Completed work will be inspected prior to returning the affected unit to operation.

3. In conjunction with the overhaul, the Contractor shall perform an operational test of low water and other controls and alarms. The Contractor shall schedule the work with the CO through the COR so that the Inspector may perform such inspections and witness tests that are required while the unit is open and before it is returned to service.

4. The contractor will make all arrangements for the boiler inspector so they can perform annual inspections during overhaul.

5. At no time, shall the plant have more than two boilers down for service and or inspection.

j. Certification

(1) The Contractor shall clean and prepare the steam system boilers and unfired pressure vessels in the system for certification by the Montana State Boiler Inspector. The Contractor shall operate the boilers during the certification as approved by the Boiler Inspector. Hydrostatic pressure testing for certification shall be performed by the Contractor.

(2) All boilers and unfired pressure vessels will be inspected and certified by the

Montana State Boiler Inspector in accordance with all applicable federal, state and local regulations. Units for which certification has been withheld will not be operated without the written concurrence of the CO through the COR. Boiler inspection safety certificates shall be void immediately on the discovery of a safety deficiency regardless of the expiration date on the certificate. The certificate will again be valid only after the deficiency has been corrected by the Contractor and checked by the Government and or inspector.

C.6. STEAM DISTRIBUTION SYSTEM. The Contractor shall maintain and repair the steam distribution system identified in Exhibit E-C2, which includes approximately 150 linear feet of steam distribution lines and supporting equipment and components. The steam distribution system originates at each Central Boiler Plant and extends approximately 75 linear feet to the main steam header. This system includes pipe anchors, valves, traps, pipe insulation, structural supports, steam piping, condensate return piping, and other related items. Seasonal start-ups and shut-downs of the steam distribution system shall be performed in accordance with the manufacturer’s instructions/procedures and/or NAVFAC publication MO-209 (http://www.efdlant.navfac.navy.mil/search/search_15.htm).

a. Preventive Maintenance (PM) Report. The Contractor shall submit a typewritten PM report for all PM inspections within 10 working days to the CO through the COR after the inspection. The frequency of inspection shall be in accordance with manufacturer’s recommended PM requirements which will be provided upon contract award. The report shall identify all items inspected, condition, deficiencies, and corrective work required and including scheduled accomplishment dates.

b. Preventive Maintenance (PM) Records. The Contractor shall maintain PM records for the steam distribution system. The PM records shall reflect PM performed including scheduled and accomplished dates. The Contractor shall update the PM records monthly within 10 days after the last day of the month. PM records are the property of the Government and shall be turned over to the CO through the COR within 10 days of the termination or close out of the contract. The Government shall be allowed to review any PM records upon request.

C.7. MINOR MAINTENANCE AND REPAIR WORK. Minor work is defined as maintenance and repair work requirements within the facility which are not described elsewhere in the statement of work, but are required to be performed under this contract. This work requires the Contractor to assist RML Shop Personnel in the maintenance and repair of equipment within the facility not otherwise listed.

C.8. PERSONNEL REQUIREMENTS

a. The services provided under this contract are considered essential to the National Institutes of Health and shall be subject to the Service Contract Act. As such the contractor, shall provide a minimum of five (5) fulltime and as many other fulltime and part time, trained, qualified and licensed personnel for the performance of this contract:

(1) The contractor shall provide as a minimum one (1) project manager or supervisor with at least five (5) years of experience and who holds a valid Montana State Second-Class Engineering license issued through the Montana State Department of Commerce. (See website in item 2.

for definition of second-class license.) The Supervisor will work as a shift operator also.

b. The Government shall be provided names and numbers for all employees and responsible company representatives that can be reached at all times, should problems arise in the operation of the plant.

(2) Each shift operator(s) shall be fully trained and qualified to perform the duties listed in this statement of work and hold the appropriate boiler license per the Department of Labor and Industry, Business Standards Division, 2001 Montana Code, Title 50, Chapter 74, Boilers and Steam Engines (http://www.discoveringmontana.com/dli/bsd/license/bsd_boards/boi_ prg/statutes.htm).

