STATEMENT OF WORK

One-Time Service for the 7’ Autoclave Composite Shop 93208, Bldg. 250 NAS North Island, San Diego, CA

1. Scope:

1.1. This Statement of Work (SOW) identifies necessary supplies and services to establish an on-site service agreement of a Government owned asset located at the Fleet Readiness Center Southwest (FRCSW), Bldg.

250, NAS North Island, San Diego, CA. 92135-7058.

1.2. The requested services is to repair the 7’ Autoclave, S/N: 250

1.3. The Period of Performance (POP) for the requested services is for a period not to exceed 90 days after contract award date.

2. Applicable Documents:

2.1. Dept. of the Navy Local Population ID Card/Base Access Pass Registration SECNAV Form 5512/1

2.2. Commander, United States Fleet Forces Command (CUSFF)/US Navy North (NAVNORTH) COVID-19

3. General Requirements:

3.1. The Contractor must be knowledgeable with the equipment referenced in Section 1.2 and be able to perform all work in accordance with the Original Equipment Manufacturer (OEM) specifications.

3.2. The Contractor is required to provide all tools and material, including but not limited to: test equipment, alignment tools, meters, support equipment and hardware required to perform the requested services.

3.3. The Contractor is required to ship all materials and supplies necessary to complete the requested services via traceable means (i.e., FedEx, UPS, USPS, etc.) to the following location:

3.3.1. Shipping documents shall clearly identify the Designated Government Representative (DGR) and this Contract Number.

3.3.2. The Government will notify the Contractor when all material has been received.

3.3.3. The Contractor is responsible for returning shipping tools, support equipment, and unused material back to the appropriate Contractor facility.

3.4. All material is required to be on-site prior to any work being performed.

3.5. The Contractor is required to perform all services in accordance with the established FRCSW regular workweek and day start and end times.

3.5.1. Work will be performed during a regular workweek from Monday through Friday, except Federal holidays.

3.5.2. Work will be performed during FRCSW normal business hours of 6:00 a.m. to 2:30 p.m., Monday through Friday, except Federal holidays. Unless arrangement were discussed with the DGR.

4. Specific/Performance Requirements:

4.1. The Contractor shall provide and replace the following material needed to repair the 7’Autoclave.

4.1.1. Replace the inlet piping from the Honeywell valve to the Autoclave.

4.1.2. Replace the two (2) 8”-300# reducing flanges, as well as 4”, 3”, and 2” pipe downstream of the

Honeywell pressurization inlet valve given their was exposure to liquid nitrogen (cryogenic exposure).

4.1.3. Replace the Honeywell pressurization inlet valve.

4.1.4. Replace the liquid nitrogen valve at the liquid nitrogen tank and vaporizer.

4.1.5. Replace the thermostat, which shuts the liquid nitrogen valve off at signs of low temperature exiting the vaporizer.

4.1.6. The Leslie steam valve and the control device for the Leslie steam valve (Powers Acritem Controller).

4.1.7. Integrate an alarm into the nitrogen system at the control panel to advise the Autoclave operators that there is an issue at the nitrogen system located outside. The alarm would activate when the liquid nitrogen begins to flow towards the Autoclave or if there is no steam present in the nitrogen heat exchanger. This early warning alarm will allow the Autoclave operator time to shut down the system and prevent damage to the Autoclave.

4.2. The Contractor shall provide testing of the machine upon completion of the installed replacement parts and materials. This will ensure the machinery/equipment/program operations are working properly before turning it over to FRCSW.

4.3. During the requested services if the Contractor identifies any discrepancies, or determines that additional parts and/or services other than what is listed in this SOW are required, they shall notify/contact the DGR via email and to include these findings in the Field Service Report (FSR).

4.4. The Government will review any reports generated and make a determination on whether to approve any additional parts or services.

4.5. FRCSW personnel are not authorized to direct the Contractor to furnish any parts/components or labor services outside the scope of the contract. To do so may constitute an unauthorized commitment. The Contractor shall not perform any additional labor or furnish parts/components outside the scope of this contract.

4.6. The FRCSW DGR and/or the primary Contractor Representative shall notify the Commander Fleet

Readiness Center - Procurement Group Contracting Officer (KO) to discuss the need for potential parts, labor, and funding.

