Lot A Statements of Work

Sub Line Item Number (SLIN) 0001AA and Options Psychiatrist Psychological Health and Traumatic Brain Injury Program (PH/TBI)

1. Site of Service. The contractor shall provide personnel for service in support of Naval Medical Center Portsmouth, VA (NMCP). The primary site of service shall be located in the Mental Health Department, NMCP.

2. Labor Category. The contractor shall provide personnel from the following labor category:

Psychiatrist (One (1) individual)

3. Qualifications. The health care worker (HCW) shall possess and maintain the applicable qualification requirements given in Section C of the basic contract. In addition, the following qualifications are required:

3.1. Have successfully completed a residency program in psychiatry which has been approved by the Accreditation Council for Graduate Medical Education or the Committee on Postdoctoral Training of the American Osteopathic Association or those Canadian training programs approved by the Royal College of Physicians and Surgeons of Canada or other appropriate Canadian medical authority.

3.2. Possess board certification in Psychiatry by the American Board of Psychiatry and Neurology, or the American Osteopathic Association.

3.3. Possess not less than three (3) years of experience within the last four (4) years as a full-time psychiatrist; OR, graduation from a psychiatry residency program within the preceding twelve (12) months.

3.4. Provide two (2) letters of recommendation written within the last two (2) years attesting to the HCW’s clinical skills. At least one (1) letter must be from a physician or supervisor who has personally observed and evaluated the HCW and who can attest to the HCW’s clinical skills in the care of patients. The other letter may be from a clinic or hospital administrator, physician or nurse who has personal knowledge of the HCW’s patient care skills in a similar clinical environment. Reference letters shall attest to the health care worker’s communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include name, title, phone number, date of reference, address and signature of the individual providing reference.

4. Staffing and Scheduling.

4.1. The contractor shall provide one (1) individual Psychiatrist. To maintain continuity of services, the HCW MUST BE A FULL-TIME INDIVIDUAL. The Commanding Officer will not approve more than one (1) individual for the required position.

4.2. The Commanding Officer or designed government representative will supervise the HCW.

4.3. Services shall normally be provided Monday through Friday, between the hours of 0630 and 2100. Services shall be provided for an eight and a half (8.5) hour shift to include an uncompensated thirty (30) minute meal break. Typically the HCW shall provide eighty (80) hours; however on occasion, the HCW may provide up to one hundred (100) hours of service (on-site service plus approved leave and holidays) per two (2) week period, unless providing on-call services listed under Section 4.4. In no instance will the HCW provide more than 2,500 hours of service in a twelve (12) month period of performance year, inclusive of the in-house rounds described in Section 4.4.2. The specific hours shall be scheduled one (1) month in advance by the supervisor. Any changes in the schedule shall be coordinated between the HCW and the Government. At the mutual agreement of the HCW and the Government, alternative schedules, such as a compressed work schedule, may be implemented. The Government reserves the right to change shift hours with seventy two (72) hours’ notice to accommodate patient care needs and mission requirements.

4.4. On-Call Services. The HCW shall equitably share rotations of on-call services with other Psychiatrists. It is anticipated that the HCW shall have five (5) on-call weekends with no more than three (3) holidays. Depending on military staffing and deployment, the number of psychiatrists can change.

4.4.1. On-Call Services (Off-site): Approximately every six (6) weeks the HCW shall be responsible for general and/or child and adolescent psychiatry on-call off-site services from 1600 to 0800 for one (1) evening, Monday through Thursday. On rare occasions, the HCW may be required to return to NMCP to provide services. The HCW will be reimbursed for not less than two (2) hours of compensatory time when required to return to the Military Treatment Facility (MTF) to provide services. On-call services will not be separately priced.

4.4.2. On-Call Services (weekend/holiday In-patient). Approximately every ten (10) weeks the HCW shall be responsible for weekend on-call services. Weekend On-call services will begin 1600 Friday evening and end 0800 Monday, unless it is a holiday, then services would end 0800 Tuesday. While on-call, the HCW shall provide in-house rounds in the General Psychiatry Inpatient Mental Health Units at NMCP for approximately six (6) hours (typically between 0800 through 1400) on Saturday, Sunday and Federal holidays. Historically, the HCW receives approximately four (4) to eight (8) calls during weekend on-call. The in-house rounds shall be compensated at the normal hourly rate in Section B of the task order; all other on-call services are not separately priced. On rare occasions, the HCW may be required to return to NMCP to provide services after in-house rounds are completed. The HCW will be reimbursed for not less than two (2) hours of compensatory time when required to return to the MTF to provide services beyond in-house rounds.

