ANSWERS TO QUESTIONS 0002

Questions:

1. I noticed that the Form SF1449 is missing. Is it a requirement as stated under "Instructions to Offers” No. 11 or can I submit my proposal on company letterhead with all the required information mentioned?

Answer:

Refer to INSTRUCTIONS TO OFFERORS on Page 14. This will better instruct you as opposed to Clause 52.212-1 on page 20.

2. Do you have a specific Past Performance form you want filled out or can I send ones from previous request on other solicitations?

Answer:

Please refer to Enclosure 1 – Past Performance Information Sheet on the last page (page 56).

3. In regards to Statement of Requirements (page 17) #2 it states as follows: "In addition to manufacturing a wide variety of implant systems the contractor shall warrant not only their abutment but the implant replacement cost if the implant fails while using their abutment." Does this mean that the contractor who manufactured the abutment is responsible for all "surgical costs" performed by a dentist surgeon for the removal, replacement, and the new dental implant in the event the implant fails intra-orally?

Answer:

Yes

4. What type of digital impression technology does the dental clinic use; Trios or Itero?

Answer:

Currently, the dental clinic does not have an established digital impression technology; such a selection is approximately 1 year out from being established.

5. Are there any models involved? In other words, when the abutment is returned to dental clinic, is the contractor expected to deliver a digitally manufactured model?

Answer:

The Area Dental Lab will send the outsource lab the soft tissue models and the outsource lab will scan, design and mill the abutments. Once the lab completes that abutment they will FEDEX that abutment and dental models back to the Area Dental Lab. The outsource lab will not deliver a digitally manufactured model to the Area Dental Lab.
