SUMMARY OF CHANGES
SECTION SF 30 - BLOCK 14 CONTINUATION PAGE
The following have been added by full text:
ADDITIONAL OFFERORS QUESTIONS
1. In the Q&A posted, question 6 says, " 6. Is this related to contract numbers N62645-12-D-5016 through 5020? Yes, refer to Question 4 above." However, those contract numbers are NOT listed in Question 4. My assumption is that they are NOT related to this opp. Is this a correct assumption?
Solicitation N62645-13-R-0024 is not related to N62645-12-D-5016 through 5020.
2. For clarification, N62645-13-R-0024 is REPLACING the contracts listed in Question 4?
Yes, the resultant contracts of N62645-13-R-0024 will replace the contracts listed in Question 4 of Amendment 0001.
3. Para K.2. on page 85 specifies completing and submitting DFAR 252.209-7995, which is correctly enclosed in the Solicitation at Section K, Enclosure 1. However, Para L.2.4.3.2. on page 96 specifies that the offeror shall complete and submit DFARS clause 252.209-7997.
The DFARs clause in Paragraph L.2.4.3.2. has been corrected to reflect DFARs 252.209-7995.
4. Can you please the current billable rate for the Occupational Health Physician, as it was newly added and was not listed with the rates given in the original solicitation?
Attachment AE has been updated to include Occupational Health Physician

L.2. PROPOSAL CONTENT AND INSTRUCTIONS FOR PREPARATION OF PROPOSALS.
L.2.1. Introduction and Purpose - This section specifies the format and content that offerors shall use in this Request for Proposal (RFP). Offerors must submit a proposal that is legible and comprehensive enough to provide the basis for a sound evaluation by the Government. Information provided should be precise, factual, and complete. Legibility, clarity, completeness, and responsiveness are important. Proposals shall be in the form prescribed by, and shall contain a response to, each of the areas identified in Section L.2. Any proposal that does not meet the requirements of the solicitation may be determined to be substantially incomplete and may result in rejection of the offeror's entire proposal.

L.2.1.4. The total number of the pages submitted for Volume I (Past Performance) shall not exceed 6 pages, excluding a title page and consent letters. If Volume I contains more than 6 pages, only the first 6 pages will be evaluated. The total number of the pages submitted for Volume II (Technical) shall not exceed 10 pages, excluding a title page and table of contents. If Volume II contains more than 10 pages, only the first 10 pages will be evaluated. Cover letters are not required for either volume; any information contained in a cover letter will not be evaluated.