4.5. Services are not normally required on the day of observance of Federal holidays, unless providing pager on-call as addressed in Section 4.4. Holiday compensation shall be provided in accordance with Section C of the basic contract.

4.6. The HCW will accrue eight (8) hours of personal leave (annual plus sick) for each eighty (80) hour period of services provided (on-site plus approved leave and holidays). The HCW shall be subject to leave approval in accordance with Section C of the basic contract.

5. Duties. The HCW shall provide services within the scope of clinical privileges granted. In addition to the applicable duties provided in Section C of the basic contract, the HCW shall:

5.1. Assess, evaluate, consult, differential diagnose, and plan treatment for all disorders defined by the Diagnostic and Statistical Manual for Mental Disorders (current edition) published by the American Psychiatric Association such as:

* Cognitive impairment disorders

* Schizophrenia and other psychotic disorders

* Mood disorders

* Anxiety disorders

* Combat stress reactions

* Somatoform disorders

* Psychological or behavioral factors affecting a nonpsychiatric medical condition

* Dissociative disorders

* Factitious disorders

* Sexual disorders

* Gender identity disorders

* Eating disorders

* Sleep disorders

* Impulse control disorders not elsewhere classified

* Adjustment disorders

* Personality disorders

* Disorders usually first diagnosed in infancy, childhood, or adolescence (mental retardation, learning disorders, tics, etc.)

* Other clinically significant problems that may be a focus of diagnosis and treatment (movement disorders, relationship problems, bereavement, etc.)

5.2. Provide diagnostic and therapeutic procedures such as:

* Clinical interviewing

* Psychosocial history taking

* Mental status examination

* Physical examination

* Neurological examination

* Interpretation of psychological testing results

* Clinical case formulations

* Interpretation of radiological testing

* Interpretation of laboratory testing

5.3. Provide major types of psychotherapy including:

* Individual therapy

* Group therapy

* Marital therapy

* Short-term therapy

* Family therapy

* Psychodynamic therapy

* Behavior therapy

* Crisis intervention

* Community outreach (health promotion, command consultation)

* Pharmacotherapy

* Drug and alcohol detoxification

* Medical, drug and alcohol rehabilitation

* Evaluations for suitability and fitness for duty

* Evaluations for special military programs (Personnel Reliability Program, weapons, etc.)

* Incapacitation determinations

* NCM Article 706 boards (sanity boards)

Sub Line Item Number (SLIN) 0002AA and Options Chiropractor

1. Site of Service. The contractor shall provide personnel in support of Naval Medical Center Portsmouth, VA (NMCP). The primary site of service will be located in the Orthopedic Department, NMCP.

2. Labor Category. The contractor shall provide personnel from the following labor category:

Chiropractor (Two (2) individuals)

3. Qualifications. The health care workers (HCWs) shall possess and maintain the applicable qualification requirements given in Section C of the basic contract. In addition, the following qualifications are required:

3.1. Possess a minimum of three (3) years full-time experience as a Chiropractor within the last four (4) years in a position in which both diagnostic and treatment services were consistently administered.

3.2. Possess experience working in a hospital setting, and/or in a multi-disciplinary clinical environment.

3.3. Provide three (3) letters of recommendation, at least two (2) of which must be from allopathic or osteopathic physicians. Letters from allopathic or osteopathic physicians must indicate referral patterns. Other letters of recommendation may be from chiropractic peers or physical therapists. Letters of recommendation must attest to clinical skills. Recommendation letters must include name, title, phone number, date of reference, address and signature of individual providing the letter. Reference letters must have been written within the preceding two (2) years.

4. Staffing and Scheduling.

4.1. The contractor shall provide two (2) full time Chiropractors. To maintain continuity of services, the HCWs MUST BE FULL-TIME INDIVIDUALS. The Commanding Officer will not approve more than one (1) individual for each of the required positions.

4.2. The Commanding Officer or other designated government representative will supervise the HCWs.

4.3. Services shall be provided Monday through Friday, between the hours of 0500 and 1730. The HCWs shall each be schedule for an eight and a half (8.5) to eleven and a half (11.5) hour shift, to include an uncompensated thirty (30) minute or one (1) hour meal break depending on length of shift. In no instance will any of the HCWs exceed eighty (80) hours of service (on-site service plus approved leave and holidays) per two (2) week period. At the mutual agreement of the HCWs and the Government, alternative schedules, such as a compressed work schedule, may be implemented. Specific shifts will be scheduled one (1) month in advance by the workplace supervisor. Any changes in the schedule shall be coordinated between the HCWs and the Government. The Government reserves the right to change shift hours with seventy two (72) hours’ notice to accommodate patient care needs and mission requirements. The HCWs may not exceed the hours in the applicable Contract Line Item Number (CLIN)/Sub Line Item Number (SLIN) as stated in Section B.

