Attachment 7: Past Performance Reference Sheet

1. POC information: Name, phone number, and email address of a point of contact at the federal, state, local government or commercial entity for which the contract was performed. Contractor shall verify the validity of the contact information provided. (The POC must have firsthand knowledge of the contract).

Answer “Has the POC agreed to be to be contacted as a reference?”

2. Contract Number and Task Order number(s), if applicable.

3. Name of contractor to whom the contract was awarded. Name of contractor who performed services, if different from awardee. Provide the CAGE Code and DUNS numbers of both the awardee and the contractor who performed the service(s).

a. What type of services were provided?

b. What tasks specifically were provided?

8. MAGNITUDE OF EFFORT:

a. What labor categories / positions were provided?

b. What was the level of effort provided (total number of employees and number of FTE’s and PTE’s per position)?

9. COMPLEXITY:

a. Did the contractor conduct a similar volume of inspections as is projected by the PWS?

b. Were the inspection sites located over a wide geographical area (such as in the PWS)?

c. Did the contractor deal with sensitive information (could be trade secrets, PII or sensitive Government information)?

10. Other Relevant Information:

11. The offeror shall provide information on problems encountered on the identified contract(s) and the contractor’s corrective actions.
