ACTIVITY: USS MONTPELIER (SSN 765)

ORIGINATOR: Jim Crudden DATE: 08 November 2019

1. SCOPE: Remove existing Ceramic/Porcelain Deck Tile and install Terrazzo in six (6) locations

2. REFERENCE/PLANS:

2.1 Naval Ships Technical Manual (NSTM) Chapter 634 - S9086-VG-STM-010

2.2 Deck Covering Materials, Interior, Cosmetic Polymeric (MIL-PRF-24613)

2.3 Deck Covering Underlay Material (MIL-PRF-3135)

2.4 Environmental Readiness Program Manual (OPNAVINST 5090.1)

3. REQUIREMENTS:

3.1 Remove existing deck material to base metal, clean and prepare deck, install MIL-PRF-24613 Type 1 Terrazzo in five (5) locations on board ship in accordance with references 2.1 through 2.4.

3.1.1 Locations:

3.1.1.1 Laundry Room: 10 square feet

3.1.1.2 CO/XO head and shower: 25 square feet

3.1.1.3 CPO Head and shower: 30 square feet

3.1.1.4 Crews Lower Level Head and Shower: 30 square feet

3.1.1.5 Trash Disposal Room Deck: 25 square feet

3.1.1.6 Middle Level Head and Shower: 85 square feet

4. NOTES

4.1 USS Montpelier (SSN 765) located at the Naval Submarine Base, Groton, CT Pier 6N.

4.2 For Contractor personnel that do not have a CAC card or a NCACS card, the following is required to obtain DBIDS credentials to gain access the base:

a. Ensure you provide the names of personnel requiring access to the SUBASE to the RSG POC. RSG will pass the information to the SUBASE Pass and ID office as your government sponsor.
b. Upon arrival at SUBASE, Present valid identification, Real ID Act-compliant state driver's license. If you do not have a Real ID, you can use a passport. If you do not have a passport you will be required to have an original birth certificate and your social security card. (not just your number).
c. Present a completed copy of the SECNAV 5512/1 form to obtain your background

check. A copy of SECNAV 5512/1 can be obtained by contacting the RSG POC.

d. Upon completion of the background check, the Visitor Control Center representative will complete the DBIDS enrollment process, which includes your photo, finger prints, base restrictions, and several other assessments; after all this is done, you will be provided with your new DBIDS credential. You may now proceed to work.

4.3 After existing deck is removed, inspection of the underlying decking material will be conducted by ships force. Additional work scope may be required for government disposition (i.e. repair to CRES, structural repairs, etc.)

4.4 Existing deck includes the 3 – 5 inch raised border at each bulkhead.

4.5 It is desirable that personnel working on the submarine have a government secret security clearance and capability to submit clearance through the JPAS system. Any personnel without security clearance will have to be escorted their entire time on board.

4.6 Ship’s Force will dispose of all HAZMAT, including any removed decking material that is determined to be HAZMAT.

4.7 Ship’s Force will be responsible for removing all interference (deck drains, toilets, etc.)
4.8 Contractor shall adhere to OPNAVINST 5090.1 for any HAZMAT needed for the performance of

work indentified herein. Hazardous Material (HM) control services will be coordinated through the New London Submarine Base Consolidated Hazardous Material Reutilization Inventory Management Program (CHRIMP). Contact CHRIMP at (860) 694-4698 for the Director or the Warehouse Supervisor at 860-694-5691.

4.9 The Government will provide forklift and crane services as required. No other services will be provided unless otherwise listed.

4.10 Local Fleet Maintenance Activity (FMA) and Ship’s Force will provide all required exhaust ventilation equipment.

4.11 Required terrazzo is DEX-O-TEX (Salt and Pepper) for installation (meets all requirements of ref 2.1, 2.2 and 2.3) and matches terrazzo installed on other areas of the ship. The government shall approve all substitutions.