4.4. Services are not required on the day of observance of Federal holidays. Holiday compensation shall be provided in accordance with Section C of the basic contract.

4.5. Each HCW will accrue eight (8) hours of personal leave (annual plus sick) for each eighty (80) hours worked. The HCWs shall be subject to leave approval in accordance with Section C of the basic contract.

5. Duties. The HCW shall provide services within the scope of clinical privileges granted, in addition to the applicable duties provided in Section C of the basic contract.

6. Travel. The HCWs shall adhere to travel requirements as given in Section C of the basic contract. Travel shall not be conducted unless the appropriate funding is included on the task order or added by modification to Contract Line Item Number (CLIN) 0008 in Section B of the task order.

Sub Line Item Number (SLIN) 0002AB and Options Clinical Psychologist Mental Health Directorate

1. Site of Service. The contractor shall provide personnel for services in support of Naval Medical Center Portsmouth, VA (NMCP). The primary site of services shall be located in the Mental Health Directorate.

2. Labor Category. The contractor shall provide personnel from the following labor category:

Clinical Psychologist (One (1) individual)

3. Qualifications. The health care worker (HCW) shall possess and maintain the minimum qualifications stated in Section C of the basic contract. In addition, the HCW shall possess the following qualifications:

3.1. Completion of one (1) year American Psychological Association (APA)-accredited Residency Program.

3.2. Possess experience as a Clinical Psychologist of at least two (2) years within the preceding three (3) years.

3.3. Provide two (2) letters of recommendation written within the last two (2) years attesting to the HCW’s clinical skills. At least one (1) letter must be from a physician or psychologist who has personally observed and evaluated the HCW and who can attest to the HCW’s clinical competence in the care of patients. The other letter may be from a clinic or hospital administrator, physician or nurse who has personal knowledge of the HCW’s patient care skills in a similar clinical environment. Reference letters shall attest to the HCW’s communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include name, title, phone number, date of reference, address and signature of the individual providing reference.

4. Staffing and Scheduling.

4.1. The contractor shall provide one (1) individual Clinical Psychologist. To maintain continuity of services, the HCW must be a full-time individual, unless a written waiver to this provision is requested by the contractor and approved by the Commanding Officer. The Commanding Officer will not normally approve more than one (1) individual for the required position.

4.2. The Commanding Officer or designed government representative will supervise the HCW.

4.3. Services shall normally be provided Monday through Friday between the hours of 0630 and 1900. The HCW will be scheduled for an eight and a half (8.5) hour shift to include an uncompensated thirty (30) minute meal break. Shifts will be on a rotated basis within the time ranges specified as clinical demand dictate. In no instance will the HCW exceed eighty (80) hours (on-site service plus approved leave and holidays) per two (2) week period. The specific hours shall be schedule one (1) month in advance by the supervisor. Any changes in the schedule shall be coordinated between the HCW and the Government. At the mutual agreement of the HCW and the Government, alternative schedules, such as a compressed work schedule, may be implemented. The Government reserves the right to change shift hours with seventy two (72) hours’ notice to accommodate patient care needs and mission requirements.

4.4. Services will not be required on the day of observance of Federal holidays. Holiday compensation shall be provided in accordance with Section C of the basic contract.

4.5. The HCW will accrue eight (8) hours of personal leave (annual plus sick) for each eighty (80) hours worked. The HCW shall be subject to leave approval in accordance with Section C of the basic contract.

