STATEMENT OF WORK (SOW)

Ship/Activity: U.S Navy Submarine

Title: Clean Ventilation System

Place of Performance: PSNS & IMF, Bremerton WA.

Period of Performance: 02/04/19 – -4/05/19

1. BACKGROUND.

1.1 U.S. Navy Submarine requires cleaning of the ships ventilation system. Puget Sound Naval Shipyard and Intermediate Maintenance Facility (PSNS&IMF) management of planned overhaul and maintenance requirements requires that work be performed by specialized skilled personnel having knowledge, expertise and specialized equipment to effectively accomplish work on ships systems.

3. WORK REQUIREMENTS/SCOPE.

3.1 Ensure all contractor and subcontractor employees, prior to start of work, are briefed in the requirements of 2.1 and 2.2 and have direct knowledge of work control procedures, be able to recognize and initiate alarms, be familiar with actions to be taken to evacuate the vessel, and reporting submerged safety draft mark conditions.

3.1.1 Accomplish a joint, on-site brief and walkthrough of the work site with the Contracting

Officer’s Representative (COR) and the Commanding Officer’s designated representative prior to start of work to identify areas of nuclear interface, equipment or systems and to identify SUBSAFE, Unrestricted Operations (URO) maintenance requirement (MRC) related components and/or systems located in the area of work, components and/or systems which may be affected by the work, and lessons learned from previous or similar work.

3.1.2 Ensure no URO MRC measured parameter is violated.

3.1.3 Maintain approved written instructions at the work site for accomplishment of non-

SUBSAFE work and do not accomplish work or disturb any system or component without specific approved written instructions.

3.1.4 Comply with the work authorization and tag-out requirements of Volume IV, Chapter 10 of

2.14 and 2.15 to ensure all work is authorized and controlled with the Government NSA acting as the Repair Activity designated representative.

3.1.4.1 Coordinate with the Government Project Work Control (Code 246) employees to verify work boundaries ensuring the entire site is isolated and the Engineering

Help-Desk for submission of Work Authorization Forms (WAF).

3.1.4.2 Review WAF with Ship's Force and Project Work Control employees to ensure understanding of authorized work areas prior to starting work and maintain a copy of the WAF posted at the work site during production work.

3.1.4.3 Submit changes to isolation or work boundaries to the Code 246 employees and upon completion of production work, sign Block 16 of the WAF and obtain

Ship’s Force concurrence for the clearing of isolation tags.

3.1.5 Evaluate Contractor services/temporary systems to be installed running through spaces containing nuclear equipment for possible leakage/spray protection and identify all possible contact with nuclear equipment or nuclear temporary/support system identified in the space prior to start of work.

3.1.5.1 Evaluate all rigging pathways for potential collateral damage to nuclear components/piping and inadvertent contact with nuclear equipment or nuclear temporary/support systems in the work area.

3.1.5.2 Report immediately inadvertent contact with or damage to nuclear equipment regardless of how minor (e.g., gouges, scratches, dents, slag, carbon arc, corrosion) to the Contracting Officer’s Representative (COR).

3.1.5.3 Prevent material (permanent or temporary) from contact with nuclear piping/components unless specifically authorized by the Naval Supervisory

Activity (NSA) including cleaning fluid sprays, dripping grease or liquids, inadvertent paint splatter, attaching rope or strings, wood, tape, plastic bags, and temporary contractor’s services contacting hot or cold nuclear piping and components

3.1.6 Comply with the fire prevention requirements of 2.11.

3.1.7 Comply with the requirements of 2.3 through 2.8 and 2.15 for environmental and safety processes and procedures.

3.1.7.1 Ensure employee and management cooperation with each environmental, health, safety, and accident/mishap investigation.

3.2 Comply with the requirements of 2.10, to prohibit accomplishment of work in areas requiring personal monitoring of radiation, i.e. wearing a Thermoluminescent-Dosimeter (TLD).

3.2.1 Planning, supervising, or performing nuclear work is not authorized without the express approval of Code 300N, Code 1200N, Code 2300, and Code 105.

3.3 Develop a detailed production schedule in Gantt-Chart format reflecting the manner in which the work will be accomplished at the activity level, listing the planned start and finish dates and durations for each ventilation system.

3.3.1 Submit one legible copy, in approved transferrable media, of the contractor’s production schedule to the Contracting Officer’s Representative (COR) one week prior to start of work and daily once work has commenced. (CDRL A001)

3.3.1.1 Submit the daily progress report to the COR starting one day prior to arrival and thereafter until completion of work to include the report number, progress percentage of each ventilation system cleaning and any issues requiring

Government assistance to resolve.

3.4 Prohibit the following items from being brought onboard any nuclear-powered vessel or nuclear support vessel no matter how small the quantity:

3.4.1 Any mercury bearing equipment such as mercury thermometers, portable fluorescent lights, black lights, or any other items containing mercury.

