SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
SECTION SF 30 - BLOCK 14 CONTINUATION PAGE
The following have been added by full text:
QUESTIONS/ANSWERS
1. Can you tell me if these services are currently (or previously) being performed by a contractor? If so, could you please provide the Incumbent name as well as the contract number?
Previous NMLC contracts included services from all seven labor bands. NMLC has decided to take a new approach by creating distinct MATO contracts for the three industry standard labor segments (Ancillary, Nursing, and Physician). As a result of this realignment, this acquisition is considered a new contract and not a follow-on contract to the existing contracts providing Ancillary services.
However, the majority of Ancillary services required at the locations contained in the solicitation are currently being provided under the following MATO contracts.
N62645-09-D-5000 Casepro, Inc.
N62645-09-D-5001 Catalyst Professional Services, Inc.
N62645-09-D-5002 Cherokee Medical Services, LLC
N62645-09-D-5003 Chesapeake Educational Services
N62645-09-D-5004 OMV Medical
N62645-09-D-5005 Professional Performance Development Group
N62645-09-D-5006 Saratoga Medical Center
N62645-09-D-5007 TCMP Staffing Services
2. If this is a new requirement, is there an estimated dollar value, or NTE value associated with this requirement?
Indefinite Delivery Indefinite Quantity (IDIQ)/ Multiple Award Task Orders (MATO) type contracts will be awarded. Negotiated task orders will awarded under a firm-fixed-price basis. The quantities shall not exceed the maximum quantities outlined in Section B. A total contract value cannot be determined at this time.
3. Please refer to Lot H of the pricing sheet for the subject procurement. The Ultrasonic Technologist CLINs 0005AF-3000AF do not specify a location, just "Naval Hospital, FL"
The pricing sheet has been modified. The locations on Lot H, CLINs 0005AF, 1005AF, 2005AF, and 3005AF have changed to Naval Hospital Pensacola, FL.
4. Can you please confirm this requirement to be brand new? If not, can you please provide incumbent information including contract numbers?
See the response to question 1.
5. Will the Government allow a font size of not less than 8 points for tables, charts, graphs, and graphics.
The allowable font size will not be modified. Refer to Section L.2.1.3. of the solicitation.
6. Please advise ASAP if the RFP #N62645-13-R-0018 is a service-disabled veteran set-aside.
Yes. Refer to sections K.1 and L.2.4.3.1. of the solicitation.
7. Would I be correct in assuming the deadline for submissions is 14 Aug?
The closing date and time for the solicitation has been changed to 2:00 PM local time 21 August 2013.
8. Who is the incumbent for this contract and the past award amount?
See the response to question 1. Basic contracts were not awarded with a dollar value. Task orders were placed on the MATO contracts. Information about award amounts may be obtained through a Freedom of Information Act (FOIA) request. http://www.med.navy.mil/sites/nmlc/Pages/RS-ContactUs.aspx
9. The FBO posting has a response date listed of August 20, SF33 of the solicitation states that proposals are due on August 14. Can you please confirm which is the correct date?
See the response to question 7.
10. I noticed that on the FBO announcement for the SE Ancillary MATO that the due date is 20 August 2013. However, in the solicitation, the due date is noted as 14 August 2013. Can you tell me which is the correct due date?
See the response to question 7.
11. Page 122, Attachment AJ, Wage Determinations. The list provided includes Key West and Atlanta/Albany. Can you confirm that no positions for those locations are part of this solicitation?
Services required at these locations shall be considered within the scope of the solicitation/resultant contracts. Refer to section H.1.8.1.
12. Page 133, L.2.2.5., written consent from a teaming partner to disclose PP information. Where should this document be included, and is it exempt from the page limit?
Written consent from a teaming partner is part of Volume I – Past Performance and does not impact the maximum number of Past Performance Information Sheets that may be submitted.
13. Page 142+, SOW. Can you confirm that the Lots represent Task orders?
Each Lot will be awarded to a single contractor. Each Lot may result in one or more task orders placed on a basic contract.
14. Are past performance surveys required to be sent to past performance references to complete and send directly to the Naval Medical Logistics Command contracting officer for this solicitation?
Past Performance Information Sheets are to be submitted in Volume 1: Past Performance of the proposal. There is no requirement for offerors to deliver Past Performance Information Sheets to Point of Contacts.
15. In the solicitation there is one instance where it is listed "Physical Therapy Assistant (1) Mayport, GA". Should that read Mayport, FL instead?