5. Duties. The HCW shall provide services within the scope of clinical privileges granted. In addition to the applicable duties provided in Section C of the basic contract, the HCW shall:

5.1. Provide a full range of child and adolescent clinical psychologist duties.

5.2. Provide consultation, differential diagnosis, and treatment planning for all disorders defined by the Diagnostic and Statistical Manual for Mental Disorders for children, adolescents, and adult family members, including, but not limited to:

* Disorders usually first evident in infancy, childhood, or adolescence, including Mental Retardation, Learning Disorders, Communication Disorders, Pervasive Developmental Disorders, and Attention-Deficit/Hyperactivity Disorder (ADHD) and Disruptive Behavior Disorders:

* Organic mental disorders

* Psychotic Disorders

* Schizophrenia

* Delusional Disorders

* Dissociative Disorders

* Mood Disorders

* Anxiety Disorders

* Post-traumatic Stress Disorder

* Somatoform Disorders

* Psychoactive Substance Use Disorders

* Sleep Disorders

* Factitious Disorders

* Impulse control Disorders

* Psychological Factors Affecting Physical Condition

* Sexual and Gender Identity Disorders

* Adjustment Disorders

* Personality Disorders

* Conditions not attributable to a mental disorder that are a focus of attention or treatment

5.3. Provide diagnostic and therapeutic procedures, including but not limited to:

* Interviewing

* Major types of psychotherapy including: individual, group, and family psychotherapy, as well as appropriate psycho-educational classes

* Community outreach (e.g., health promotion and command consultation)

* Mental status examination

* Crisis intervention/management

* Case management and multidisciplinary consultation

* Medical discharge planning

* Psychosocial history taking

* Psychological test administration and interpretation

5.4. Participate in the provision of monthly in-service training to non-healthcare practitioner members of the clinical and administrative staff on subjects germane to mental health services. This shall include responsibility for developing and implementing psycho educational programs, community relations, information and referral.

6. Travel. The HCW shall comply with the travel requirements in Section C.11 and subparagraphs in the basic contract. Travel shall not be conducted unless the appropriate funding is included on the task order or added by modification to Contract Line Item Number (CLIN) 0008 in Section B of the task order.

Sub Line Item Number (SLIN) 0002AC and Options Clinical Psychologist Post Deployment Health Re-Assessment (PDHRA) Directorate of Primary Care (DPC)

1. Site of Service. The contractor shall provide personnel for service in support of the Directorate of Primary Care and Branch Clinics at Naval Medical Center Portsmouth, VA (NMCP) focusing on PDHRA tasks (see http://www.pdhealth.mil/sites/default/files/images/docs/DHCC_AR_2014.pdf). The goal is to remove barriers to patient care for service members and their families and to identify emerging health conditions related to deployment while providing care in the patient’s medical home. As such, the clinic staff shall actively market to, identify, and seek out patients for whom services are appropriate. Services shall only be performed at clinical locations throughout the hospital's service area that provide direct support to deployment health.

2. Labor Category. The contractor shall provide personnel from the following labor category:

Clinical Psychologist (One (1) individual)

3. Qualifications. The health care worker (HCW) shall possess and maintain the minimum qualifications stated in Section C of the basic contract. In addition, the HCW shall possess following qualification:

3.1. A minimum of one (1) year of full-time experience within the preceding three (3) years working with a patient population with Post Traumatic Stress Disorder.

4. Staffing and Scheduling.

4.1. The contractor shall provide one (1) individual Clinical Psychologist. To maintain continuity of services, the HCW MUST BE A FULL-TIME INDIVIDUAL. The Commanding Officer will not approve more than one (1) individual for the required position.

4.2. The Commanding Officer or designated government representative will supervise the HCW.

4.3. Hours of Operation. The Clinic operates between the hours of 0700 and 1900 and is not open for patient care on the day of observance of Federal holidays. Clinic hours may extend past the scheduled closing time to complete treatment of scheduled patients. Services will be provided at the Primary Care Clinic or the Operational Commands. The services will require that the HCW travel to or occupy spaces at a Military Treatment Facility (MTF), Branch Health Clinic, Regiment Aid Station (RAS), Battalion Aide Station (BAS) or other MTF facilities in order to coordinate with the deployment training schedule of the Command requesting services.

4.4. Services shall normally be provided Monday through Friday between the hours of 0600 and 1800. Clinic hours may extend past the scheduled closing time to complete treatment of scheduled patients. The HCW shall be scheduled for an eight and a half (8.5) to twelve (12) hour period including an uncompensated thirty (30) or sixty (60) minute meal break, depending on shift length. Individual shift hours may occasionally be extended to accommodate the travel team requirements or mass screenings; however in no instance will the shift exceed twelve (12) hours in length. The contract HCW may be required to occasionally work a Saturday shift to accommodate the deployment training schedule. The requirement for working on a Saturday will not exceed twelve (12) Saturdays per annum. The specific schedule for each two (2) week period will be provided one (1) month in advance by the government supervisor or designated representative. The HCW shall be on duty for eighty (80) hours every two (2) weeks, except during periods of approved leave and holidays. In no instance will the HCW be required to provide services (on-site service plus approved leave and holidays) in excess of eighty (80) hours per two (2) week period.

