3. REQUIREMENTS:
3.1 Accomplish the following, in addition to each specific requirement of the Job Order, for each Work Item which identifies a reservation of manhours or material dollars.
3.2 Identify each task/discrepancy and initiate an estimate of each manhour and material dollar on Reservation Task Request/Control Form (Attachment A) for each discrepancy or task identified to be accomplished under a reservation item or reservation paragraph, entering the date and estimate of labor and material, and deliver the estimate to the
SUPERVISOR.

The contractor will then sign the "Contractor Obligation/Agreement to Quote" blank and enter the estimated start date. The RMC/NSA contracts representative will initial the form in the blank provided.
3.3.1 If there is a difference of opinion in the estimate provided by the contractor and RMC/NSA, each manhour and material dollar will be negotiated immediately to resolve the difference. Each negotiated manhour and material dollar will then be entered in the provided "Revised Estimate" blocks with each required signature as indicated in 3.3.
3.4 Ensure the control form has the work completion date, signed and dated by the contractor, and submit to the SUPERVISOR within two (2) days after completion of the tasking authorized.

completion of the manifest. The manifest prepared must be presented to the SUPERVISOR for completion after the hazardous waste has been identified.
3.2.2 Documentation related to hazardous waste generated solely by the physical actions of contractor personnel (termed Contractor- Generated Hazardous Waste) must bear a generator identification number issued to the contractor pursuant to applicable law.
Regardless of the presence of other material in or on the shipboard systems or structure which may have qualified a waste stream as hazardous, where the contractor performs work on a system or structure using materials (whether or not the use of such materials was specified by the Navy) which by themselves would cause the waste from such work to be a hazardous waste, documentation related to such waste must only bear a generator number issued to the contractor.
3.2.3 Documentation related to hazardous waste generated by the combined physical actions of Navy and contractor personnel (termed Co-Generated Hazardous Waste) must bear a generator identification number issued to the contractor pursuant to applicable law and must also cite in the remarks block a generator identification number issued to the Navy pursuant to applicable law. When the contractor merely drains a system and such drainage creates hazardous waste or the contractor performs work on system or structure using materials which by themselves would not cause the waste from such work to be hazardous waste but such work nonetheless creates a hazardous waste, documentation related to such waste must bear a generator identification number issued to the contractor and must also cite in the remarks block a generator identification number issued to the Navy. The contractor must sign the generator certification on the Uniform Hazardous Waste Manifest whenever use of the manifest is required for disposal. The contractor must obtain SUPERVISOR's concurrence with the categorization of the wastes as co-generated before completion of the manifest. Manifests prepared must be presented to the SUPERVISOR for completion after the hazardous waste has been identified.
3.3 If the contractor, while performing work at a Government facility, cannot obtain a separate generator identification number from the state in which the availability will be performed, the contractor must notify the SUPERVISOR within three business days of receipt of written notification by the state. After obtaining approval of the SUPERVISOR, the contractor must use the Navy site generator identification number and insert in the remarks block the contractor generator identification number issued for the site where his main facilities are located.
3.4 If, for availabilities at a contractor-owned or controlled facility, the Navy cannot obtain a separate generator identification number for use at a contractor facility, the Navy must notify the contractor within three business days of receipt of notification by the state. The contractor must dispose of hazardous waste in accordance with 2.1, 2.2, and 3.2.3.