(3) The contractor shall provide relief operators qualified (in accordance with C.8. a.1. and 2. above) as needed, to cover sickness, vacation, and all other absences which may occur.

b. To ensure that each shift operator(s) is fully trained and qualified to perform all the duties listed in this statement of work, all personnel assigned/working under this contract shall work a rotational work schedule (day shift, swing shift, mid shift). This will ensure all operators are fully trained and qualified to work any shift and can perform all the duties listed in the statement of work. All personnel assigned to this contract should be scheduled for a maximum shift of eight (8) hours. When overtime is required, maximum overtime per 24-hour period per operator shall not exceed four (4) hours. There shall be a minimum of eight (8) hours between consecutive shift assignments. Work/shift schedules shall be submitted to the CO through the COR. All state and federal laws pertaining to hours of work shall be followed in the assignment of work schedules.

c. Every supervisor and operator shall be trained and qualified to operate, repair and maintain all the systems and equipment assigned to the contractor. All operators shall be assigned to all operation and maintenance phases of the Contractor’s assigned areas of responsibility and shall be assigned on a predetermined and approved schedule by the supervisor. Each operator must have a minimum of two weeks supervised training on the specific equipment and operating procedures in the NIH/RML boiler plant.

d. Supervisory personnel shall have had at least five (5) year’s supervisory experience at a level equivalent to that of this position. Resumes for proposed supervisory personnel must be provided and approved by the Contracting Officer prior to the start of the contract.

e. All employees assigned to this contract shall be drug free and physically able to do their assigned work and shall be free of any communicable disease. Each employee shall be given a physical examination and pre-employment drug test without cost to the government, and a letter which will be provided to the CO through the COR signed by a Board-Certified Doctor indicating that the employee has passed the exam, drug test and is free of communicable diseases before the employee is assigned to duty. Under no circumstances shall the contractor assign an employee having a record involving alcohol or drug abuse to work at the Rocky Mountain Laboratories. All employees must pass an initial drug screening test and will be subject to random drug testing at the discretion of the Government paid for by the contractor. Any employee working under the influence of drugs or alcohol will have their access revoked in accordance with the FAR Clause 52.223-6.

f. All employees must pass a Public Trust Level 5b background investigation

C.9. GOVERNMENT FURNISHED PROPERTY AND SERVICES

a. Government Furnished Facilities:

(1) The Government will furnish or make available to the Contractor the facilities described in Exhibit E-C3. The Contractor shall assume responsibility and accountability of such facilities provided for his/her use and shall take adequate precautions to prevent fire hazards, odors and vermin. Janitorial services will be supplied by the Government to clean the restroom, office area and the exterior side of windows. Cleaning of the boiler room space will be the responsibility of the Contractor and will be performed on a weekly basis at the minimum. These services will include the following: sweep and mop floors; clean up spills; wipe excess oil off equipment;

clean and; dispose of refuse in appropriate receptacles. The contractor shall on quarterly basis clean the interior side of windows. The Contractor shall obtain written approval from the Contracting Officer through the COR prior to making any modifications or alterations to the facilities. Any such modifications or alterations approved by the Government will be made at the expense of the Contractor. At the completion of the contract, all facilities shall be returned to the Government in the same condition as received, except for reasonable wear and tear. The Contractor shall be held responsible for the cost of any repairs caused by negligence or abuse on his/her part, or on the part of his/her employees.

b. Government Furnished Equipment:

(1) The Government will provide the Contractor the use of existing and available Government owned tools and equipment for the performance of the contract.

