ADDTIONAL QUESTIONS

Question: CLIN 0001AB does not appear to contain Option 2. Was that intentional?

Answer: All of the CLINs have Option 2. However, Option 2 was mislabled as Option 1. The pricing sheet has been revised.

Question: LOT I - CLIN 2001IF - The Quantity is 3120 for the ending stub period which is more than the hours for the FULL Year and this is a part time position. Probably should be 312?

Answer: The pricing sheet has been corrected to reflect the appropriate amount of hours.

Question: While the language in the RFP is very specific regarding font type, style, and margins on each page, we are unable to find any specification regarding the size requirements for the pages of the proposal (i.e., no reference to 8 ½ x 11, 11 x 17, etc.). Please confirm that there are no restrictions as to size of each page of the proposal and that both standard 8 ½ x 11 and 11 x 17 inch pages are acceptable.

Answer: Proposals should be submitted on 8 ½ X 11 inch page size. Sections L.2.1.3 and L.2.1.4 have been revised.

Question: We have encountered a couple issues with Tab A of the pricing workbook. First, the “blended rate explanation” cell is locked and will not allow the offerors to enter required information. Additionally, cell E46 on Tab A does not accurately capture all of the costs associated with CLIN 0001AB and all options.

Answer: The pricing sheet has been revised.

SECTION L

Failure to comply with any of the instructions may result in the proposal being removed from further consideration.

L.1. SUBMISSION OF COST OR PRICING DATA.

L.1.1. It is expected that this contract will be awarded based upon a determination that there is adequate price competition; therefore, the offeror is not required to submit additional cost or price data (beyond that required by Section L.2) or to certify cost or pricing data with its proposal.

L.1.2. If, after receipt of the proposals, the Contracting Officer determines that adequate price competition does not exist in accordance with FAR 15.403-3 and FAR 15.403-4, the offeror shall provide other information requested to be submitted to determine fairness and reasonableness of price, or certified cost or pricing data as requested by the Contracting Officer.

L.2. PROPOSAL CONTENT AND INSTRUCTIONS FOR PREPARATION OF PROPOSALS.

L.2.1. Introduction and Purpose - This section specifies the format and content that offerors shall use in this Request for Proposal (RFP). The intent is not to restrict the offerors in the manner in which they will perform their work but rather to ensure a certain degree of uniformity in the format of the responses for evaluation purposes. Offerors must submit a proposal that is legible and comprehensive enough to provide the basis for a sound evaluation by the Government. Information provided should be precise, factual, and complete. Legibility, clarity, completeness, and responsiveness are important. Proposals shall be in the form prescribed by, and shall contain a response to, each of the areas identified in Section L.2. Any proposal that does not meet the requirements of the solicitation may be determined to be substantially incomplete and may result in rejection of the offeror's entire proposal.

L.2.1.2. The proposal shall be submitted in three separate volumes:

Volume I – Past Performance

Volume II – Technical

Volume III – Business

A complete Volume I - Past Performance; Volume II -Technical; and Volume III - Business; shall be submitted at the same time to the electronic mail (e-mail) address in Paragraph L.3.3. by the closing time and date referenced in Block 9 of the SF 33. All three Volumes shall be included as attachments within one e-mail from the offeror. If more than one e-mail is received per offeror, the entire proposal will be not be considered. No further consideration will be given to any offeror who submits any of these volumes late in accordance with FAR 15.208(b).

L.2.1.3. The Past Performance and Technical volumes (Volumes I and II, respectively) shall be in Microsoft Word for Windows (2007)(.docx), on 8 ½ X 11 inch size, with 1 inch margins all around, Times New Roman font of not less than 10 point font size inclusive of tables, charts, graphs, graphics, etc. Text, tables, charts, graphs, graphics, etc. that are not in compliance will not be considered by the Government in its evaluation. Each page of each document shall have a footer indicating the name of the offeror and “page X of Y.”

L.2.1.4. The total number of the pages submitted for Volume I (Past Performance) shall not exceed 6 (8 ½ X 11 inch) pages, excluding a title page and consent letters. If Volume I contains more than 6 pages, only the first 6 pages will be evaluated. The total number of the pages submitted for Volume II (Technical) shall not exceed 10 (8 ½ X 11 inch) pages, excluding a title page and table of contents. If Volume II contains more than 10 pages, only the first 10 pages will be evaluated. Cover letters are not required for either volume; any information contained in a cover letter will not be evaluated.

