About this file

This Limited-Sources Justification document details a sole-source procurement by the Defense Logistics Agency (DLA) Troop Support for the Medical Department of Navy Expeditionary Combat Command (NECC). The procurement involves 1,650 medical first aid kits from North American Rescue LLC (NAR), specifically the Expeditionary Casualty Response Kit (ECRK) and the CCR-Maritime Assault-Coy Kit (CMACK), with part numbers 80-0889 and 80-0015 respectively. These kits are critical for addressing preventable combat deaths by providing specialized trauma care equipment, including combat tourniquets, chest seals, hemostatic dressings, and other essential medical supplies.

The justification emphasizes that NAR's kits are the only products meeting NECC's minimum requirements under Authorized Medical Allowance List (AMAL) 4058 and 4060. Market research conducted in August 2025 determined that competing products from CRO Medical and ADS lack critical compatibility features, such as backwards integration with existing equipment, visual indicators, and one-handed tourniquet application. The procurement will support up to 20,000 personnel daily, with kits designed for deployment in INDOPACOM, AFRICOM, and CENTCOM regions. The DLA Troop Support has already determined the pricing to be fair and reasonable under the Electronic Catalog (ECAT) Multiple Award Schedule Program.

NAR products, the expeditionary casualty response kit (ECRK) and the CCR-Maritime Assault-

COY kit (CMACK) are integral components of Tactical Combat Casualty Care (TCCC)’s standardized approach to pre-hospital trauma care, required for training and real-world operational environments to support the mission of the expeditionary mission at the NECC.

4. This action is being conducted under the authority of the ECAT Multiple Award Schedule

Program. The circumstances justifying waiving the competition requirements is FAR 8.405-

6(a)(1)(i)(B) – only one source is capable of providing the supplies required at the level of quality required because the supplies are unique or highly specialized. As explained below, this particular product is essential to the Government’s requirements, and market research has established that other companies’ similar products cannot meet all of the Government’s needs.

The ECRK & CMACK shall be compatible with existing North American Rescue Individual

First Aid Kit (IFAK) equipment utilized by NECC for rapid trauma treatment. The standardized rapid response to preventable deaths in combat situations requires seamless integration with and the existing kit design, visual indicators for immediate identification and proper application in first aid combat scenarios. The ECRK is a lightweight platform designed to rapidly access and organize all essential trauma management equipment. The system is segregated into 6 segregated trauma management pouches organized by color coded quick pull handles for rapid access in operational environments. The ECRK contains essential kit components to include a combat tourniquet with one handed application, trauma blanket, nasopharyngeal airways & decompression devices, gauze & hemostatic dressings, chest seals, burn management dressings, and additional essential trauma management equipment. The requirement for the Medical

Department of NECC’s Authorized Medical Allowance List (AMAL) has become an item that is issued to the Medical Department of NECC’s boats and vehicles based on the expeditionary mission set of the sailors that carry kits. As the central issue facility and the Medical Department of NECC’s limitations, the sailors do not have the capacity to carry multiple iterations and refresh various items from different kits.

ECRK has lifesaving equipment items standard in expeditionary boats and vehicles, that the

Medical Department of NECC’s force trains to these items for emergency first aid response to preventable deaths in combat situations. ECRK kits shall be interoperable with other equipment/systems (AMAL 4058) or existing systems at the Medical Department of NECC.

Deployed forces have and can employ these kits in INDOPACOM, AFRICOM, CENTCOM.

ECRK fits the Medical Department of NECC’s standard AMAL list.

The CCR-Maritime Assault-Coy Kit (CMACK) is the standardized Individual First Aid Kit

(IFAK). The NECC’s minimum requirements AMAL 4060 for an IFAK are:

• Cutter, strap.

• Gauze bandage 4.5”X4.1YD Cotton Compressed, 6 ply, sterile.

• Bandage ETD 6 inches by 56 inches length.

• Bandage, Gauze, Hemostatic, Quik clot 3 inches by 4 yards.

• Dressing chest seal HyFin vent twin pack for entry/exit penetrating wounds.

• Gloves exam Nitrile non-latex rubber overall powder-free, large size, 100 per box.

• Tourniquet nonpneumatic combat application one-handed 30.5 inches long.

• Nasal trumpet 28 FR.

• Needle Decompression device 14GA by 3.25 inches.

• Shield, eye, post trauma, polycarbonate, ventilated, application directions, orange, (1) each.

• Bag molle battle pack coyote leg mounted.

• CAT holder coyote.

• Tag ID personnel cardboard 8.188 inches X 4.062 inches field triage 50S.

CRO Medical’s Operator Hybrid IFAK has combat gauze, Z-fold gauze, occlusive dressing x2, needle decompression 14G, a nasopharyngeal airway (NPA), 4” elastic bandage, casualty card, Nitrile gloves, combat application tourniquet (CAT) Gen 7, and sharpie. CRO Medical’s

Operator Hybrid IFAK does not meet the minimum requirements AMAL 4060, because it is missing the following items:

• CAT holder coyote.

• Bag molle battle pack coyote leg mounted.