(a) Government furnished tools and equipment is listed in Exhibit E-C4. The Contractor shall provide periodic servicing, maintenance and repair of the equipment listed, and the total or partial breakdown or failure of the Government furnished equipment shall not relieve the Contractor of the requirement to fully perform the work of the contract. Upon completion or termination of the contract, all Government owned equipment shall be returned to the Government in the same condition as received, except for normal wear and tear. Equipment listed at E-C4 that is necessary to fulfill the requirements of the contract which becomes worn out due to normal wear and tear shall be returned to the Government and will be replaced. The Contractor shall be responsible for the cost of any repairs or replacement caused by negligence or abuse by the Contractor or his/her employees.

(b) The Contractor and the COR shall conduct a joint inventory before commencing work under this contract to determine the exact number and serviceability of Government furnished equipment. The Contractor shall then certify the findings of this inventory, assume accounting responsibility, and subsequently report inventory discrepancies to the CO through the COR.

Government furnished equipment shall not be removed from the NIH/RML Campus unless approved by the Contracting Officer through the Project Officer in writing.

c. Government Furnished Material:

(1) The Government will furnish the material described in Exhibit E-C5 to the Contractor on an ongoing basis for use only on this contract. The use of Government furnished material for any other purpose is prohibited. The Contractor and the COR shall conduct a joint inventory before commencing work under this contract to determine the exact number and serviceability of

Government furnished materials. The Contractor shall then certify the findings of this inventory, assume accounting responsibility for all materials supplied, and shall provide documentation supporting issue/use of such material. The Contractor shall be responsible for tracking Government furnished materials and notify the CO through the COR when new materials are required, except as otherwise specified herein. Upon completion or termination of this contract a second joint inventory shall be conducted, if necessary, of all unused Government furnished materials. The Contractor shall be held liable for all materials which cannot be accounted for by issue/use documentation.

(a) Experience has shown that selected items of long lead time parts and materials must be stocked to insure repair of critical equipment in the event of failure. A list of these long lead time parts and materials and minimum stocking levels are contained in Part B of Exhibit E-C5.

The Government shall provide the Contractor all items in at least the minimum quantities listed in Part A and B of Exhibit E-C5. The Contractor shall maintain at least the minimum quantity of all the items specified and notify the CO through the COR if these items fall below the minimums specified. These items will be used by the Contractor in the maintenance and repair of the facilities/systems only as follows:

1. Long lead time parts and materials shall be used on the systems, facilities, or Government Furnished Equipment with which they are associated.

2. The Contractor shall notify the CO through the COR within two working days of any replacement of a long lead time part or material which causes the total quantity on hand to fall below the minimum specified level.

3. Upon completion or termination of the contract, all long lead time parts and materials shall be returned to the Government in the minimum specified quantities.

d. Availability of Utilities. The Government will furnish the following utility services at existing outlets, for use in those facilities provided by the Government and as may be required for the work to be performed under the contract: electricity, natural gas, fresh water, sewage service, and refuse collection. Information concerning the location of existing outlets may be obtained from the COR.

(1) Utilities specified above will be furnished at no cost to the Contractor.

(2) Commercial telephone service shall be provided by the Government at no cost to the Contractor. All long-distance toll charges (except for calls for work-related emergencies) shall be paid for by the Contractor.

C.10. CONTRACTOR FURNISHED ITEMS. Except for the items listed in Exhibit E-C4 the Contractor shall provide all management, supervision, tools, equipment, materials, and licensed boiler operators to perform the requirements of this contract. The Contractor will also be required to provide a copy of each operator’s annual boiler license described in C.8. (a)(2) To the CO through the COR prior to start of the contract and each option period. The contractor will provide and furnish its own computer, network system and provide email address’ of company personnel to the COR for the CO’s use. Network connection must be coordinated and approved by the Contracting Officer.