L.2.2. Volume I: Past Performance. In accordance with electronic submission requirements given in this section, the offeror shall include the following:

L.2.2.1. The offeror shall complete a maximum of six Past Performance Information Sheets (Section L Enclosure 1). Each of the Past Performance Information Sheets shall be completed in its entirety. The Past Performance Information listed on each sheet shall be for clinical dental services. If more than 6 Past Performance Information Sheets are provided, only the first six will be considered. Each Past Performance Information Sheet shall identify no more than one Point of Contact (POC). Each Past Performance Information Sheet may include multiple contracts or one or more task orders under single IDIQ contracts. The same contract may appear on more than one Past Performance Information Sheet if more than one POC exists. If more than one POC is provided for any Past Performance Information Sheet, only the first POC will be considered. Each Past Performance Information Sheet shall not exceed one page. If a Past Performance Information Sheet contains more than one page, only the first page will be considered. Contracts cited must be current, or have ended not more than three years prior to the closing date of the solicitation. If the contract performance ended more than three years prior to the closing date of the solicitation it will be determined not relevant.

L.2.2.2. The offeror may submit past performance that demonstrates prior experience of key personnel. The past performance submission shall identify, by name, the key personnel and clearly identify their previous roles and responsibilities and their proposed roles and responsibilities under the resultant contract. If the submission does not clearly describe the roles and responsibilities of the key personnel and the how the key personnel will contribute to the performance under the resultant contract, it will be determined not relevant.

L.2.2.3. The offeror may submit past performance that demonstrates prior experience by teaming partners (e.g. proposed subcontractor, partner, mentor, protégé, parent company, sister company, etc.) only if they will be performing in support of the contract resulting from this solicitation. Such past performance submissions shall clearly identify the relationship of the teaming partners to the offeror and the how they will contribute to the performance under the resultant contract. If the submission does not clearly describe the relationship of teaming partners to the offeror and the how the teaming partners will contribute to the performance under the resultant contract, it will be determined not relevant. A maximum of three Past Performance Information Sheets shall be for services provided by entities other than the offeror, members of SBA approved Joint Ventures, and key personnel.

L.2.2.4. If Past Performance is submitted for a contract under which the offeror or teaming partner provided services as a subcontractor, partner, mentor, protégé, parent company, sister company, etc., the Past Performance Information Sheet shall clearly describe the corporate relationships among those who provided the services, citing key responsibilities for each, to include identifying the specific services provided (in terms of numbers and types of personnel, locations, etc) and nature of services provided (e.g., recruitment, payroll, benefits administration, etc). If the submission does not clearly detail the specific responsibilities and relationship under a contract, it will be determined not relevant.

L.2.2.5. If a teaming partner is proposed, the offeror shall submit written consent from that teaming partner to disclose their Past Performance information to the contractor. If such consent is not provided as part of the submission, the Past Performance of that teaming partner will be determined not relevant.

L.2.2.6. If neither the offeror nor its teaming partner possesses relevant past performance, the offeror shall submit a statement affirming that it possesses no relevant Past Performance.

L.2.2.7. The Government may contact the POCs listed on the Past Performance Information Sheets. POCs shall be either Government personnel (civil service or military) or employees of private sector dental/medical facilities for which the contractor has provided services. Past Performance Information Sheets that list POCs that work for, or have worked for, the offeror or with a teaming partner(s) directly or indirectly will be determined not relevant.

L.2.3. Volume II: Technical (Management Plan and Market Research). In accordance with electronic submission requirements given in this section, the offeror shall include the following:

L.2.3.1. The offeror shall discuss the corporate personnel who will be responsible for contract start-up and the ongoing administration of all key functional areas, such as but not limited to recruitment, scheduling, human resources, program management, security, and credentialing. The offeror shall list the name, title, and employer (i.e. offeror or teaming partner) of all key corporate personnel. The plan shall describe the range of responsibilities for each individual and should discuss how the qualifications and experience of each individual will contribute to successful contract operations.

L.2.3.2. The offeror shall discuss their knowledge of the marketplaces represented in the solicitation and detail the sources and methodologies through which they will recruit and retain required health care workers. Further, the offeror shall discuss their knowledge of the marketplaces represented in the solicitation and discuss how marketplace conditions (such as, but not limited to scarce labor categories or remote locations) will impact the ability to recruit and retain required health care workers. Without revealing the prices submitted in response to this solicitation, the offeror shall demonstrate market research and discuss how it has applied the marketplace knowledge in development of the proposal.