Mayport, GA is not listed in the solicitation nor should it be. Mayport is located in Florida.
16. Who are the incumbent(s)?
See response to question 1.
17. What is the history of task orders on the current contract including location, quantity, and skills ordered?
NMLC will not provide any additional information on current task orders. See Section J, Attachment AF current task order information.
18. On Page 133, Section L.2.2.5 states that that “If a teaming partner is proposed, the offeror shall submit written consent from that teaming partner to disclose their Past Performance information to the contractor. If such consent is not provided as part of the submission, if such consent is not provided as part of the submission, the Past Performance of that teaming partner will be determined not relevant.”
Given that the Volume I page limit is 6 pages, should this documentation be included in Volume I? Will these documents count against the page limit, or should they be included in an Annex that does not count against the page limit?
See response to question 12.
19. Are subcontractors required to complete Section K, Representations and Certifications or is this only to be completed by the prime contractor?
Section K, Representations and Certifications is only to be completed by the prime contractor.
20. The statement of work, para C.3.3.4. States that “In the instance where the Government directs the HCW to remain on duty in excess of their scheduled shift due to an unforeseen emergency or to complete patient treatment where lack of continuity of care would otherwise jeopardize patient health, the HCW shall remain on duty. The HCW will be given an equal amount of compensatory time to be scheduled upon mutual agreement of the HCW and the Commanding Officer. This provision is not intended to apply to the time required to complete routine tasks (e.g., completion of paperwork or routine administrative tasks at the end of a shift) which are to be completed as part of the shift.
As each state has specific requirements concerning overtime, will the Government compensate the Contractor when provisions to provide compensatory time are not adequate to relieve the contractor from state-mandated overtime rules?
In accordance with Section C.5.9. of the solicitation, "The Contractor shall comply with all applicable federal, state, and local laws, Department of Defense, Department of Navy, Bureau of Medicine and Surgery, and MTF instructions and policies." Therefore the contractor is responsible for compliance as stated above. The Government will not pay the contractor for services that exceed the hours required by the contract. An offeror’s unit price should be calculated to adequately cover the costs associated with such compliance.
21. Will the Government provide fingerprinting services as a part of the security processing or must these be obtained from an outside source?
See Section H.8.1.3. The Government will conduct the fingerprinting process.
22. Pricing Sheet for LOT A – CLIN 1002AA (Option Period 1) quantity of hours appears to be incorrect. Please clarify if hours should be 8325 as stated or 8352 (4 individuals * 2088 hours).
The hours for Lot A – CLIN 1002AA (Option Period 1) have been changed to 8352 on the pricing sheet.
23. Pricing Sheet for LOT C – CLIN x006AH is shown as Optometric Technician rather than Ophthalmic Technician (as shown in Enclosure 5-Lot C Statement of Work). Which is correct?
The position title for LOT C, CLINs 0006AH, 1006AH, 2006AH, and 3006AH on the pricing sheet has been changed to Ophthalmic Technician.
24. Pricing Sheet for LOT D—quantity of hours for Base Period appear to be incorrect based on a Period of Performance of 04 May 2014 – 30 September 2014 as it appears the pricing sheet is using 2080, 2088 or 2096 for this period rather than 856. Option Period 1, 2, and 3 quantity hours are also not correct based on the number of individuals noted in each CLIN description, nor do the hours match the Periods of Performance for each Option Period. Please provide an updated pricing sheet workbook with corrected quantity of Hours. In addition, CLINs 2007AI and 3007AI appear to have an error in the description as 2007AI shows it is for Option Period 3 and 3007AI shows it is for Option Period 2 and has an ending POP date of 04 May 2017 rather than 30 September 2017. Please clarify what the correct ending POP date is to be for 3007AI.
All incorrect hours and periods of performance have been modified on the pricing sheet. CLIN 2007AI and 3007AI Option periods have been corrected.
25. Pricing Sheets for Lots E, I, J, K, L, N and O – Period of Performance appears to be inconsistent with the other Lots. Specifically: Base Year POP is shown to be 04 May 2014 – 03 May 2015 instead of 04 May 2014 – 30 September 2014; Option Period 1 POP is shown as 04 May 2015 – 03 May 2016 instead of 01 October 2014 – 30 September 2015; Option Period 2 POP is shown as 04 May 2016 – 03 May 2017 instead of 01 October 2015 – 30 September 2016; and Option Period 3 is shown as 04 May 2017 – 30 September 2017 instead of 01 October 2016 – 30 September 2017. Are these Lots to have different Periods of Performance? If not, please provide an updated pricing sheet workbook with the corrected Periods of Performance and correct quantity of hours for each period.