5. Inspection and Acceptance:

5.1. The Government will perform at minimum, periodic and random inspections while the Contractor is on-site to ensure that the Contractor is performing all tasks in accordance with the contract.

5.2. Acceptance by the Government shall be dependent on the Contractor demonstrating that the 7’ Autoclave is operational per OEM specifications.

5.3. A Certificate of Completion (COC) in accordance with FAR 52.246-15 shall function as the Government’s inspection and acceptance (I/A) document. Upon completion of services:

5.3.1. The Contractor shall provide the DGR with a COC.

5.3.2. The DGR shall review, sign, and provide a signed copy of the COC to the Contractor prior to departing NAS North Island.

6. Deliverables:

6.1. Upon completion of services, the Contractor shall provide a FSR to the DGR.

6.1.1. The report shall include the name of the technician(s) performing the services.

6.1.2. Date(s) the services were performed.

6.1.3. Model and serial numbers of the equipment serviced with a detailed report of the work performed.

6.1.4. An electronic copy of the FSR shall be provided to the DGR via email within seven (7) days of completing services.

6.1.5. Any discrepancies should be noted along with a list of parts and/or services suggested to return, or keep the equipment operational.

7. Warranty: The Government requires that the Contractor provide a standard commercial warranty for a minimum period of twelve (12) months after the POP for the requested services. Parts requiring replacement under warranty shall be at no additional cost to the Government and must include material, labor, shipping and handling.

8. Special Requirements:

8.1. Base Access:

8.1.1. The Contractor shall provide the DGR with a completed copy of the SECNAV Form 5512/1 requiring the following;

8.1.1.1 Each contractor personnel will provide Full name (first, middle, and last), date of birth, place of birth, citizenship, full Social Security Number of each person requiring base access in ordered to facilitate the vetting process required at least ten business (10) days in advance of the scheduled site visit.

8.1.2. Upon arrival, all personnel will be required to check-in at the Visitor Control Center (Bldg.680), which is located near the Main Gate to Naval Base Coronado (NBC), NAS North Island, San Diego, CA. Following receipt of a clearance, personnel will then be required to coordinate with the DGR and obtain visitor badges through the FRCSW Quarterdeck, located in Building 94.

252.232-7006 WIDE AREA WORKFLOW PAYMENT INSTRUCTIONS (DEC 2018)

(a) Definitions. As used in this clause—

“Department of Defense Activity Address Code (DoDAAC)” is a six position code that uniquely identifies a unit, activity, or organization.

“Document type” means the type of payment request or receiving report available for creation in Wide Area WorkFlow (WAWF).

“Local processing office (LPO)” is the office responsible for payment certification when payment certification is done external to the entitlement system.

“Payment request” and “receiving report” are defined in the clause at 252.232-7003, Electronic Submission of Payment Requests and Receiving Reports.

(b) Electronic invoicing. The WAWF system provides the method to electronically process vendor payment requests and receiving reports, as authorized by Defense Federal Acquisition Regulation Supplement (DFARS) 252.232- 7003, Electronic Submission of Payment Requests and Receiving Reports.

(c) WAWF access. To access WAWF, the Contractor shall—

(1) Have a designated electronic business point of contact in the System for Award Management at https://www.sam.gov; and

(2) Be registered to use WAWF at https://wawf.eb.mil/ following the step-by-step procedures for self-registration available at this web site.

(d) WAWF training. The Contractor should follow the training instructions of the WAWF Web-Based Training Course and use the Practice Training Site before submitting payment requests through WAWF. Both can be accessed by selecting the “Web Based Training” link on the WAWF home page at https://wawf.eb.mil/.

(e) WAWF methods of document submission. Document submissions may be via web entry, Electronic Data Interchange, or File Transfer Protocol.

(f) WAWF payment instructions. The Contractor shall use the following information when submitting payment requests and receiving reports in WAWF for this contract or task or delivery order:

(1) Document type. The Contractor shall submit payment requests using the following document type(s):

(i) For cost-type line items, including labor-hour or time-and-materials, submit a cost voucher.

(ii) For fixed price line items—

(A) That require shipment of a deliverable, submit the invoice and receiving report specified by the Contracting Officer.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records--Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