L.2.3.3. If a subcontractor has been proposed, the contractor shall specifically address FAR 52.219-14, Limitations on Subcontracting; the offeror’s Technical volume shall clearly detail the nature of the relationship between the parties, (i.e. subcontractor, partner, etc) and shall address the responsibilities of all parties as they will relate to provision of services under the resultant contract.

L.2.3.4. If the offeror is proposing as a mentor/protégé or business relationship other than subcontracting, the offeror’s Technical volume shall clearly detail the nature of the relationship between the parties, (i.e. mentor-protégé, joint venture, etc) and shall address the responsibilities of all parties as they would relate to provision of services under the resultant contract.

L.2.3.5. Offerors are cautioned not to include any pricing or cost information in any portion of the Technical volume. Note that this exclusion does not apply to presentation of the results of the offeror's recruitment market survey or price information for past contracts.

L.2.4. Volume III: Business. In accordance with file submission requirements given in this section, the offeror shall include the following:

L.2.4.1. Standard Form (SF) 33. This form will be provided to the offeror electronically as part of the solicitation package. It shall be appropriately completed, inclusive of the offeror's name, address, and required Commercial and Government Entity (CAGE) codes, signed and dated. The offeror's identity will be determined by the name submitted on the SF 33. It is not necessary to submit the entire solicitation.

NOTE: The blank space contained in Block 12 of the SF 33 should read 365 calendar days.

L.2.4.2. Amendments. Each amendment shall be acknowledged through instructions in Block 11 of the SF 30. Signed copies of the cover page are acceptable. It is not necessary to submit the entire amendment. The solicitation and all amendments will be posted to the Federal Business Opportunities website (FBO) at https://www.fbo.gov/ . It is the offeror’s sole responsibility to periodically check the website for amendments and to ensure that all amendments issued prior to the closing date are acknowledged in accordance with instructions in Block 11 of the Standard Form (SF) 30.

L.2.4.3. Section K - Representations, Certifications and Other Statements of Offerors of this Solicitation.

L.2.4.3.1. The offeror must be registered in Online Representations and Certifications Application (ORCA) in accordance with Section K, FAR 52.204-8, and DFARS 252.204-7007. The offeror shall verify its ORCA is current, accurate, complete and applicable to this solicitation as of the date of this offer and is incorporated in this offer by reference. The offeror shall also be registered in ORCA as a Small Business under the applicable NAICS code for this solicitation, 621210. The small business size standard is $7M. (Please note that the Online Representations and Certifications Application (ORCA) has been replaced with the System for Award Management (SAM) https://www.sam.gov/portal/public/SAM/). No documentation is required to be submitted in response to this paragraph.

L.2.4.3.2. The offeror shall complete and submit DFARS clause 252.209-7994 [Representation By Corporations Regarding An Unpaid Delinquent Tax Liability or a Felony Conviction Under Any Federal Law-Fiscal Year 2014 Appropriations (Deviation 2014-O0004) (OCTOBER 2013), (Refer to Section K, Enclosure 1)].

L.2.4.4. Offeror’s Information Form located in Section L, Enclosure 2 shall be completed and submitted. The offeror shall fill in every field on the form for it to be considered complete. Please note that all communication regarding this solicitation and proposals will be directed to the designated contacts on the form.

L.2.4.5. Pricing (offeror’s hourly rates) and supplemental pricing (health care workers’ compensation rates) shall be submitted on a spreadsheet. An electronic file will be posted to the Federal Business Opportunities web site (https://www.fbo.gov/) along with the solicitation and any amendments for downloading. The file will be named “Pricing Sheet for N62645-14-R-0001.xlsx”. The offeror shall complete the pricing worksheet and supplemental pricing worksheets required on all tabs included in the electronic spreadsheet. Blue boxes designate those fields into which the offeror can enter data. The completed file shall be submitted in the e-mailed proposal with the Volume III – Business attachment. The file shall be renamed: [name of offeror] pricing sheet.xlsx. Please note that any reformatting of the pricing spreadsheet or failure to follow the naming conventions provided in this section may cause a serious delay in the evaluation process and may result in rejection of the offeror's proposal.

L.2.4.5.1. Pricing and supplemental pricing for this solicitation represents minimum quantities plus two option periods (lots). Lots for this requirement are outlined in the electronic pricing spreadsheet and Enclosures 3-11. Enclosures 3-11, located in Section L, contain the statements of work for each lot. The offeror shall submit pricing and supplemental pricing for each lot on the electronic pricing spreadsheet.