The periods of performance on the pricing sheet are not all the same and are correct.
26. Pricing Sheet for Lot F – CLIN x007AO – hours appear to be incorrect for 2 Individuals – looks like it is calculating using 1 individual rather than 2 as noted in both the Lot SOW and CLIN description. Please provide corrected pricing sheet for this lot to reflect 2 individuals for this CLIN for Base Period and all Option Periods.
The hours for CLINs 0007AO, 1007AO, 2007AO, and 3007AO on Lot F of the pricing sheet have been corrected.
27. Pricing Sheet for Lot K – CLINs x007BI have a different Period of Performance than the rest of the CLINs in this LOT, but the quantity of hours does not appear to match the Period of Performance shown. Please clarify what the correct Period of Performance and the quantity of hours should be for pricing.
The periods of performance have been corrected for CLINs 0007BI, 1007BI, 2007BI, and 3007BI on Lot K of the pricing sheet.
28. Licensed Practical Nurse (LPN) (included as an option with Psychiatric Technician or LPN or LVN in Lot F in New Orleans, Meridian, Pensacola; Lot G in Gulfport, Pensacola; Lot H in Millington, Pensacola; Lot J in Gulfport)
There is no error with these positions.
29. Licensed Vocational Nurse (LVN) (included as an option with Psychiatric Technician or LPN or LVN in Lot F in New Orleans, Meridian, Pensacola; Lot G in Gulfport, Pensacola; Lot H in Millington, Pensacola; Lot J in Gulfport)
There is no error with these positions.
30. Operating Room Technician in Lot I in Pensacola.
There is no error with this position.
31. Registered Polysomnographic Technologists (Sleep Lab Technologists) in Lot J in Pensacola
There is no error with this position.
32. MRI Technologists in Lots A (Jacksonville) and E (Jacksonville)—the job description is really a Radiologic Technologist (which they included as a WD position)
The job description for the MRI Technologist in Lots A and E is correct.
33. Medical Laboratory Technologist, Lead in Lot B (Mayport) – this job title isn’t in Section C.8, but there is a Medical Laboratory Technician (MLT) in C.8.4.5 and the MLT is a WD position
The position has changed to Medical Technologist.
34. Diagnostic Medical Sonographer (Ultrasound Technologist) in Lot H (Pensacola)—the job description is really a Radiologic Technologist (which they included as a WD position)
The job description for Diagnostic Medical Sonographer (Ultrasound Technologist) in Lot H is correct.
35. Please supply the SCA occupation code to be used for each position in this solicitation.
All SCA Occupation codes can be found on the wage determination documents. Refer to Section J Attachment AJ.
36. Section L.2.3.2, Page 134 – Given the variety of labor categories requested and service locations, would the Government consider increasing the page limit of Volume II by 5 pages to allow offerors to provide a more thorough discussion of the requirements?
The page limit will not be revised. Refer to L.2.1.3. for page limits.
37. Section L.2.4.5.1 states “pricing and supplemental pricing for this solicitation represents minimum quantities plus four option periods (lots).” However, the pricing spreadsheet shows only Base Period and three option years.
Can the Government confirm pricing is only required for Base Period plus three options years?
Section L.2.4.5.1. has been modified. Pricing and supplemental pricing for this solicitation represents minimum quantities plus four option periods (lots).
38. I have noticed the posting on the FBO site has a Response due date. The Response due date SF 33 is 14 Aug 2013. Please clarify for all concerned.
See the response to question 7.
39. Please advise when the above solicitation is due. On FBO the proposal is due on August 20th at 2pm however on the SF33 the bid is due on August 14th at 2pm.
See the response to question 7.
40. Please advise what the continuity of Service Clause mean?
The clause is below. All clauses can be found at http://farsite.hill.af.mil/ 52.237-3 -- Continuity of Services. (Jan 1991)
(a) The Contractor recognizes that the services under this contract are vital to the Government and must be continued without interruption and that, upon contract expiration, a successor, either the Government or another contractor, may continue them. The Contractor agrees to --
(1) Furnish phase-in training; and
(2) Exercise its best efforts and cooperation to effect an orderly and efficient transition to a successor.
(b) The Contractor shall, upon the Contracting Officer’s written notice,
(1) furnish phase-in, phase-out services for up to 90 days after this contract expires and
(2) negotiate in good faith a plan with a successor to determine the nature and extent of phase-in, phase-out services required.
