The following have been added by full text:

AMENDMENT 02

UPDATES AND REVISIONS:

1. Section L, Attachment 4 Self Scoring Worksheet is revised, see attached.

2. Section L, Attachment 3 Pricing Workbook is revised, see attached.

QUESTIONS AND ANSWERS:

Question 28. Since California has strict overtime laws for hourly employees, it is imperative that the Government provides the anticipated schedules so that offerors can budget for overtime costs. The following CLINs have been identified with a possible schedule of “8, 10, or 12 hour shifts” – please specify which shift is likely to be scheduled for each CLIN:.

a. Answer 28: The contractor is responsible for complying with overtime laws. The Government has considered this request; however, the requirement will remain as is.

Question 29. Can the government confirm that the Psychiatrist labor category falls under the Physician Labor band for technical scoring purposes?

a. Answer 29: Yes, Psychiatrist fall under the Physician labor band.

Question 30. Section_L,_Attachment_3 _Pricing_Workbook_ for_ N62645-17-R-0014. Lot C. Option Period 5 for CLIN 5003AA has the same number of hours as other option periods, however, the period of performance is not as long as the other option periods. Please confirm this is an error.

a. Answer 30: This is an error. Section L, Attachment 3 Pricing Workbook, Lot C is revised to correct error.

Question 31. Section_L,_Attachment_3 _Pricing_Workbook_ for_ N62645-17-R-0014. Lot F. The unit prices for Option Period 5 are linked to unit prices for Option Period 3 which does not accurately reflect the costs for Option Period 5 since they should be linked to Option Period 4. Please confirm this is an error.

a. Answer 31: Yes, Section L, Attachment 3 Pricing Workbook is revised to correct error.

Question 32. Section_L,_Attachment_3 _Pricing_Workbook_ for_ N62645-17-R-0014. Lot A-I; K,L. The periods of performance listed for many of the CLINs listed in the lots appear to start and end on different dates and do not align correctly. Is this an error or should we plan for performance during the periods listed?

a. Answer 32: The Government has reviewed this question; however, the pricing sheet shall be completed as provided.

Question 33. Section_L,_Attachment_3 _Pricing_Workbook_ for_ N62645-17-R-0014. Lot H. There are two Option Period 4s in Lot H which is double-counting Option Period 4. Please confirm this is an error.

a. Answer 33: Yes, Section L, Attachment 3 Pricing Workbook is revised to correct error.

Question 34. Will the government allow our reference who will provide proof of an IDIQ CONTRACT for the Prime to only fill out Section I? The master contract holder can't tell you about the performance at the different contract levels but can attest that we were awarded several contracts under the master contract. For example, our 621 I VA Schedule is an IKIQ contract and we have several awards throughout DoD, but the Contracting Officer can't answer Section II. Any assistance would be greatly appreciated. Thanks in advance.

a. Answer 34: Past Performance shall be completed in accordance to Section L of the RFP.

Question 35. Please note that Section L, Attachment 2 (from Amendment 1 of the Western MATO) downloads as a word document that states “Section L, Attachment 2 Reserved.” I’ve attached it here. There is no other information given. Is this supposed to be the past performance document where we list which references we are using for the PPIs? If so, can you please repost the correct version? If not, can you please let us know if that was posted in error?

a. Answer 35: As addressed in L.9. of the RFP, Section L, Attachment 2 is “Reserved”. The FBO attachment is correct. No further action is required.

Question 36. The formula in Cell H59 is not calculating on the correct data elements: i.e., = SUM(C58:F58) versus SUM(C59:F59). Should offerors correct this error?

a. Answer 36: The point value function sums preceding hidden row is correct. Note, Attachment 4 – Self Scoring Worksheet is revised, see attached.

Question 37. The formula in Cell H72 is not calculating on the correct data elements: i.e., = SUM(C71:F71) versus SUM(C72:F72). Should offerors correct this error?

a. Answer 37: The point value function sums preceding hidden rows. Attachment 4 – Self Scoring Worksheet is revised, see attached.

Question 38. The formula in Cell H74 is not calculating on the correct data elements: i.e., = SUM(C73:F73) versus SUM(C74:F74). Should offerors correct this error?

a. Answer 38: The point value function sums preceding hidden row is correct. Note, Attachment 4 – Self Scoring Worksheet is revised, see attached.

Question 39. There do not appear to be instructions regarding the duplicate descriptions of data elements in lines 14 through 44 and lines 91 through 114.

a. Answer 39: The Government has reviewed this question; however, there is no duplication of data elements presented.

Question 40. Are lines 14 through 44 intended to display data for both the prime and any subcontractors?

a. Answer 40: Refer to Self-Scoring Worksheet Instruction Page and Section L of the RFP for properly responding to past performance experience.

Question 41. Are lines 14 through 44 intended to display data for work performed as a prime contractor and as a subcontractor?

a. Answer 41: Refer to Answer 40.

Question 42. Are lines 91 through 114 intended to display data for the prime contractor whether as a prime contractor or a subcontractor?

a. Answer 42: Lined 91 through 114 are for experience as Prime Contractor.

Question 43. Are lines 91 through 114 intended to display data for work performed by the prime offeror as a prime contractor only?

a. Answer 43: Refer to Answer 42.

Question 44. Address each Data Element by marking the individual Data Element Lines with an “X”. Only one score will be received per Data Element Line. Do not leave a Data Element Line blank. When completed, there should be 23 "X" responses. “Address each Data Element by marking the individual Data Element Lines with an “X”. Only one score will be received per Data Element Line. a. Answer 44: There are 26 responses on the Self Scoring Worksheet. Attachment 4 – Self Scoring Worksheet is revised, see attached.

Question 45. We noticed some typo errors in the CLINs; for instance, in Lot B, CLIN 0001AA (0.6 FTE OB/GYN Physician) goes on to 1001AA, then 2002AA, 3002AA, 4002AA, and finally 5002AA. We bring this to your attention because there *is* a CLIN 0002AA (6 FTE Clinical Psychologists) and this could get confusing during preparations and subsequent deliberation.

a. Answer 45: The Government has considered this request; however, the requirement will remain as is.

Question 46. Line Item # 72 [C/D, H], Data Element: Geographic Location - Contract with clinical HCW performance in Hawaii – the point value is not computing the points

a. Answer 46: Attachment 4 – Self Scoring Worksheet is revised, see attached.

Question 47. Line Item # 74 [C/D, H], Data Element: Geographic Location - Contract with clinical HCW performance in Washington – the point value is not computing the points

Question 48. Lot F Option Period 5 is linked to Option Period 3 instead of Option Period 4

Question 49. Lot H contains duplicates Option Period 4 on the worksheet

Question 50. OPTION PERIOD 4 has two (2) option periods with the same CLIN, Description, Quantity (Hours), Unit Price and Total Price. Please advise if we should include both Option Periods in our pricing document.