L.2.4.5.2. The information contained in Enclosures 3-11 is provided to assist in the development of pricing and supplemental pricing for the CLINs. These enclosures shall not be submitted with an offeror’s business volume; it is for informational purposes only. Pricing and supplemental pricing shall be completed in accordance with Section L.2.4.5.

L.3. FILE SUBMISSION. Offerors are responsible for submitting their proposals in sufficient time to allow them to reach NMLC by the time specified in this solicitation. To avoid the possibility of late receipt, whether caused by high system usage or any potential technical and/or input problems involving the applicant’s own equipment, it is strongly recommended that proposals or other required preaward documents be submitted electronically to NMLC not later than 5:00 PM one working day prior to the deadline established in this solicitation.

L.3.1. No hard copies of proposals will be accepted.

L.3.2. The Past Performance, Technical, and Business volumes shall be submitted by e-mail only. Please note only 1 e-mail per offeror will be accepted. The e-mailed proposal (to include all volume attachments) shall not be larger than 10 megabytes and shall be in Microsoft Office for Windows format (2007), either .docx or .xlsx files, as specified herein. If the e-mail is larger than 10 mega bytes, it will not be accepted. Consent letters and forms that require signatures may be submitted as a .pdf file and included with the corresponding volume attachment.

L.3.3. Each e-mailed proposal shall have the name of the offeror and the solicitation number in the subject line. Any requests for hand delivery will not be honored. It is the Offeror's responsibility to ensure that proposals are received by the due date and time required. The Government will acknowledge receipt of all proposals. If an acknowledgement is not received, it is the Offeror’s responsibility to follow-up with NMLC to guarantee receipt.

The e-mail address for proposal submission is:

nmlc-matrix@med.navy.mil L.3.4. The offeror is responsible for ensuring that proposals do not contain corrupted files such that they are not readable by the Government.

L.3.5. The Government is explicitly raising the prescribed file content and submission requirements from what might be argued as a matter of "form", to one that is materially substantive in that it reflects upon the offeror's ability and willingness to follow instructions and fulfill its obligations. Upon receipt of proposals, and prior to actively engaging in proposal evaluation, a compliance review will be conducted to determine if each offeror met the submission requirements for each volume as outlined in sections L.2. and L.3. In order to be eligible for award, a proposal must comply in all material respects with the terms and conditions set forth in this solicitation. Failure to properly submit any items listed in sections L.2 and L.3 may result in rejection of the offeror's entire proposal.

L.4. PROPOSAL EVALUATION. Proposals will be evaluated in accordance with the evaluation criteria as outlined in Sections H for Lot Awards and M for Contract Awards.

L.5. QUESTIONS. Questions should be presented in writing to NMLC no later than 10 calendar days after the issue date (Block 5 of SF33) of this solicitation via email to nmlc-matrix@med.navy.mil. Questions should be listed chronologically and reference the corresponding solicitation section and paragraph. NMLC’s responses to questions will be presented in the form of an amendment. The source of the questions will not be disclosed.

L.6. REVIEW OF AGENCY PROTESTS. The contracting activity, NMLC, will process agency protests in accordance with the requirements set forth in FAR 33.103(d).

L.6.1. Pursuant to FAR 33.103(d)(4), an agency protest may be filed directly with the appropriate reviewing authority; or a protester may appeal a decision rendered by a Contracting Officer to the appropriate reviewing authority.

L.6.2. The reviewing authority for the Contracting Officer is the Director of Acquisition Management, NMLC, Code 02, 693 Neiman Street, Fort Detrick, MD 21702-9203. Agency procurement protests should clearly identify the initial adjudicating official, i.e., the, “Contracting Officer” or the, “Reviewing Official”.

L.6.3. Offerors should note this review of the Contracting Officer’s decision will not extend GAO’s timeliness requirements. Therefore, any subsequent protest to GAO must be filed within 10 days of knowledge of initial adverse agency action.

L.7 MISCELLANEOUS

L.7.1 Offerors shall not contact any incumbent health care workers during their official duty hours.

L.7.2. In accordance with the Naval Marine Corps Acquisition Regulation Supplement (NMCARS) 5232.903, for Prompt Payment Act purposes, contracts will be subject to the 7 calendar day constructive acceptance period.

L.7.3. In accordance with FAR 52.232-18, funds are not presently available for the resulting contracts. The Government's obligation under this solicitation is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for the resulting contracts and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer via the award of a task order.

