Continuation of Description

STATEMENT OF WORK (Option Years) for

"Root Cause and Human Factor Analysis Classification Systems Training and Support"

1.0 ORGANIZATION: NNSY Quality Engineering and Analysis Division Code 136.

1.1 Background: Root Cause (RC) and Human Factor Analysis and Classification System (HFACS) framework have been used within the military to systematically examine underlying human causal factors and to improve investigations. The development of HFACS was prompted by increasing problems with human performance. The current state of NNSY requires the application of RC and HFAC methodologies in support of process improvement. When NNSY encounters mishaps, near misses, safety non-compliance, injuries etc., it is essential for these incidents to be captured and documented to communicate lessons learned and derive mitigating actions going forward. This is accomplished through Team Learning Sessions, with the expectation that the session be performed immediately following an incident and/or accident. NNSY is currently experiencing a challenge in solving problems and investigating incidents involving human error. The timeliness of the completion of TLS Reports has also presented a challenge. To address the concerns, NNSY requires Apollo Root Cause Analysis (ARCA) training supplemented by HFACS training.

1.2 Objective/Scope: The objective of this task is to purchase a 3-year (Base +2 option years) of Root Cause Analysis (RCA) training utilized in the Human Factor Analysis and Classification System (HFACS) training. The overall training will enhance organizational capability to investigate, analyze, and expose root causes including human influences in operational investigations of incidents and accidents to improve operational performance and safety. The training will help safety professionals reduce the accident rate through systematic, data-driven investigative strategies and objective evaluation of mitigation methods. The desired end state is to have participants be equipped with efficient and effective knowledge for investigative insights that will encompass and expand the established NNSY Team Learning Session (TLS) Training. NNSY requires the training to produce a class of students with an ability to clearly understand known causal relationships to determine which causes that if removed or controlled will support an effective outcome and prevent reoccurrence of errors. This training should equip NNSY personnel with skills and abilities that will improve the timely completion of TLS with accurate reporting.

1.3 Equipment Description/Specifications: The contractor shall hold an accreditation for Root Cause Analysis Facilitator(s) and Human Factor Analysis. The contractor shall provide an RCA and HFAC certified instructor to administer and facilitate the training. New users will be trained in RCPro web-based software by the contractor and will be given 1 year of access to the program upon completion of the course.

1.4 Services Required:

NNSY requires the contractor to develop and administer role specific learning curriculum covering the subject and application of Root Cause and Human Factors Analysis combined and tailored to NNSY. The training shall be instructor led by instructor(s) with certification in both Root Cause Analysis and Human Factor Analysis.

The contractor must provide all necessary and associated instructors, training material and all modules on a web-based platform. The curriculum shall encompass causal analysis system to target root causes and eliminate conjecture upon establishment of key facts in the investigative process. The contractor is required to have the training facilitated by an instructor with experience developing programs for complex investigations that involve human error scenarios. The goal is that each participant will add to their personal development by applying new additional tools of methodology to become a more effective and efficient investigator for their respective roles.

- NNSY would require from the Vendor/Contractor to tailor training toward behavior conflict and resolution to have key objectives to prevent a mindset of making unwarranted decisions that caused the unplanned event.

- NNSY requires training for an online principle-based software that incorporates a mapped path to a final resolution that forms root causes and aligns the associated Human Factors.

- NNSY requires the contractor to develop and administer role specific learning curriculum (Department Head Group/Senior Manager, Event Owner, Facilitator) in a systematic approach that reinforces the importance of Human Factors and causal alignment starting with the NNSY Leadership that carries to the TLS Team members (See Figure 1).

- Support TLS Administrators with investigation mentoring, investigation tool support, RCA/HFA support to support trend analysis.

