(CDRL A020).
Deliverable: CDRL A020 List Failure Analysis and Corrective Action Report (FACAR) – non-conformances prior to Factory Acceptance Test and during Production Screening
4.9.2 Factory Acceptance Test Non-Conformances
The contractor shall perform fault isolation and identify root cause for each FAT non-conformance and submit a Failure Analysis and Corrective Action Report – non-conformances discovered during First Article, POM, Factory Acceptance Testing, Government POM, and Conditional Acceptance, or after Final Government Acceptance when cooperatively determined to be a latent manufacturing defect (CDRL A019).
Deliverable: CDRL A019 Failure Analysis and Corrective Action Report – non-conformances
4.9.3 Systemic Non-Conformances
The contractor shall develop and document a Corrective Action Plan (CDRL A021) for systemic non-conformances which is defined as three of more units displaying fundamentally identical non-conformance symptoms. The contractor shall provide status of any Corrective Action Plan(s) and the corresponding implementation results under the Integrated Program Management Report: Program Manager’s Report (CDRL A005).
If, at any time during production, a systemic pre-FAT or production screening failure is identified, the contractor shall conduct root cause analysis and perform an engineering analysis and report it via Engineering Analysis Report (CDRL A022).
The contractor shall isolate the failure cause to the lowest level reasonably achievable. The contractor shall define Levels as system, card, circuit, component, and root cause physics. In cases where it is not practical to determine the root cause physics, the contractor shall provide:
(1) A fault tree type diagram showing all the evidence accumulated during the investigation
(2) A description of how the evidence supports or refutes any plausible failure scenarios
(3) The rationale for terminating the investigation above the root cause physics level
Deliverable: CDRL A005 Program Manager’s Report CDRL A021 Corrective Action Plan CDRL A022 Engineering Analysis Report
5.0 Progress Reports
The Contractor shall prepare a Contractor’s Status Report (CDRL A023) that indicates the progress of work, status of the program(s), and existing or potential problem areas for all assigned tasks. The Contractor shall submit the Contract Status Report for the same timeframe as each invoice submitted in the Wide Area Workflow (WAWF) Module of the Procurement Integrated Enterprise Environment (PIEE).
The contractor shall include Subcontractor/Suppliers performance against the schedule baseline established in the PMP, (CDRL A005), Technical Instructions/Engineering Services, Obsolescence Reviews, Risk Management, Failure Analysis, Returned Goods, Technical Instruction Status, and status of manufacturing & testing on deliverable hardware in its contractor status report.
Deliverable: CDRL A005 Program Manager’s Report
CDRL A023 Contractor’s Status Report
6.0 Government Furnished Information
No Government Furnished Information will be given at this time.
7.0 Government Furnished Property
The Government may provide Government Furnished Property (GFP) as listed in Attachment 3.
As specified in DFARS Clause 252.211-7007, the Contractor shall submit the GFP Baseline Report via the GFP Module of the Procurement Integrated Enterprise Environment (PIEE).
Monthly, the Contractor shall prepare a Government Property Inventory Report (CDRL A024) indicating the monthly consumption of GFP.
Deliverable: CDRL A024 Government Property Inventory Report