Enclosure 3 – Lot A

STATEMENT OF WORK

Dental Hygienists

2nd Dental Battalion/Naval Dental Center, Camp Lejeune, NC

And associated clinics

(CLINs 0001AA, 0001AB, 0001AC, 0001AD, and 0001AE)

1. Labor Category.

1.1. The contractor shall provide personnel from the following labor category:

Dental Hygienists* (12 full-time individuals and 2 part-time individuals)

*Wage Determination

2. Locations. The contractor shall provide 12 full-time dental hygienists and 2 part-time dental hygienists in support in support of the 2nd Dental Battalion/Naval Dental Center, Camp Lejeune, NC. Each healthcare worker (HCW) will be assigned to a primary location; however, the government retains the right to assign the HCWs to another location within a 50-mile commuting radius of their assigned MTF/DTF. The HCWs shall receive notification two weeks prior to reassignment to locations within a 50-mile limit. The government will consider the use of CLIN 0006 to reimburse HCWs for reassignments within a 50-mile limit on a case-by-case basis. Services shall be provided in the Dental Clinics located at H-1, Cherry Point, French Creek, Mainside, Osborne, and other surrounding clinics of the Naval Dental Center Camp Lejeune, NC, as follows:

H-1– 2 full-time individuals

Cherry Point – 3 full-time individuals and 2 part-time individuals

French Creek– 3 full-time individuals

Mainside – 2 full-time individuals

Osborne - 2 full-time individuals

3. Scheduling and Duty Hours.

3.1. Each full-time HCW shall normally be on duty for 80 hours per two-week. Services shall normally be provided for an 8.5 or 9 hour period (to include a 30 minute or 1 hour uncompensated meal break, depending on shift length) each day. Services shall be provided between the hours of 0600 and 1800 on each day scheduled, Monday through Friday.

3.2. Each part-time HCW shall normally be on duty for 40 hours per two-week period. Services shall normally be provided for a 4 hour period (no meal break) or an 8.5 or 9 hour hour period (to include a 30 minute or 1 hour uncompensated meal break, depending on shift length) each day. Services shall be provided between the hours of 0600 and 1800 on each day scheduled, Monday through Friday.

3.3. Specific days, hours, and shift length will be scheduled by the Commanding Officer or designee. Any changes in the schedule shall be coordinated between the HCW and the government.

3.4. In order to maintain continuity of services, services shall normally be provided by full-time time individuals. However, on a case-by-case basis, the government may permit more than one individual to fill a position. These specific positions will be identified after award. No more than two individuals are permitted to fill each position.

3.5. The Commanding Officer or designated government representative will supervise the HCWs.

4. Holidays and Leave.

4.1. Leave will be accrued by the quarter hour (.25) for total hours worked during the pay period. The HCWs will accrue 8 hours of personal leave at the end of every 80 hour period worked. Planned absences from assigned duties shall be requested with 15 working days advance notice, in writing, to the HCW’s supervisor. Personal Leave accrues to the individual actually providing services. The HCWs shall be compensated by the Contractor for these periods of authorized planned absence. Leave shall be managed in accordance with Section C.3 of the basic contract.

4.2. Federal Holidays. Services of the HCWs shall not be required on the day of observance of federally established holidays. Federal holidays shall be managed in accordance with Section C.3. of the basic contract.

4.3. Non-Service Work Days.

4.3.1. Services of the HCWs shall not be required on the day of observance of local base holidays (i.e. Easter). Additionally, services may not be required on the workdays immediately preceding, and/or following federally established holidays and local base holidays as stated in the Commanding Generals, II Marine Expeditionary Forces message addressing Holiday Routine Observances for CY2016 and following years. This message is updated and published annually to identify Holiday Routine Observances and non-service/non-paid workdays.

4.3.2. Services may not be required on safety stand down days.

4.3.3. Use of leave is not authorized on non-service workdays.

4.3.4. The COR will provide a copy of the schedule referencing non-service/non-paid workdays (when the message is released) on an annual basis, or as necessary (See Attachment I).