4.5. Services are not normally required on the day of observance of Federal holidays. Holiday compensation shall be provided in accordance with Section C of the basic contract.

4.6. The HCW will accrue eight (8) hours of personal leave (annual plus sick) for each eighty (80) hour period of services provided (on-site services plus approved leave and holidays). The HCW shall be subject to leave approval in accordance with Section C of the basic contract.

5. Treatment.

5.1. The HCW shall provide care for eligible patients resulting from deployment related screenings in accordance with the guidelines located at http://www.pdhealth.mil/sites/default/files/images/docs/DHCC_AR_2014.pdf. Emphasis will be on the treatment of mental health to include post-traumatic stress disorders and other combat related psychological symptoms. Family member care may be included.

5.2. The HCW shall support deployment related healthcare services, such as but not inclusive of the Periodic Health Assessment, Pre or Post Deployment Health Assessments, Deployment Health Screenings, and assess the results, treat, and refer patients for further treatment as appropriate. The HCW shall provide deployment health screening services to all service members either individually or in groups, at the DPC or at the Operational Unit as determined necessary.

5.3. The HCW shall work with MTF and other contract medical personnel to schedule initial and follow-on appointments or referrals for service members before they leave the clinic or the screening at the Line unit.

5.4. The HCW shall ensure that all service members that require follow-on appointments or referrals will have them before leaving the clinic.

5.5. The HCW assigned to the DPC and Branch Clinics may be utilized for walk-in or other scheduled appointments in support of other MTF clinics.

6. Duties. The HCW shall provide services within the scope of clinical privileges granted. In addition to the applicable duties provided in Section C of the basic contract, the HCW shall:

6.1. Perform clinical evaluations including administration and interpretation of psychological, neuropsychological, cognitive and personality tests, and provide clinical consultation regarding selection of psychological tests. The HCW may also be required to administer neuropsychological assessments and perform clinical evaluation and/or referral based on the neuropsychological assessment tool.

6.2. Communicate with family members and significant others in order to further the effectiveness of patient treatment and progress.

6.3. Become proficient with the use of the Navy and Marine Corps Public Health Center electronic methods of completing the Department of Defense (DD) Forms - An electronic or web-enabled DD Form 2900 (or Standard Form 600 if electronic or web-enabled access is not available) shall be utilized for post deployment health reassessments, an electronic or web-enabled DD Form 2795 shall be utilized for pre-deployment assessments, and an electronic or web-enabled DD Form 2796 shall be utilized for post deployment screenings. The HCW shall use the Standard Form (SF) 600 which is the primary form of documentation for medical encounters, and the (SF) 513, which is the primary consultation form.

6.4. Document all patient cases and ensure that patient encounters are captured in the workload in Composite Health Care System (CHCS)/Armed Forces Health Longitudinal Technology Application (AHLTA) under the Medical Expense Performance Reporting System (MEPRS) code designated by the Bureau of Medicine and Surgery (BUMED), Accounting Guidance, published July 2006 or subsequent revision.

6.5. Perform Mental Health screenings in association with the DD Form 2900 and other deployment related assessments and evaluations when required. When requested, assess the results, treat patients, and refer patients for further treatment as appropriate.

7. Orientation and Training.

7.1. The HCW may be required to complete on-line training or attend Post Traumatic Stress Disorder or Deployment Health related classes to meet Deployment Health program guidelines (http://www.pdhealth.mil/sites/default/files/images/docs/DHCC_AR_2014.pdf). The HCW may be required to complete additional Navy Medicine, Department of Defense (DOD), or Command identified training requirements. If authorized by the Commanding Officer, training related expenses and/or travel will be reimbursed according to Section B and in accordance with Section C and associated subparagraphs of the basic contract.

8. Travel. The HCW may be required to travel to Branch Health Clinics or Line Units within a thirty five (35) mile radius of the military treatment facility. The HCW may be required to periodically travel beyond thirty five (35) miles for training or to provide temporary services to outlying clinic locations. Lodging and per-diem rates will apply. The HCW will be reimbursed for travel and per diem according to Section B and in accordance with Section C and associated paragraphs of the basic contract. The HCW shall use car pools, or any other modes of transportation to reduce travel expenses. Requirements to travel to locations beyond a fifty (50) mile radius shall not exceed twelve (12) per annum. Travel shall not be conducted unless the appropriate funding is included on the task order or added by modification to Contract Line Item Number (CLIN) 0008 in Section B of the task order.

