PERFORMANCE WORK STATEMENT

Scope:

1. Scheduled Maintenance

1.1 Vendor shall provide parts and skilled labor to perform the periodic maintenance listed in this document and in the OEM service manuals for the equipment for two low pressure breathing air systems. In addition to the listed maintenance, the vendor shall perform other needed inspections, adjustments, and minor repairs needed to meet the manufacturer’s maintenance requirements and keep the equipment operating safely and correctly.

1.2 Periodic maintenance and other inspections and maintenance shall be accomplished during four “maintenance visits” per year to the equipment location. These maintenance visits shall be scheduled by the customer’s Equipment Point of Contact, with a minimum of 10 days prior notice to the vendor, approximately every 3 months. A maintenance visit may last for more than one day and may require more than one technician. The duration of maintenance visits and the number of personnel required shall be determined by the vendor.

1.3 During the performance of periodic maintenance, the vendor shall inspect each complete system and perform troubleshooting, as needed, to identify any repairs or maintenance actions needed outside the scope of this contract. Provide a service report of the findings to the Equipment Point of Contact.

1.4 Contractor Furnished Property: The vendor shall supply shop supplies and maintenance parts necessary for the performance of scheduled maintenance listed in this document and in the OEM service manuals for the equipment.

2. Telephone Support

Vendor shall provide toll-free telephone support during the vendor’s normal working hours to help the Equipment Points of Contact to resolve questions regarding equipment operation and maintenance.

3. Equipment Location:

SWFPAC Bldg 7114 Wahoo Rd Service Pier, Submarine Base Bangor Silverdale, WA 98315

Note: The service location can only be accessed by United States citizens with a valid US passport or birth certificate.

4. Quality Assurance

4.1 All parts used shall be new and supplied by the original equipment manufacturer (“OEM”). Repairs and maintenance shall be in accordance with the OEM’s written instructions and shall be performed by maintenance personnel trained by Kobelco or the component OEM.

4.2 After the completion of maintenance and repairs, the vendor shall operate the equipment to ensure repaired components are operational and working correctly. The equipment shall be allowed to come up to normal operating temperature during the test, then a full set of log readings shall be taken by the vendor, recorded on a Kobelco Daily Operating Record, and mailed or delivered to the address provided in the Kobelco Instruction Manual. Provide Documentation of completion on CIDRL-DI-MISC-81356.

4.3 Prior to departing the equipment location after performing inspection, maintenance, or repair work, the vendor shall clear the worksite of all debris and vendor material, and the equipment restored to a safe and secure condition. The Equipment Point of Contact and Waste Disposal Point of Contact shall each be notified by the vendor prior to the vendor leaving the worksite at the end of the workday. Alignment of the equipment to supply air to the connected LPA system shall only be accomplished by the Equipment Point of Contact.

5. General Requirements:

5.1 Maintenance, troubleshooting, and repair shall be performed at the listed equipment location, at times to be coordinated with the listed Equipment Point of Contact.

5.2 Maintenance listed as yearly maintenance shall be accomplished during the second of the four maintenance visits.

5.3 Three year maintenance shall be performed in accordance with the following schedule, during one of the four yearly maintenance visits:

5.3.1(a) Kobelco serial #06H0302 with Zeks purifier – Three year maintenance due in 2022 and 2025.

5.3.1(b) Kobelco serial #08H6111166 with Pneumatic Products purifier – Three year maintenance is due in 2023 and 2026.

5.5 Equipment lock out and tag out shall be accomplished by the vendor prior to any work being performed, except work which requires the system to be energized or operating. If the equipment is in operation, only the Equipment Point of Contact can secure the equipment. If the equipment is energized but not in operation, only the Equipment Point of Contact can authorize the vendor to secure the equipment. An exception is if a severe defect is detected which threatens personnel or the equipment with immediate injury or damage unless the equipment is secured.

5.6 Cranes, forklifts, and other heavy lifting equipment needed to support vendor work shall be supplied by the government. Requests for this support shall be made to the points of contact at least two working days in advance.

5.7 Hazardous waste generated during maintenance and repair shall not be removed from the government facility. One week prior to generating hazardous waste, such as condensate or used oil, the Waste Disposal Point of Contact shall be notified so that empty waste containers can be delivered to the worksite. Great care shall be taken by the vendor to ensure no liquid waste contaminates the surfaces and waterways at the government facility.

5.8 Personnel conducting maintenance shall have received training from both Pneumatic

Products and Zeks on the operation and maintenance of air purification equipment.

Personnel shall also have received operation and maintenance training from either Kobelco or a company authorized by Kobelco to perform training on the operation and maintenance of Kobelco compressors. Documentation from Zeks, Pneumatic Products, and Kobelco shall document the date of this training. Either the described training or refresher training from Zeks, Pneumatic Products or Kobelco shall have occurred within 48 months of the date of any maintenance is performed. In place of training by Kobelco, training can be conducted by a company approved in writing by Kobelco to conduct the training. Provide Documentation of training on CIDRL-DI-MISC-81356

5.9 The bid shall include documentation of training and qualification of personnel who will conduct the specified maintenance. If training in the maintenance and operation of Kobelco compressors was not conducted by Kobelco, the bid shall also include documentation from Kobelco stating the source of training is approved.