C.11. MANAGEMENT. The Contractor shall manage the total work effort associated with the operations, maintenance, repair, and all other services required herein to assure fully adequate and timely completion of these services. Included in this function will be a full range of management duties including, but not limited to, planning, scheduling, cost accounting, report preparation, establishing and maintaining records and inventories, and quality control. The Contractor shall provide an adequate staff of personnel with the necessary management expertise to assure the performance of the work in accordance with sound and efficient management practices.

a. Work Control. The Contractor shall implement all necessary work control procedures to ensure timely accomplishment of work requirements, as well as to permit tracking of work in progress. The Contractor is responsible for planning and scheduling work to assure material, labor, and equipment are available to complete work requirements within the specified time limits and in conformance with the quality standards established herein. Scheduling and status reports shall be provided when requested by the CO or COR.

b. Scheduling. The Contractor shall schedule and arrange work to cause the least interference with the normal occurrence of Government business and mission. In those cases, where some interference may be essentially unavoidable, the Contractor shall make every effort to minimize the impact of the interference, inconvenience, equipment downtime, interrupted service, customer discomfort, etc.

c. Records and Reports. The contractor shall maintain management, operation, and maintenance records and prepare management, operation, and maintenance reports as set forth in Exhibit E- C6, "RECORDS AND REPORTING REQUIREMENTS". This Attachment specifies which records and reports are the contractor's responsibility and those for which the contractor must provide data to be used by the Government Representative in preparation of Government reports.

The contractor will be responsible for developing a standard format for all reports. All records and copies of reports shall be turned over to the CO through the COR within five days after contract completion.

C.12. CONTINUITY OF SERVICES. To ensure continuity of essential services, the successful offeror shall be prepared to fully commence work on the start date of this contract.

C.13. HISTORICAL DATA. Exhibit E-C7 contains some historical data taken from the activity's records. The information provided is not, however, by itself, considered sufficiently accurate for proposal purposes.

C.14. FACILITY HISTORY FILES. A facility history file for the facility shall be maintained by the Contractor. Each file shall contain a copy of all work accomplished by the Contractor in the facility on equipment/components, and preventive maintenance inspection reports. A copy of all work completed by the Contractor and/or issued by the CO or COR shall be included in the file along with a list of Government owned equipment. The Contractor shall maintain all warranty information and inventories complete with serial numbers in this file. The Government shall require access to these files and they shall be available for periodic review. All documents shall be filed within ten days of the completed transaction. These files shall become the property of the Government at the termination of this contract.

C.15. REFERENCES AND TECHNICAL DOCUMENTS. Publications and other pertinent documents referenced in this specification are indicated in Exhibit E-H1. If available, government publications will be furnished by the Government at the start of the contract. All available boiler system drawings, records, manufacturer's equipment manuals, equipment history files, and other available pertinent data shall be turned over to the Contractor. The Contractor shall update these items to reflect all changes implemented during the contract period. Upon completion of the contract, the Contractor shall return all items in the same condition as received less normal wear and tear.

C.16. TRAINING. During the term of the contract, the Government may make changes/alterations to the present Central Boiler Plant or equipment. Training for any new equipment may be required for all existing contractor personnel. Training will be covered under a separate contract line item in the schedule or costs/pricing of the contact.

GOVERNMENT FURNISHED MATERIAL

The following material will be made available for use by the Contractor, as specified in clause C.4.

PART A - RECURRING

All maintenance, repair and operational types of material will be provided by the Government.

The Contractor will be required to submit a list of needed materials to the COR or his/her designated representative. Recurring types of materials may include but not limited to: oils, grease, rags, water chemicals, nuts, bolts, screws, various types of pipe fittings, etc. The Contractor shall submit a purchase request form to the CO through the COR for recurring material and replacement parts.

PART B - INSURANCE ITEMS

All insurance parts and materials will be maintained and provided by the Government. When repair parts are required by the Contractor, the Contractor must notify the Government Representative for issue of the item. A list of insurance items will be made available after award of contract.

The following reports should be submitted to CO through the COR in accordance with the statement of work (All Reports are to be submitted to the CO through the COR monthly unless indicated otherwise)

Charts include stack temperature, steam flow, steam pressure, and makeup water.

8. Plant Boiler Maintenance and Repair Report—Contractor will maintain existing plant boiler and repair records that are currently on file.

9. Natural Gas Usage Report—Contractor will maintain daily natural gas usage records.