Sub Line Item Number (SLIN) 0002AD and Options Clinical Psychologist Primary Care and Mental Health Directorates Psychological Health and Traumatic Brain Injury Program (PH/TBI)

1. Site of Service. The Contractor shall provide personnel for services in support of Naval Medical Center Portsmouth, VA (NMCP). The primary site of service will be in the Mental Health Directorate; however, the health care worker (HCW) may be required to perform services at another location as directed by the government. The government will normally provide at least one (1) week advanced notice should such a change in work location be required. The government reserves the right to require the HCW to perform services at another location with minimal notice. If government notice occurs once the HCW is at their specific work location, the contractor will be compensated at the normal CLIN rate for the HCW’s travel time.

2. Labor Category. The contractor shall provide personnel from the following labor category:

Clinical Psychologist (One (1) individual)

3. Qualifications. The health care worker (HCW) shall possess and maintain the applicable qualification requirements given in Section C of the basic contract. In addition, the following qualifications are required:

3.1. Completion of one (1) year American Psychological Association (APA)-accredited Residency Program.

3.2. Possess experience as a Clinical Psychologist of at least two (2) years within the preceding three (3) years.

3.3. Provide two (2) letters of recommendation written within the last two (2) years attesting to the HCW's clinical skills. At least one (1) letter must be from a physician or psychologist who has personally observed and evaluated the HCW and who can attest to the HCW's clinical competence in the care of patients. The other letter may be from a clinic or hospital administrator, physician or nurse who has personal knowledge of the HCW's patient care skills in a similar clinical environment. Reference letters shall attest to the HCW's communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include name, title, phone number, date of reference, address and signature of the individual providing reference.

4. Staffing and Scheduling.

4.1. The contractor shall provide one (1) individual Clinical Psychologist. To maintain continuity of services, the HCW MUST BE A FULL-TIME INDIVIDUAL. The Commanding Officer will not approve more than one (1) individual for the required position.

4.2. The Commanding Officer or designated government representative will supervise the HCW.

4.3. Services shall normally be provided Monday through Friday between the hours of 0630 and 1900. The HCW will be scheduled for an eight and a half (8.5) hour shift to include an uncompensated thirty (30) minute meal break. Shifts will be on a rotated basis within the time ranges specified as clinical demand dictate. In no instance will the HCW exceed eighty (80) hours (on-site service plus approved leave and holidays) per two (2) week period. The specific hours shall be schedule one (1) month in advance by the supervisor. Any changes in the schedule shall be coordinated between the HCW and the Government. At the mutual agreement of the HCW and the Government, alternative schedules, such as a compressed work schedule, may be implemented. The Government reserves the right to change shift hours with seventy two (72) hours’ notice to accommodate patient care needs and mission requirements.

4.4. Services will not be required on the day of observance of Federal holidays. Holiday compensation shall be provided in accordance with Section C of the basic contract.

4.5. The HCW will accrue eight (8) hours of personal leave (annual plus sick) for each eighty (80) hour period of services provided (on-site services plus approved leave and holidays). The HCW shall be subject to leave approval in accordance with Section C of the basic contract.

5. Duties. The HCW shall provide services within the scope of clinical privileges granted. In addition to the applicable duties provided in Section C of the basic contract, the HCW shall:

5.1. Provide consultation, differential diagnosis, and treatment planning for all disorders defined by the Diagnostic and Statistical Manual for Mental Disorders to adults, including but not limited to:

* Organic mental disorders

* Psychotic disorders

* Schizophrenia

* Delusional disorders

* Mood disorders

* Anxiety disorders

* Somatoform disorders

* Psychoactive substance use disorders

* Sleep disorders

* Factitious disorders

* Impulse control disorders

* Psychological factors affecting physical condition

* Disorders usually first evident in infancy, childhood, or adolescence that manifest in an adult patient, such as eating disorders and gender-identity disorders

* Conditions not attributable to a mental disorder that are a focus of attention or treatment

* Sexual disorders

* Adjustment disorders

* Personality disorders

* Dissociative disorders

* Post-traumatic stress syndrome

5.2. Provide diagnostic and therapeutic procedures (includes but not limited to):

* Interviewing

* Major types of psychotherapy including: short term, long term, psychodynamic, family, marital, group, individual and behavioral therapy

* Community outreach (e.g., health promotion and command consultation)

* Mental status examination

* Crisis intervention

* Case management

* Medical discharge planning

* Psychosocial history taking

* Psychological test administration and interpretation

5.3. Provide behavioral health care for all enrollees in the patient centered Medical Home Port (MHP) using evidenced-based interventions and Clinical Practice Guidelines where appropriate for treating behavioral health problems.

