SUMMARY OF CHANGES

SECTION SF 30 - BLOCK 14 CONTINUATION PAGE

The following have been added by full text:

ANSWER ADDITIONAL QUESTIONS

1. Can you please specify if we are to base our proposal pricing off the wage determinations incorporated into the solicitation or based on the most up to date wage determinations?

Answer: The Wage Determinations in Section J, Attachment AJ have been updated. The offeror is required to comply with the current Wage Determination.

2. In the process of filling out the pricing sheet I noticed that there was an inconsistency in LOT F. I noticed that 0002AC has 4176 hours, but the FTE is listed as 1. In the other lots, every instance of 4176 hours was associated with 2 FTEs. 0002AC Licensed Clinical Social Worker (1 Individual) at Naval Hospital Lemoore, CA. Base Period-01 October 2014 through 30 September 2015.

Answer: The pricing sheet has been updated and posted to the Federal Business Opportunities web site (https://www.fbo.gov/) with Amendment 0001 on 28 June.

3. Can the Government provide the SCA classification for each of the positions in Lots A-G?

Answer: The SCA classification for Nurse Practitioner is RN Level III, the classification for Certified Nursing Assistant is Nursing Assistant Level II.

4. QUESTION: Are offerors required to discuss the marketplace only for the 8 primary listed Naval Hospital locations and associated branch clinics or must the discussion also include all the states where performance might be performed, as identified in Section H.1.8.1?

If discussion is required for all the states, can the Government please provide a narrower scope than the entire state?

Answer: Offerors are advised to discuss their knowledge of the marketplaces for all labor bands and locations represented in paragraph H.1.8.1 of the solicitation. This includes all states and territories listed. The government does not intend to reduce the scope as services may be required in any or all of the locations listed during the period of performance of the potential contract.

5. I noticed that a new price sheet with an undated summary sheet has not yet been posted. When can we expect a new price sheet and the responses to the questions and answers?

Answer: See answer to question 2.

6. According to Section L.2.4.5, the Supplemental Pricing should be included in an Excel Spreadsheet provided by the Govt. Neither of the current price sheets (attached) provide an area to include the Supplemental Pricing as outlined below. (Min and Average in Excel Spreadsheet).

L.2.4.5. Pricing (offeror's hourly rates) and supplemental pricing (health care workers' compensation rates) shall be submitted on a spreadsheet. An electronic file will be posted to the Federal Business Opportunities web site (https://www.fbo.gov/) along with the solicitation and any amendments for downloading. The file will be named "Pricing Sheet for N62645-13-R-0004.xlsx". The offeror shall complete the pricing worksheet and supplemental pricing worksheets required on all tabs included in the electronic spreadsheet. Blue boxes designate those fields into which the offeror can enter data. The completed file shall be submitted on the Volume III - Business CD-ROM. The file shall be renamed: [name of offeror] pricing sheet.xlsx. Please note that any reformatting of the pricing spreadsheet or failure to follow the naming conventions provided in this section may cause a serious delay in the evaluation process and may result in rejection of the offeror's proposal.

Please advise how we should address?

Answer: There is a place to add minimum and average rates on each Lot Tab, for each CLIN, on the pricing sheet on Columns F and G.