2. Locations. The contractor shall provide dental assistant services in support in support of the 2nd Dental Battalion/Naval Dental Center, Camp Lejeune, NC. Services shall be provided in the clinics located at Annex Camp Geiger, Annex Camp Johnson, Annex Hadnot Point/65, Annex Caron, French Creek, New River, Mainside, and other surrounding clinics of the Naval Dental Center Camp Lejeune, NC as follows:

Annex Camp Geiger – 2 individuals

Annex Camp Johnson – 3 individuals

Annex Hadnot Point/65 – 5 individuals

Annex Caron – 5 individuals

French Creek – 13 individuals

New River – 10 individuals

Mainside – 15 individuals

3. Scheduling and Duty Hours.

3.1. Each healthcare worker (HCW) may work an alternate work schedule not to exceed a 70 hours per two week period. Services shall not exceed an 8 hour period to include a .5 or 1 hour uncompensated meal break, depending on shift length. Services shall be provided between the hours of 0600 and 1800 on each day scheduled, Monday through Friday. Specific days, hours, and shift length will be scheduled by the Commanding Officer or designee. Any changes in the schedule shall be coordinated between the HCW and the government.

3.2. In order to maintain continuity of services, services shall normally be provided by full-time time individuals. However, on a case-by-case basis, the government may permit more than one individual to fill a position. These specific positions will be identified after award. No more than two individuals are permitted to fill each position.

3.3. The Commanding Officer or designated government representative will supervise the HCWs.

4. Holidays and Leave.

4.1. Leave will be accrued by the quarter hour (.25) for total hours worked during the pay period. Each HCW will accrue 7 hours of personal leave at the end of every 70 hour period worked. Planned absences from assigned duties shall be requested 15 working days in advance, in writing, to the Clinic Supervisor. Personal Leave accrues to the individual actually providing services. The HCWs shall be compensated by the Contractor for these periods of authorized planned absence. Leave shall be managed in accordance with Section C.3 of the basic contract.

4.2. Federal Holidays. Services of the HCWs shall not be required on the day of observance of federally established holidays. Federal holidays shall be managed in accordance with Section C.3. of the basic contract.

4.3. Non-Service Work Days.

4.3.1. Services of the HCWs shall not be required on the day of observance of local base holidays (i.e. Easter). Additionally, services may not be required on the workdays immediately preceding, and/or following federally established holidays and local base holidays as stated in the Commanding Generals, II Marine Expeditionary Forces message addressing Holiday Routine Observances for CY2016 and following years. This message is updated and published annually to identify Holiday Routine Observances and non-service/non-paid workdays.

4.3.2. Services may not be required on safety stand down days.

4.3.3. Use of leave is not authorized on non-service workdays.

4.3.4. The COR will provide a copy of the schedule referencing non-service/non-paid workdays (when the message is released) on an annual basis, or as necessary (See Attachment I).

2. Locations. The contractor shall provide 14 individual dental hygienists in support in support of the 2ndDental Battalion/Naval Dental Center, Camp Lejeune, NC. Each HCW will be assigned to a primary location; however, the government retains the right to assign the HCWs to another location within a 50-mile commuting radius of their assigned MTF/DTF. The HCWs shall receive notification two weeks prior to reassignment to locations within a 50-mile limit. The government will consider the use of CLIN 0006 to reimburse HCWs for reassignments within a 50-mile limit on a case-by-case basis. Services shall be provided in the dental clinics located at Carron, Cherry Point, French Creek, Hadnot Point, New River, Osborne, and other surrounding clinics. Services shall be provided as follows:

Carron Dental Clinic - 2 individuals

Cherry Point Dental Clinic – 2 individuals

French Creek Dental Clinic– 2 individuals

Hadnot Point Dental Clinic –3 individuals

New River Dental Clinic – 3 individuals

Osborne Dental Clinic – 2 individuals

2. Location. The contractor shall provide one individual dental hygienist in support in support of the Naval Hospital Camp Lejeune, NC.

3. Scheduling and Duty Hours.

3.1. The healthcare worker (HCW) shall normally be on duty for 40 hours per week. Services shall be required for a period of 8.5 to 9 hours with an uncompensated 30 minute to 1-hour meal break, between the hours of 0600-1800, Monday through Friday. Specific days, hours, and shift length will be scheduled by the Commanding Officer or designee. Any changes in the schedule shall be coordinated between the HCW and the government. Unless otherwise specified, services required by an individual HCW shall not exceed 80 hours per two-week period.

3.2. In order to maintain continuity of services, services shall normally be required by full-time individuals. However, on a case-by-case basis, the government may permit more than one individual to fill a position. The specific position will be identified after award. No more than two individuals are permitted to fill the position.

3.3. The Commanding Officer or designated government representative will supervise the HCW.

The HCW shall have the option to take leave or be placed in a Leave without Pay (LWOP) status during these clinic closures.