6. Travel. The HCW shall adhere to the travel requirements in Section C.11 and subparagraphs in the basic contract. Travel shall not be conducted unless the appropriate funding is included on the task order or added by modification to Contract Line Item Number (CLIN) 0008 in Section B of the task order.

Sub Line Item Number (SLIN) 0002AE and Options Clinical Psychologist Psychological Health and Traumatic Brain Injury Program (PH/TBI)

1. Site of Service. The contractor shall provide personnel in support of Naval Medical Center Portsmouth, VA (NMCP). The primary site of services shall be located at the Mental Health Department.

2. Labor Category. The contractor shall provide personnel from the following labor category:

Shifts are assigned on a rotational basis within the time ranges specified as clinic demands dictate. At the mutual agreement of the HCW and the government, alternative schedules, such as a compressed work schedule, may be implemented; however, if the HCW agrees to a compressed work schedule, compensation will always be based on the applicable hourly rate for the position without compensation at an accelerated (overtime) rate. The HCW shall be on duty for eighty (80) hours every two (2) weeks, except during periods of approved leave and holidays. The specific schedule will be provided one (1) month in advance by the government supervisor or designated representative. Any changes in the schedule shall be coordinated between the HCW and the government. The government reserves the right to change shift hours with seventy two (72) hours’ notice to accommodate patient care needs and mission requirements.

3. Qualifications. The health care worker (HCW) shall possess and maintain the applicable qualification requirements given in Section C of the basic contract. In addition, the following qualifications are required:

3.1. Have at least two (2) years of post-graduate supervised experience as a clinical social worker within the last three (3) years.

3.2. Provide two (2) letters of recommendation written within the last two (2) years attesting to the HCW’s clinical skills. At least one (1) letter must be from a physician or supervisor who has personally observed and evaluated the HCW and who can attest to the HCW’s clinical competence in the care of patients. The other letter may be from a clinic or hospital administrator, physician or nurse who has personal knowledge of the HCW’s patient care skills in a similar clinical environment. Reference letters shall attest to the HCW’s communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include name, title, phone number, date of reference, address and signature of the individual providing reference.

4.1. The contractor shall provide one (1) full time LCSW. To maintain continuity of services, the HCW MUST BE A FULL-TIME INDIVIDUAL. The Commanding Officer will not approve more than one (1) individual for the required position.

4.2. The Commanding Officer or other designated government representative will supervise the HCW.

4.3. Services shall be provided Monday through Friday, between the hours of 0600 and 1900. The HCW shall normally provide services for either an eight and a half (8.5) or ten and a half (10.5) hour shift, to include an uncompensated thirty (30) minute or one (1) hour meal break depending on shift length. In no instance will the HCW exceed eighty (80) hours (on site services plus approved leave and holiday) per two (2) week period. Hours during the work week may vary according to Neonatal Intensive Care Unit (NICU) Prime and Specialty Clinics hours of operation. Specific hours and days shall be scheduled one (1) month in advance by the workplace supervisor. Any changes in the schedule shall be coordinated between the HCW and the Government. The Government reserves the right to change shift hours with seventy two (72) hours’ notice to accommodate patient care needs and mission requirements. The HCW may not exceed the hours in the applicable Contract Line Item Number (CLIN)/Sub Line Item Number (SLIN) as stated in Section B.

4.4. Services are not required on the day of observance of Federal holidays. Holiday compensation shall be provided in accordance with Section C of the basic contract.

4.5. The HCW will accrue eight (8) hours of personal leave (annual plus sick) for each eighty (80) hours worked. The HCW shall be subject to leave approval in accordance with Section C of the basic contract.

5. Duties. The HCW shall provide services within the scope of clinical privileges granted. In addition to the applicable duties provided in Section C of the basic contract, the HCW shall:

5.1. Assist in the development and implementation of educational programs. Take initiative in seeking out opportunities to present programs to meet the needs of service members and their families.

5.2. Provide crisis intervention for families upon admission to NICU environment (i.e. closing the "loop'' from parent/parents on labor and delivery (L/D) and baby in the NICU environment).

5.3. Provide community and hospital resources to baby's families particular to baby's needs. Assist family during hospitalization to take advantage of these resources.

5.4. Provide bereavement support, consultation and support to NICU families if a Bereavement Committee is established; serve as a member of that committee.

