STATEMENT OF WORK

Background

The mission of the National Institutes of Health (NIH) is to uncover new knowledge that will lead to better health for everyone. The NIH accomplishes that mission by conducting research in its own laboratories; supporting the research of non-Federal scientists in universities, medical schools, hospitals, and research institutions throughout the country and abroad; helping in the training of research investigators; and fostering communication of biomedical information.

The NIH Clinical Center (CC) is a 200-bed federally funded, biomedical research hospital located on the

NIH campus in Bethesda, Maryland. The Clinical Center is the delivery setting for all NIH intramural clinical research protocols. The hospital complex consists of two main facilities that are connected to each other by “bridges”. The original 14-story building is 2.5 million square feet and continues to house the clinical laboratories, imaging sciences, surgical suites, and several outpatient clinics. The new 870,000 square foot facility houses inpatient units and day hospitals. The Clinical Center accounts for about half of all NIH-funded clinical research beds in the United States and accommodates about 7,000 inpatient and

70,000 outpatient visits a year. Patients are admitted to the NIH Clinical Center from all over the world for the sole purpose of participating in a clinical research protocol.

The Department of Clinical Research Informatics (DCRI) is responsible for the network infrastructure that supports the technology of all mainframe and desktop computers, and the management and development of applications and databases.

Objective

The objective of this Statement of Work is to provide the Clinical Center, Department of Clinical

Research Informatics with 4D Database technical support maintenance. The Clinical Center utilizes 4D software for the applications supporting several clinical functions. Some of these applications are critical to the hospitals primary function of providing safe and efficient patient care.

Past Performance.

The Past Performance submission must include past performance for delivery of similar items and services.

PROVISIONS INCORPORATED BY REFERENCE

52.212-2 - Evaluation-Commercial Items (Oct 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

Factor 1: Technical Capability

Factor 2: Price

Factor 3: Past Performance

Technical and past performance, when combined, are More Important than Price.

(b) The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the options(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

(End of provision)

Addendum FAR 52.212-2 Evaluation - Commercial Items

Evaluation Narrative Process Basis for Award

It is in the best interest of the Government to evaluate proposals and select the successful offeror using the best value approach. This process permits tradeoffs between non-cost factors and cost/price and allows the Government to accept other than the lowest priced proposal or other than the highest technically rated proposal to achieve a best value contract award based on established evaluation criteria. Inherent in this process is the necessity to make tradeoffs considering the non-cost strengths and weaknesses, risks, and the cost (or price) offered in each proposal. Technical is the most important factor and more important than all of the remaining factors combined. Technical is significantly more important than Past

Performance. The Past Performance Factor is more important than the Cost/Price Factor.

Each evaluation factor will be narratively assessed to determine Strengths, Deficiencies, Weaknesses, and any Uncertainties. The following shall be used to adjectively rate each proposal at the factor level.

Technical Merit Adjectival Ratings

ADJECTIVAL DEFINITION

Outstanding Proposal meets requirements and indicates an exceptional approach and understanding of the requirements. Strengths far outweigh any weaknesses.

Risk of unsuccessful performance is very low.

Acceptable Proposal meets requirements and indicates an adequate approach and understanding of the requirements. Strengths and weaknesses are offsetting or will have little or no impact on contract performance. Risk of unsuccessful performance is moderate.

Marginal Proposal does not clearly meet requirements and has not demonstrated an adequate approach and understanding of the requirements. The proposal has one or more weaknesses which are not offset by strengths. Risk of unsuccessful performance is high.

Unacceptable Proposal does not meet requirements and contains one or more deficiencies.

Proposal is not awardable.

Price Evaluation

Price will be evaluated from the overall price of the entire ordering period to include all options. Attached

Excel Pricing Table must be submitted in its entirety including Total Estimated Amount, all Option

Periods.

Past Performance

Past Performance will be evaluated as satisfactory or unsatisfactory based upon submitted past performance information from similar work, information gathered from the Contractor Performance

Assessment Reporting System, and all evaluators’ prior knowledge of the offeror as follows.

ADJECTIVAL DEFINITION

Satisfactory Past performance provides a majority of positive information which outweighs any marginal assessments. Risk of unsuccessful performance is very low.

Marginal Past performance information provides satisfactory reports balanced or slightly overshadowed by marginal reports. Risk of unsuccessful performance is moderate.

Unsatisfactory Past Performance clearly provides a majority of marginal to unsatisfactory performance. Risk of unsuccessful performance is high.

(End of Addendum to 52.212-2)

52.212-3 -- OFFEROR REPRESENTATIONS AND CERTIFICATIONS-Commercial Items (OCT

2018)