The plan shall specify a training program and a date for transferring responsibilities for each division of work described in the plan, and shall be subject to the Contracting Officer’s approval. The Contractor shall provide sufficient experienced personnel during the phase-in, phase-out period to ensure that the services called for by this contract are maintained at the required level of proficiency.
(c) The Contractor shall allow as many personnel as practicable to remain on the job to help the successor maintain the continuity and consistency of the services required by this contract. The Contractor also shall disclose necessary personnel records and allow the successor to conduct on-site interviews with these employees. If selected employees are agreeable to the change, the Contractor shall release them at a mutually agreeable date and negotiate transfer of their earned fringe benefits to the successor.
(d) The Contractor shall be reimbursed for all reasonable phase-in, phase-out costs (i.e., costs incurred within the agreed period after contract expiration that result from phase-in, phase-out operations) and a fee (profit) not to exceed a pro rata portion of the fee (profit) under this contract.
(End of Clause)
41. FBO states the bid is due 20 August 2013, at 1400 EST; however, the actual solicitation states 14 August 2013, at 1400 EST
See response to question 7.
42. We noticed on Attachment AF, there is a current bill rate given for an Optician Technician at Pensacola and on page 139 it is listed that in Enclosure 17 a Statement of Work will be provided for the optician technician at Pensacola. However, there is no Optician technician being requested on the price sheet, nor is the an actual statement listed in enclosure 17. Can the government please clarify if we are to provide pricing for an Optician Technician at Pensacola, and if so please provide a revised price sheet to include the position and a PWS for the position?
The Optician Technician position has been removed from Section L list of enclosures, Enclosure 17 – Lot O Statement of Work and Section J, Attachment AF.
*NOTE: The following question will be answered via an additional amendment which will be posted to FedBizOps.
1. Pages 139 and 283-287. Page 139 lists a Clinical Social Worker for San Antonio and an Optician Technician for Pensacola (NOMI), but the detailed work statements on pp. 283-287 do not mention either of these positions. Should they be included as part of this solicitation?

SECTION L - INSTRUCTIONS, CONDITIONS AND NOTICES TO BIDDERS
The following have been modified:
SECTION L
Failure to comply with any of the instructions may result in the proposal being removed from further consideration.
L.1. SUBMISSION OF COST OR PRICING DATA.
L.1.1. It is expected that this contract will be awarded based upon a determination that there is adequate price competition; therefore, the offeror is not required to submit additional cost or price data (beyond that required by Section L.2) or to certify cost or pricing data with its proposal.
L.1.2. If, after receipt of the proposals, the Contracting Officer determines that adequate price competition does not exist in accordance with FAR 15.403-3 and FAR 15.403-4, the offeror shall provide other information requested to be submitted to determine fairness and reasonableness of price, or certified cost or pricing data as requested by the Contracting Officer.
L.2. PROPOSAL CONTENT AND INSTRUCTIONS FOR PREPARATION OF PROPOSALS.
L.2.1. Introduction and Purpose - This section specifies the format and content that offerors shall use in this Request for Proposal (RFP). The intent is not to restrict the offerors in the manner in which they will perform their work but rather to ensure a certain degree of uniformity in the format of the responses for evaluation purposes. Offerors must submit a proposal that is legible and comprehensive enough to provide the basis for a sound evaluation by the Government. Information provided should be precise, factual, and complete. Legibility, clarity, completeness, and responsiveness are important. Proposals shall be in the form prescribed by, and shall contain a response to, each of the areas identified in Section L.2. Any proposal that does not meet the requirements of the solicitation may be determined to be substantially incomplete and may result in rejection of the offeror's entire proposal.
L.2.1.2. The proposal shall be submitted in three separate volumes:
Volume I – Past Performance
Volume II – Technical
Volume III – Business
A complete Volume I - Past Performance; Volume II -Technical; and Volume III - Business; shall be submitted at the same time to the address in Paragraph L.3.3. by the closing time and date referenced in Block 9 of the SF 33. If any one proposal volume is received past the stated closing date specified in this solicitation, the entire proposal will be considered late. No further consideration will be given to any offeror who submits any of these volumes late in accordance with FAR 15.208(b).
L.2.1.3. The Past Performance and Technical volumes (Volumes I and II, respectively) shall be in Microsoft Word for Windows (2007)(.docx) with 1 inch margins all around, Times New Roman font of not less than 10 point font size inclusive of tables, charts, graphs, graphics, etc. Text, tables, charts, graphs, graphics, etc. that are not in compliance will not be considered by the Government in its evaluation. Each page of each document shall have a footer indicating the name of the offeror and “page X of Y.”