3.4.2 Nickel-Cadmium fasteners.

3.4.3 Any device containing a source of radioactivity.

3.4.4 Bright yellow tools, bags, or equipment.

3.5 Maintain or establish lost cleanliness of the submarine ventilation system to “Clean” condition in accordance with Appendix B of 2.12.

3.6 Accomplish cleaning of identified vent systems 2.13, approximately 1,200 linear feet of ducting, including fans and associated equipment with the below listed exceptions:

3.6.1 Fan systems 7 through 10 and portion of fan system 27 upstream of and including

(Ventilation Valve VH-8) servicing the Reactor Compartment (RC). Access to the RC is

NOT permitted.

3.6.2 Submit one legible copy, in approved transferrable media, of a condition found report of remaining systems requiring cleaning if ducting exceeds the approximate 1,200 lineal feet.

3.6.3 Plan for approximately 20% of ventilation system ducting to be already removed off ship to support other maintenance activity work.

3.6.3.1 Accomplish cleaning of the previously removed sections of the ventilation system at a location provided dockside.

3.6.3.2 Plan for heavy contamination conditions in the ship’s galley (exhaust system 28) and the ship’s laundry (exhaust system 27).

3.6.4 Prevent damage of the ventilation system ducting and equipment from the Contractor’s cleaning processes.

3.6.4.1 Submit one legible copy, in approved transferrable media, of a condition found report identifying any deficiencies found beyond the scope of this contract action to the COR within 2 business days of the discovery.

3.6.4.2 Include a description of the deficient condition including annotated sketches or drawings, recommendation for repair, to include a listing of material(s) and proposed labor hours.

3.7 Segregate/separate all waste including hazardous waste and complete waste disposal forms per the requirements of 2.6, 2.8 and 2.9.

3.8 Comply with the requirements of 2.16 to prevent possible contamination or damage of work environment, equipment or components while performing tasks by taking necessary protective actions during access, disassembly, or removal of ventilation components and equipment.

3.9 Accomplish a close out inspection of ducting and ventilation components, prior to reassembly, with the

Contracting Officer’s Representative (COR) and Commanding Officer’s designated representative for inspection of the work accomplished.

3.9.1 Proceed with reassembly upon final acceptance by the COR/ Commanding Officer’s designated representative.

3.9.2 Conduct a joint tightness inspection, upon restarting ventilation system, and resolve any joint leakage.

3.9.2.1 Submit one legible copy, in approved transferrable media, of the Contractor’s joint inspection report to the COR upon completion of each system test. (CDRL

3.9.2.2 Coordinate with the Ship’s Force to independently test each ventilation system fan motor for proper operation.

3.9.2.3 Prohibit Contractor employees from energizing and operating ventilation equipment. Operation of shipboard equipment is the sole responsibility of

Ship’s Force.

3.9.2.4 Observe all safety precautions during the operational test to include ensuring employees are protected from possible harm, during initial start-up, from loose objects, debris or waste dislodging and becoming a hazard near supply vents.

3.9.2.5 Contractor will not be responsible for post installation testing of ventilation system sections disassembled by other maintenance activities.

3.9.2.6 Submit one legible copy, in approved transferrable media, of a service report identifying conditions found and actions completed on supply systems, recirculation systems, and exhaust systems. (CDRL A005)

3.10 Provide Contractor Hazardous Material Inventory (CHMI) and Safety Data Sheets, for contractor furnished hazardous material, five (5) days after contract award to the COR. (CDRL A006)

4. NOTES.

4.2 Government Supported Work Hours: Normal ship working hours are 0700 to 1600, Monday through

Friday. If Government support is required outside normal work hours, special arrangements in advance will need to be made via the Project Team Representative.

4.3 Government observed holidays: 12 and 22 November, 2018 are observed government holidays.

5. GOVERNMENT FURNISHED MATERIAL/SERVICES.

5.1 Provide disposal of waste, including hazardous waste, as designated on the Waste Information Sheet

(WIS) provided to the Contractor.

5.2 Provide assist rigging services shipboard and adjacent to the pier/dock.

5.3 Provide temporary electricity (less than 200 amps) and utility services.

5.4 Provide Contractor equipment laydown area, 30’ x 30’ (maximum square footage).

5.5 Provide an enclosed area 10’ x 10’ with negative temporary ventilation for cleaning of off-hull ducting.

5.6 Provide requested technical information and/ or drawings of systems or equipment.

5.7 Government Project Code 246 will ensure work authorization requirements are met in support of contracted vent cleaning, including preparation and approval of Work Authorization Forms (WAFs), isolation of work site (Tag-outs), and coordination with Ship’s Force.
