ADDENDUM 1 –BUSINESS PROPOSAL INSTRUCTIONS

In accordance with FAR 12.302, this addendum provides additional information and instructions to offerors to assist in proposal submission:

ADDITIONAL BUSINESS PROPOSAL INSTRUCTIONS

These additional Business Proposal instructions reflect the requirements of the solicitation. These instructions are tailored to the specific requirements of the solicitation and provide uniform costing assumptions and business clarifications.

The business proposal shall be submitted as a separate volume from the technical proposal. The following information should be used to assist offerors in preparing the business proposal.

Provide estimates of cost for the activities listed below using the information provided below and in the Statement of Work. Note that the list of activities below is provided for costing purposes only and to give offerors an idea of the type and frequency of services needed during each anticipated 12-month Delivery Order period. The actual number and mix of services may vary depending upon the needs of the government. The government may order similar or complementary services offered by the Contractor which also fall within the scope of work for this contract.

Offerors shall use the following monthly estimates to derive costs as follows:

A. Provide a fixed unit price for each of the sequencing services listed below. Offerors should identify and include any applicable discounts in pricing.

B. Provide an estimated not to exceed (NTE) amount for each 12-month Delivery Order period.

The NTE amount for each period of performance should be derived by taking into account the estimated number of fixed unit prices for the sequencing services above.

C. Offerors should submit a costing spreadsheet showing the fixed unit prices and the NTE ceiling price for each delivery order period of performance. In addition, offerors shall submit a separate cost spreadsheet which summarizes the estimated total for the entire 5-year contract ordering period.

D. Offerors should also submit a copy of their most current commercial price list which provides pricing for similar or complementary services which fall within the contract Statement of Work.

Offerors should clearly indicate any further discounts that will be offered on these additional services during the period of the contract.

E. Other considerations:

• Government Furnished Materials - NIEHS will furnish the biological samples for analysis.

• Travel – There will be no approved travel under this contract.

• Shipping and Packaging Requirement - Biological materials will be picked up at the NIEHS main campus located at 111 T.W. Alexander Drive, Durham, NC. Offeror will notify NIEHS of receipt of materials on the same day as receipt.

In performance of this contract all widely accepted industry practices shall be adhered to including but not limited to:

• Instrumentation maintained to manufacturer specifications;

• Use of an internal positive control;

• Minimum of 600 bases of data with a quality value (qv) >20;

• Turnaround time of up to 2 working days (5 working days if generation of template is required);

• Repeat of failed samples due to instrument failure or other technical issues at no additional charge;

• Troubleshooting assistance at no additional charge;

• Universal vector primers at no additional charge.