L.2.1.4. The total number of the pages submitted for Volume I (Past Performance) shall not exceed 6 pages (Past Performance Information Sheets), excluding a title page and consent letters. If Volume I contains more than 6 pages, only the first 6 pages will be evaluated. The total number of the pages submitted for Volume II (Technical) shall not exceed 10 pages, excluding a title page and table of contents. If Volume II contains more than 10 pages, only the first 10 pages will be evaluated. Cover letters are not required for either volume; any information contained in a cover letter will not be evaluated.
L.2.2. Volume I: Past Performance. In accordance with electronic submission requirements given in this section, the offeror shall include the following:
L.2.2.1. The offeror shall complete a maximum of six Past Performance Information Sheets (Section L Enclosure 1). Each of the Past Performance Information Sheets shall be completed in its entirety. The Past Performance Information listed on each sheet shall be for clinical medical or dental services. If more than 6 Past Performance Information Sheets are provided, only the first six will be considered. Each Past Performance Information Sheet shall identify no more than one Point of Contact (POC). Each Past Performance Information Sheet may include multiple contracts or one or more task orders under single IDIQ contracts. The same contract may appear on more than one Past Performance Information Sheet if more than one POC exists. If more than one POC is provided for any Past Performance Information Sheet, only the first POC will be considered. Each Past Performance Information Sheet shall not exceed one page. If a Past Performance Information Sheet contains more than one page, only the first page will be considered. Contracts cited must be current, or have ended not more than three years prior to the closing date of the solicitation. If the contract performance ended more than three years prior to the closing date of the solicitation it will be determined not relevant.
L.2.2.2. The offeror may submit past performance that demonstrates prior experience of key personnel. The past performance submission shall identify, by name, the key personnel and clearly identify their previous roles and responsibilities and their proposed roles and responsibilities under the resultant contract. If the submission does not clearly describe the roles and responsibilities of the key personnel and the how the key personnel will contribute to the performance under the resultant contract, it will be determined not relevant.
L.2.2.3. The offeror may submit past performance that demonstrates prior experience by teaming partners (e.g. proposed subcontractor, partner, mentor, protégé, parent company, sister company, etc.) only if they will be performing in support of the contract resulting from this solicitation. Such past performance submissions shall clearly identify the relationship of the teaming partners to the offeror and how they will contribute to the performance under the resultant contract. If the submission does not clearly describe the relationship of teaming partners to the offeror and how the teaming partners will contribute to the performance under the resultant contract, it will be determined not relevant. A maximum of three Past Performance Information Sheets shall be for services provided by entities other than the offeror, members of SBA approved Joint Ventures, and key personnel.
L.2.2.4. If Past Performance is submitted for a contract under which the offeror or teaming partner provided services as a subcontractor, partner, mentor, protégé, parent company, sister company, etc., the Past Performance Information Sheet shall clearly describe the corporate relationships among those who provided the services, citing key responsibilities for each, to include identifying the specific services provided (in terms of numbers and types of personnel, locations, etc) and nature of services provided (e.g., recruitment, payroll, benefits administration, etc). If the submission does not clearly detail the specific responsibilities and relationship under a contract, it will be determined not relevant.
L.2.2.5. If a teaming partner is proposed, the offeror shall submit written consent from that teaming partner to disclose their Past Performance information to the contractor. If such consent is not provided as part of the submission, the Past Performance of that teaming partner will be determined not relevant.
L.2.2.6. If neither the offeror nor its teaming partner possesses relevant past performance, the offeror shall submit a statement affirming that it possesses no relevant Past Performance.
L.2.2.7. The Government may contact the POCs listed on the Past Performance Information Sheets. POCs must be either Government personnel (civil service or military) or employees of private sector clients (such as public or private sector dental/medical facilities) with whom it has provided services. Further, the offeror shall discuss their knowledge of the marketplaces represented in the solicitation and discuss how marketplace conditions (such as, but not limited to scarce labor categories or remote locations) will impact the ability to recruit and retain required health care workers. Without revealing the prices submitted in response to this solicitation, the offeror shall demonstrate market research and discuss how it has applied the marketplace knowledge in development of the proposal. The plan will also address how the offeror will comply with FAR 52.222-46. Specifically, it shall provide evidence that compensation rates will be sufficient enough to provide uninterrupted, high-quality services.