5.5. Serve as a member of the NICU Care Committee and Ethics Committee in the NICU.

5.6. Serve as leader/facilitator of parent support group, resource group for families, sibling group, or other groupings as family needs dictate.

5.7. Facilitate communication between NICU staff and families.

5.8. Serve as part of the interdisciplinary team for NICU babies.

5.9. Teach in NICU course for nursing and in other areas relevant to staff education.

5.10. Provide psychosocial assessment of needs of baby and family.

5.11. Provide family focused care.

5.12. Participate in the discharge planning process for babies.

5.13. Provide telephonic follow-up with families as needed post discharge.

Sub Line Item Number (SLIN) 0002AG and Options Licensed Clinical Social Worker (LCSW) Wounded, Ill, and Injured (WII)

1. Site of Service. The contractor shall provide personnel for services in support of Naval Medical Center Portsmouth, VA (NMCP). The primary site of service will be in the Mental Health Directorate, Psychiatry Department, and Psychiatric Intensive Outpatient Program (PIOP).

2. Labor Category. The contractor shall provide personnel from the following labor category:

LCSW (One (1) individual)

3. Qualifications. The health care worker (HCW) shall possess and maintain the applicable qualification requirements given in Section C of the basic contract. In addition, the following qualifications are required:

3.1. Provide two (2) letters of recommendation written within the last two (2) years attesting to the HCW’s clinical skills. At least one (1) letter must be from a physician or social work supervisor who has personally observed and evaluated the HCW and who can attest to the HCW’s clinical skills in the care of patients. The other letter may be from a clinic or hospital administrator, physician or nurse who has personal knowledge of the health care worker’s patient care skills in a similar clinical environment. Reference letters shall attest to the HCW’s communication skills and ability to relate to patients as well as professional and other interpersonal skills among staff members and must include name, title, phone number, date of reference, address and signature of the individual providing reference.

3.2. Have a minimum of two (2) years of post-masters experience as a clinical social worker within in the last three (3) years.

4.4. As part of the HCW’s schedule, services may be required on the day of observance of Federal holidays. Should the HCWs be required to work on a holiday, they shall receive another day off as compensation within the following fourteen (14) calendar days.

4.5. The HCW will accrue eight (8) hours of personal leave (annual plus sick) for each two (2) weeks worked. The HCW shall be subject to leave approval in accordance with Section C of the basic contract.

5. Duties. The HCW shall provide services within the scope of clinical privileges granted. In addition to the applicable duties provided in Section C of the basic contract, the HCW shall:

5.1. Provide behavioral health care for all enrollees in the patient centered Medical Home Port (MHP) program.

5.2. Provide training and/or technical direction as applicable to supporting Government employees (i.e. hospital corpsmen, aides, students) assigned to the HCW during the performance of social work services. The HCW shall perform limited administrative duties that include maintaining statistical records of the HCW’s clinical workload, participating in education programs, and participating in clinical staff Performance Improvement Functions and Process Action Teams, as prescribed by the Commander.

5.3. Participate in monthly meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist.

5.4. Participate in the provision of monthly in service training to non-healthcare practitioner members of the clinical and administrative staff on subjects germane to social work services. This shall include responsibility for developing and implementing psycho educational programs, community relations, information and referral.

5.5. Independently function as the primary mental health therapist, providing assessment and therapeutic interventions in outpatient settings utilizing clinical modalities exercising mature professional judgment and utilizing a wide range of social work skills to include individual, family, couples, and group counseling to assist patients and their families in dealing with acute, routine and chronic mental health conditions.

5.6. Conduct comprehensive developmental, behavioral, educational/vocational, medical, psychiatric and/or psychosocial assessments to determine patient/family needs and resources (both family support and community support). Provide counseling to patient and/or family in matters directly related to identify limitations, adjustment to psychiatric/behavioral condition and ongoing treatment. Develop and implement plan of treatment based upon clinical assessment of patient or family's diagnosis, current level of functioning, stated goals and necessity of support services.

6. Travel. The HCW shall comply with the travel requirements in Section C and subparagraphs in the basic contract. Travel shall not be conducted unless the appropriate funding is included on the task order or added by modification to Contract Line Item Number (CLIN) 0008 in Section B of the task order.

Sub Line Item Numbers (SLINs) 0002AH, 0002AJ, 0002AK, 0002AL, 0002AM, and Options Licensed Clinical Social Worker (LCSW) Psychological Health and Traumatic Brain Injury Program (PH/TBI)
