STATEMENT OF WORK (SOW)
1. SCOPE:

The purpose of this procurement is to provide and maintain each temporary portable head and washbasin during USS SAN DIEGO (LPD-22) 6C1 availability.
Job specification 38DX286319-00-A01 is attached as enclosure SOW Atch (1).
Provide all labors, tools, materials, and equipment to accomplish the requirements of the Job
Specification listed in 1.2.
The place of the performance shall be in accordance with the Job Specification listed in 1.2 in
Sasebo, Japan.
The on-site work stated on this SOW shall be completed from 17 Feb 2026 through 31 Jul 2026.
The effective period of this order is from the date of award through 31 Jul 2026.
The contractor shall coordinate the work schedule with the Government Technical Point of
Contact (POC) to accomplish all requirements on this SOW. All other related works shall be completed by the expiring date of this order.
Submit reports required by the Job Specification listed in 1.2 in accordance with reference 2.1 of the Job Specification.

3. DESCRIPTION OF TASK
3.1. Accomplish the requirements of the Job Specification listed in 1.2.
4. GOVERNMENT FURNISHED
4.1 MATERIAL (GFM): None.
4.2 EQUIPMENT (GFE): None.
4.3 SERVICE: The Government Technical POC will arrange, and the Government will provide following services within Sasebo Naval Base and/or at site.
(a) Compressed air services within Government’s capabilities (available pressure and capacity).
(b) Supply of water and electricity.

5.1.1 Only the Government Technical POC has authority to coordinate schedule with the contractor within terms and conditions of this contract. The Government Technical POC will coordinate base access and provide escorts for the contractor if escorts are required for personnel entering, departing, and working in required areas. Ask any payment issue as the first POC.
5.1.2 Only the Contracting Officer and/or designated Contracting POC have authority to change any terms and conditions of this contract. Report any conditions, discrepancies, and/or finding to the Contracting POC via the Government Technical POC. In the event the Contractor effects any change at the direction of any person other than the
Contracting Officer, the change will be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in charges incurred as a result thereof.

2. REFERENCES:
2.1 None.
3. REQUIREMENTS:
3.1 Provide and maintain each temporary portable head and washbasin listed in 1.3.1 through 1.3.3 at each location designated by SHIPSUPT during the availability.
3.1.1 Each door must have locking device that can be operated from inside.
3.1.1.1 Install each label plate indicated in both ENGLISH
and JAPANESE letters as FOR MALE USE on each door of head listed in 1.3.1.
3.1.1.2 Install each label plate indicated in both ENGLISH
and JAPANESE letters as FOR FEMALE USE on each door of head listed in 1.3.2.
3.1.2 Provide each hose for the heads and washbasin.
3.1.3 Provide each toilet paper and soap during period for the temporary portable heads and washbasin.
3.1.4 Clean each temporary head and washbasin two times a day.
3.1.5 Apply Approximately 200 cubic centimeters (CC) of disinfectant (deodorizer) to each head (urinals and toilet bowls) to keep clean sanitary condition.

3.1.6 Application of the disinfectant (deodorizer) must be made using materials such as creosote two times a day after clean up.
3.1.7 Each temporary head and washbasin must be secured to prevent from damage against bad weather and set up properly (flat level and height).
3.2 Collect, transport and dispose of the sewage (total estimated 8,100 gallons) accumulated in each temporary head and drain water from washbasins listed in 1.3.1 through 1.3.3 in accordance with Japan Environmental Governing Standards (JEGS).
(V) "REPORT"
3.2.1 Submit one legible copy of a report listing the amount of sewage discharged actually to SUPERVISOR after disposal of the sewage.
3.2.1.1 The report must be written in English and unit for
the amount of the sewage must be in Gallon.
3.3 Remove each portable head and washbasin listed in 1.3.1 through 1.3.3 when notified by SHIPSUPT.

4.1 Initial Condition - Ship requires 6 male and 3 female porta potties and 2 wash station in support of CNO availability FY26.