• Shield, eye, post trauma, polycarbonate, ventilated, application directions, orange, (1) each.

• Hemostatic, Quik clot 3 inches by 4 yards.

• Tourniquet nonpneumatic combat application one-handed 30.5 inches long.

CMACK is the only IFAK product in our contract that meets NECC’s minimum requirements, AMAL 4060. It has all tools to mitigate the three leading causes of preventable combat death

(60% extremity bleeding, 33% tension pneumothorax, and 6% airway obstruction), a color-coded trauma treatment system and a quick start guide for rapid treatment of three preventable causes of trauma death. The CMACK contains essential kit components to include a combat tourniquet with one handed application, nasopharyngeal airway & decompression devices, gauze & hemostatic dressings, chest seals, and additional essential trauma management equipment. It has removable belt and leg traps. It is a small, lightweight & efficient IFAK with sand and water resistant, and self-repairing zipper with silent cord pulls. CMACKs will be issued to all deploying sailors, who have Tactical Casual Combat Care (TCCC) medical training, to provide first aid response in support of the expeditionary mission. The CMACK is compatible with the existing equipment at the Medical Department of NECC. The usage of CMAK with the existing equipment at the Medical Department of NECC allows kit organization and standardization that is not achievable with other products. The standardization and quick-start guide offered with

CMACK and ECRK resupply facilitates easier identification of critical components and faster response times under adverse conditions. The Medical Department of NECC requires 7 days of supply for up to 20,000 personnel a day.

In combat operations, rapid response is crucial when treating preventable deaths. As mentioned above, the standardization of the color-coded trauma treatment system and quick-start guide combined with the organization within North American Rescue ECRK and CMACK significantly reduces the response time when applying first aid on the battlefield. Reduced response time and simplified application is especially vital when first aid is being administered in high pressure, life or death situations, by sailors who lack medical training. There are no other competitors that can offer the same features.

No other manufacturer has the capability of meeting the Medical Department of NECC’s requirements listed above. Only the NAR Individual First Aid Kit kits comprised of the NAR expeditionary casualty response kit and NAR CCR-Maritime Assault-COY kit are capable of satisfying the requirements.

5. DLA Troop Support has already determined the prices of the supplies under the ECAT

Multiple Award Schedule Program fair and reasonable based on a favorable comparison to other than certified cost or pricing data.

A determination by the ordering activity contracting officer that the order represents the best value consistent with 8.404(d).

6. Market research was conducted in August 2025 using the FDA website, internet search, Emergency Case Research Institute (ECRI), and published literature.

Within the product category of boat bag first aid kit there are two products available on ECAT contracts. These products are from manufactured by North American Rescue LLC (NAR) and

CRO Medical.

The CRO model did not meet all the NECC’s minimum requirements AMAL 4058 because it is missing the following below items:

• Blanket survival blizzard pack reflex cell military green.

• Towelette benzalkonium chloride impregnated 8X5 inches alcohol free 100 sheets.

• Sling (triangle) with safety pins 37 inches X 37 inches X 52 inches.

• SAM splint malleable 36 inches X 4.5 inches gray.

NAR’s expeditionary casualty response kit (ECRK) meets all NECC’s requirements, and it is complete on arrival and compatible with The Medical Department of NECC’s existing equipment, boats and vehicles. The CRO model lacks the backwards compatibility and visual indicators the ECRK provides and would require additional training for proper application.

Beyond the visual application indicators, the inclusion of the C-A-T tourniquet incorporates a windlass system that allows for precise and secure tightening with minimal physical effort.

Additionally, the C-A-T features a one-handed application design that is critical in scenarios where the user may need to self-apply the device with only one free hand-a capability not uniformly present in the CRO Medical model.

Within the product category of individual first aid kit, variations of IFAK resupply kits with similar contents available on the market include ADS’ Tactical Medical Solutions, CRO

Medical’s CBP IFAK 2, and North American Rescue LLC’s CCR-Maritime Assault-Coy kit

(CMACK) for the AMAL requirement. The CCR-Maritime Assault-Coy Kit is the only IFAK compatible with the Medical Department of NECC’s existing equipment issued to each sailor that allows for standardized kit placement and organization. The ADS & CRO Medical models lack the backwards compatibility and visual indicators the CMACK provides and would require additional training for proper application. Beyond the visual application indicators, the inclusion of the C-A-T tourniquet incorporates a windlass system that allows for precise and secure tightening with minimal physical effort. Additionally, the C-A-T features one-handed application design that is critical in scenarios where the user may need to self-apply the device with only one free hand-a capability not uniformly present in the CRO Medical or ADS models.

These items are essential components of TCCC’s standardized approach to pre-hospital trauma care. Only North American Rescue’s expeditionary casualty response kit and CCR-Maritime

Assault-Coy kit are capable of satisfying the requirements at The Medical Department of Navy

Expeditionary Combat Command (NECC).

7. In an effort to remove or overcome barriers that led to the restricted consideration in this acquisition, DLA Troop Support will continue to attempt to locate other acceptable sources for future requirements at the Medical Department of NECC as they arise; however, any new item would need to meet all of the Medical Department of NECC’s requirements.
