ACIO Project Logic Model The logic model is a visual representation of a program that illustrates how planned activities are linked to program results. Logic models come in many different formats, but they all present the shared perspective of an “if…then” statement: “If we obtain the necessary resources and conduct certain activities, we will achieve our desired outcomes.” Below is a logic model template that includes an example of typical health information outreach project inputs, activities, outputs, outcomes and impact.
· Inputs are the resources you need for the outreach project, including people, time, money, materials, equipment, and technology.
· Activities include what you do—conduct training sessions, provide services—and who is reached—participants, agencies, community-based organizations.
· Outputs are direct products of program activities-training sessions created or print materials, videos, social media postings created.
· Outcomes are the results or benefits of your project, including short-term outcomes such as changes in knowledge, intermediate outcomes such as changes in behavior, and long-term outcomes such as changes in individuals’ health or medical access, social conditions or population/community health (Impact).
· Impact the intended change in individuals’ health or medical access, social conditions or population/community health that occur as a result of program activities within 7-10 years.
How to Complete the ACIO Project Logic Model:
1. Identify the overall project mission/purpose. (Goal should be the same as presented in the quotation) (1-2 goals per project)
2. Define Objectives. Objectives should be the same and presented in the quotation. An objective should also be “SMAART” Specific, Measurable, Action-oriented, Achievable, Realistic and Time-bound
3. Determine what you have to have in place (inputs) to make each key activity possible. Determine the people (stakeholder(s)) who will provide each identified input. What resources/staff do we need to accomplish our goals?
4. Identify the key activities involved in the project that will help achieve the objectives/outcomes. What activities will accomplish the outcomes? Some examples: resource trainings, mobile App development, SMS and/or social media campaigns.
5. Determine the output(s) of each activity.
6. Decide on the desired short, mid- and long- term effects (outcomes) that will result from successful activities.
a. Short-term outcomes: What change do you expect to occur either immediately or in the near future?
b. Intermediate outcomes: What change do you want to occur after that?
c. Long-term outcomes: What change do you hope will occur over time?
7. Determine the long term effects (impact) on the target population, community and/or larger population. What change will occur in your community as a result of your program?
8. The Reality Check: My organization’s assumptions about the project, target community or available resources correct? What are the outside challenges and barriers that may impact your ability to conduct the project?

Example of Completed Logic Model:
Goal: To identify the health information needs of African American MSM, ages 18-28, and provide access to NLM HIV/AIDS information resources through digital and social media.

Staff Funding Experience Curriculum Website Partnerships
1. Develop and provide digital messages via Facebook, Instagram, Vine and Twitter social media channels
2. Develop and implement an online training curriculum to increase accessibility and utilization of NLMs online HIV/AIDS information database
3. Host meet-ups at our facility to get consumers excited about accessing and utilizing NLM online HIV/AIDS information databases; consumers will complete online training.
· 50 digital messages created
· Training Curriculum completed and posted to our website
· Quarterly (4) meet ups planned and implemented
· Short-Term Increase awareness, knowledge and use of NLM’s online HIV/AIDS information database (AIDSinfo, NLM HIV/AIDS Portal, ClinicalTrials.gov, MedlinePlus) among consumers
· Intermediate Train/assist consumers to access and utilize HIV/AIDS information
· Long-term: see Impact
· Consumers are able to identify and use appropriate prevention behaviors as a result of accessing and learning about this information via NLM HIV/AIDS databases.
· Decrease the incidence of HIV/AIDS infections among target population by improving access to prevention information.
Assumptions (Should be confirmed before beginning the program)
· Organization has long standing ties to local community and the health needs of an urban, young, MSM population.
· Organization has done successful social media campaigns in the past with this target population.
· Organization has a facility with regular activities for target population already in place.
External Factors (Should be identified before beginning the program)
• Having a central facility makes the target population easier to reach
• (-) Fear and stigma about HIV/AIDS in the community.