REQUIREMENTS FOR ACCESS TO NAVY INSTALLATIONS

Commander, Navy Installations Command (CNIC), has established the Defense Biometric Identification System, (DBIDS), a standardized process for granting unescorted access privileges to vendors, contractors, suppliers and service providers not otherwise entitled to the issuance of a Common Access Card (CAC) who seek access to and can provide justification to enter Navy installations and facilities. Participation in the DBIDS is not mandatory, and if the Contractor chooses to not participate, the Contractor's personnel will have to obtain daily passes, be subject to daily mandatory vehicle inspection, and will have limited access to the installation. The Government will not be responsible for any cost or lost time associated with obtaining daily passes or added vehicle inspections incurred by non-participants in the DBIDS. Alternatively, if the vendor so chooses, it may voluntarily elect to obtain long-term credentials through enrollment, registration, background vetting, screening, issuance of credentials, and electronic validation of credentials.

Further information regarding DBIDS can be found at www.cnic.navy.mil/om/dbids.html

POSTAWARD SMALL BUSINESS PROGRAM REREPRESENTATION (FEB 2026) (DEVIATION 2026-O0038)

(a) . As used in this clause-Definitions

Long-term contract means a contract of more than five years in duration, including options. However, the term does not include contracts that exceed five years in duration because the period of performance has been extended for a cumulative period not to exceed six months under the clause at 52.217-8, Option to Extend Services, or other appropriate authority.

Small business concern-

(1) Means a concern, including its affiliates, that is independently owned and operated, not dominant in its field of operation, and qualified as a small business under the criteria in 13 CFR part 121 and the size standard in paragraph (c) of this clause.

(2) , as used in this definition, means business concerns, one of whom directly or indirectly controls or has the power to control the others, Affiliates or a third party or parties control or have the power to control the others. In determining whether affiliation exists, consideration is given to all appropriate factors including common ownership, common management, and contractual relationships. SBA determines affiliation based on the factors set forth at 13 CFR 121.103.

(b) If the Contractor represented that it was a small business concern, a small disadvantaged business concern, or a joint venture that was any of the small business concerns identified in 19.000(a)(3) prior to award of this contract, the Contractor shall rerepresent its size and socioeconomic status according to paragraph (e) of this clause or, if applicable, paragraph (g) of this clause, upon occurrence of any of the following:

(1) Within 30 days after execution of a novation agreement or within 30 days after modification of the contract to include this clause, if the novation agreement was executed prior to inclusion of this clause in the contract.

(2) Within 30 days after a merger or acquisition that does not require a novation or within 30 days after modification of the contract to include this clause, if the merger or acquisition occurred prior to inclusion of this clause in the contract.

(3) For long-term contracts-

(i) Within 60 to 120 days prior to the end of the fifth year of the contract; and

(ii) Within 60 to 120 days prior to the date specified in the contract for exercising any option thereafter.

(c) The Contractor shall rerepresent its size status in accordance with the size standard in effect at the time of this rerepresentation that corresponds to the North American Industry Classification System (NAICS) code(s) assigned to this contract. The small business size standard corresponding to this NAICS code(s) can be found at https://www.sba.gov/document/support--table-size-standards.

(d) The small business size standard for a Contractor providing an end item that it does not manufacture, process, or produce itself, for a contract other than a construction or service contract, is 500 employees, or 150 employees for information technology value-added resellers under NAICS code 541519, if the acquisition-

(1) Was set aside for small business and has a value above the simplified acquisition threshold;

(2) Used the HUBZone price evaluation preference regardless of dollar value, unless the Contractor waived the price evaluation preference; or

(3) Was an 8(a), HUBZone, service-disabled veteran-owned, economically disadvantaged women-owned, or women-owned small business set-aside or sole-source award regardless of dollar value.

(e) Except as provided in paragraph (g) of this clause, the Contractor shall make the representation(s) required by paragraph (b) of this clause by validating or updating all its representations in the Representations and Certifications section of the System for Award Management (SAM) and its other data in SAM, as necessary, to ensure that they reflect the Contractor's current status. The Contractor shall notify the contracting officer in writing within the timeframes specified in paragraph (b) of this clause, that the data have been validated or updated, and provide the date of the validation or update.

(f) If the Contractor represented that it was other than a small business concern prior to award of this contract, the Contractor may, but is not required to, take the actions required by paragraphs (e) or (g) of this clause.
