About this file
This document is a Price Appendix FAQ for a Request for Quotation (RFQ) issued by the Department of Defense (DoD) for the DoD Pharmacy Uniform Formulary Blanket Purchase Agreement/Uniform Formulary Additional Discount Program. The RFQ is for pharmaceutical agents within several drug classes, including antilipidemics, leukemia and lymphoma agents, and antipsychotic agents for Parkinson's psychosis, to be considered for placement on the DoD Uniform Formulary.
The document provides detailed instructions and requirements for vendors to submit pricing quotes using spreadsheet templates. Key details include: completing yellow cells, understanding formulary notes, inputting package sizes and non-FAMP prices, and providing additional offered refunds. Vendors may submit quotes for up to 10 NDCs, and can request additional tables if needed. The RFQ solicitation number is HT9402-24-Q-9301, the P&T Committee meeting is scheduled for August 2024, and the pre-quote teleconference is on April 24, 2024. Vendors must submit responses to clinical questions by the deadline stated in the RFQ.
