QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

NRTL and IPE Evaluation

IN SUPPORT OF US NAVY’S SUBMARINE MAINTENANCE PROGRAM

1. PURPOSE

This Quality Assurance Surveillance Plan (QASP) is a Government developed and applied document used to make sure that systematic quality assurance methods are used in the administration of the Performance Based Service Contract (PBSC) standards included in this contract and in subsequent task orders issued there under. The intent is to ensure that the Contractor performs in accordance with performance metrics set forth in the contract documents, that the Government receives the quality of services called for in the contract and that the Government only pays for the acceptable level of services received.

1. AUTHORITY

Authority for issuance if this QASP is provided under FAR 52.212-4(a), Inspection/Acceptance, and FAR 52.212-4(m), Termination for cause and documentation called for in the contract to be accomplished by the Contracting Office or his duly authorized representative.

1. SCOPE

The QASP is put in place to provide Government surveillance oversight of the Contractor’s quality control efforts to assure that they are timely, effective and are delivering the results specified in the contract or task order. The QASP is not a part of the contract nor is it intended to duplicate the Contractor’s Management Plan.

If it is determined that Technical Instructions (TI) are needed or required, it will be the responsibility of Government to provide them in writing to the contractor. The government may also require the contractor to provide a copy of their TI. The purpose is to provide guidance, clarify the Scope of Work, while remaining within the stipulated scope of the contract. In conjuction with the Scope of Work it should, simultaneously also develop and refine the QASP.

1. GOVERNMENT RESOURCES

The following definitions for Government resources are applicable to this plan:

Contracting Officer - A person duly appointed with the authority to enter into, administer, or terminate contracts and make related determinations and findings on behalf of the Government.

Contracting Officer’s Representative (COR) - An individual designated in writing by the Contracting Officer to act as his authorized representative to assist in administering a contract. The source and authority for a COR is the Contracting Officer. COR limitations are contained in the written letter of appointment.

1. RESPONSIBILITIES

The Government resources shall have responsibilities for the implementation of this QASP as follows:

Contracting Officer – The Contracting Officer ensures performance of all necessary actions for effective contracting, ensures compliance with the terms of the contract and safeguards the interests of the United States in the contractual relationship. It is the Contracting Officer that assures the Contractor receives impartial, fair, and equitable treatment under the contract. The Contracting Officer is ultimately responsible for the final determination of the adequacy of the Contractor’s performance.

Contracting Officer’s Representative (COR) - The COR is responsible for assuring proper Government surveillance of the Contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. Any changes that the Contractor deems may affect contract, price, terms, or conditions shall be referred to the Contracting Officer for action. The COR will serve as the Quality Assurance Evaluator (QAE).

1. METHODS OF QUALITY ASSURANCE SURVEILLANCE

The below listed methods of surveillance shall be used in the administration of this QASP.

The method used in the administration of this QASP is survailence and will be used to validate performance objectives. All documented surveillances, meetings, E-mails, phone calls and texts are cosiderad survailences. Surveillance are to be conducted by; Code 420 with alternate surveillances provided by Code 980 and Code 106. All surveillances are to documented and turned in to C420. Code 420 will maintain these records and use them as written Objective Quality Evidence (OQE) to support weather the contractor has met or not met the performance objectives. Written OQE will also be used to support.

1. Is to track, identify and validate positive trends and eliminate negative trends, that are idenfied as perforemnce objectives. Cost.

2. Surveillances are to be performed monthly at a minimum, all documented surveillances can be used.

Inspections – Each phase of the services rendered under the contract is subject to Government inspection during the Contractor’s operations and after completion of a task. Inspections may be routine functions performed by the COR. Government inspections may be conducted in an unannounced manner by the COR. The COR shall maintain a Contract Discrepancy Report (CDR), a copy of which will be provided to the Contractor. Within three days, the Contractor shall reply in writing to CDR(s) by stating reason(s) for unsatisfactory performance(s) and shall identify the corrective action(s) that will be taken to prevent recurrence(s).

Performance – The contractor provided 3rd party NRTL inspection services from a trained and qualified employee of an OSHA recognized NRTL who is authorized to certify and label the Government owned equipment with a registered certification mark or label to designate that the equipment is in conformance with the applicable safety and technical standards.

Deliverables – NRTL Field Evaluation Reports shall be provided for all equipment evaluated on a given NRTL evaluation visit. The NRTL Field Evaluation Report shall be provided within 30 calendar days of the evaluation visit.

7. DOCUMENTATION

The COR will, in addition to providing documentation to the Contracting Officer, maintain a complete Elctronic Quality Assurance file or COR Folder. All such records will be retained for the life of this contract. Information in the Quality Assurance file will be considered when completing the annual Contractors Performance Assessment Reporting Systems (CPARs) report.
