1. SCOPE:

3. REQUIREMENTS:

3.1 Manage and dispose of all hazardous waste listed in 3.5 in accordance with 2.1 and 2.2.

3.1.1 When a Navy generator number is required by this Work Item, submit the original of 2.3 to the SUPERVISOR for assignment of Environmental Protection Agency (EPA) or delegated state environmental agency identification number.

3.1.2 Manage and transport for Navy disposal, Navy-generated

hazardous waste listed in 3.5 in accordance with 2.1 and 2.2, as designated by the SUPERVISOR.

3.1.3 Submit one legible copy of 2.3 signed by the owner or operator of the disposal facility to the SUPERVISOR within 48 hours of receipt from owner or operator of disposal facility.

3.2 Complete documentation required by 2.1 and 2.2, using EPA or delegated state environmental agency identification number in accordance with 2.4.

3.3 If the contractor, while performing work at a Government facility, cannot obtain a separate generator identification number from the state in which the availability will be performed, the contractor shall notify the SUPERVISOR within three business days of receipt of written notification by the state. After obtaining approval of the SUPERVISOR, the contractor shall use the Navy site generator identification number and insert in the remarks block the contractor generator identification number issued for the site where his main facilities are located.

3.4 If, for availabilities at a contractor-owned or controlled facility, the Navy cannot obtain a separate generator identification number for use at a contractor facility, the Navy shall notify the contractor within three business days of receipt of notification by the state. The contractor shall dispose of hazardous waste in accordance with 2.1, 2.2, SHIP: USS CHICAGO (SSN 721)

3.5.1 Provide 0 dollars for managing and disposing of all hazwaste listed in 3.5. Total cost greater or less than dollar amount when authorized will be the subject of an equitable adjustment.

3.6 Notify the SUPERVISOR at least one working day prior to shipment of hazardous waste for disposal.

3.7 Submit one legible copy, in approved transferrable media, of a report identifying type, amount, and disposal cost of waste listed in 3.5 that was removed during the performance of this Job Order to the

SUPERVISOR.

3.7.1 The report shall include analysis or other method used to identify the waste and state whether each listed waste was hazardous (with generator assignment), non-hazardous, or did not exist.

3.7.1.1 Chemical analysis shall be accomplished by laboratories with state or EPA approved quality assurance programs.

3.7.2 The contractor shall make an effort to minimize hazardous waste generation by reducing the volume or toxicity by neutralizing, recycling, or otherwise removing it from the requirements of Subtitle C of 2.1 and include a description of such efforts in the report.

3.8 Nothing contained in this Work Item shall relieve the contractor from complying with applicable federal, state, and local laws, codes, ordinances, and regulations, including the obtaining of licenses and permits in connection with hazardous waste handling and disposal in the performance of this contract.

4. NOTES:

4.1 The waste listed in 3.5 is based on the best information available at the time of preparation of the solicitation. Hazardous waste generated during the actual performance of the work may vary in type or amount from waste listed in 3.5 which may result in renegotiation for credit or increase pursuant to Paragraph (b) of 2.4. The contractor is expected to use best management practice to identify and dispose of all hazardous waste. Some of the substances listed in 3.5 may be neutralized, recycled, or otherwise removed from the requirements of

Subtitle C of 2.1. Inclusion of these substances in the waste listed in

3.5 does not preclude the contractor from taking action consistent with

2.1 to reduce or eliminate the hazardous constituents of any waste required to be disposed of under the contract in accordance with 2.2.

Processes that add hazardous constituents to the bilges may require that bilge water be disposed of as a hazardous waste.

4.1.1 The types and amounts of wastes listed in 3.5 are estimates of waste to be disposed of under this contract as required by 2.4. They are not estimates of the amount of the work involved in generating that waste. The work requirements of each individual Work Item specify the actual work to be accomplished.

4.2 Hazardous wastes are determined by one or more of the following methods:

4.2.1 Chemical analysis which shows that the material

characteristics of ignitability, corrosivity, reactivity, and/or toxicity (Toxicity Characteristic Leachate Procedure - TCLP) exceed the limits for that material in 40 CFR 261.20 Subpart C.

4.2.2 Reference to a Material Safety Data Sheet (MSDS).

4.2.3 Applying knowledge of the hazardous characteristics of the waste in light of the materials or the process used.

4.3 Asbestos, bilge water, oil/water including sludge, debris and other contaminants, sludge which includes solids and sludge from ballast tanks, CHT tanks, voids, oily waste tanks, fuel ballast tanks, fuel oil tanks, skegs (West coast), PCB's (Maryland), etc., apply only in those states listing them as hazardous waste. When an availability is to be performed in a state where these items are hazardous waste, an estimate of the amount to be generated shall be included in 3.5.

3. REQUIREMENTS:

3.1 The contractor shall provide a safety and health plan as applicable to the contract work per the requirements of 2.2 prior to the start of work (i.e. hazard communication program, confine space program, respiratory protection plan, emergency response plan, etc.).

3.1.1 Submit one legible copy, in approved transferrable media, of a report with the requirements of 3.1 to the SUPERVISOR.

3.2 The contractor shall provide all cleaning equipment, holding tank(s), machinery, material, lights, ventilation, transportation, and disposal service to accomplish work required in this work item. The contractor shall ensure that all equipment used does not exceed the height requirements listed in 2.5.

3.2.1 In the event of a partial or total loss of electrical power or compressed air at the dockside source outlets, the contractor shall provide standby portable diesel electric generator(s), air compressor(s), and associated equipment to maintain and restore backup shore power and integrity of all temporary utilities with equal capability of systems disrupted in the performance of his work.

3.2.2 The contractor shall provide 440 volts, 3 phase circuit breakers at dockside for his own equipment. The contractor shall provide step-down transformer/breakers and shall run all his leads from these connections. The contractor is responsible for any modifications required to hook up his leads/make up his connections.

3.2.2.1 All service leads (air, water and electricity etc.)

shall be properly secured and routed in such a manner as to not present a tripping or other safety hazard. The service leads shall be protected by chafing material as required to prevent damage.

3.2.3 The Government will provide source outlets from dockside and hook-ups for air, water and electricity. The contractor shall supply his own connectors and manifolds and run all his leads and hoses from these connections. The contractor is responsible for all modifications required to hook-up his leads/make up connections.

3.3 Clean, ventilate, gas free and certify the tank listed in 1.2 "Safe for Workers" and "Safe for Hot Work" in accordance with 2.3.

3.3.1 Submit one legible copy, in approved transferrable media, of certified marine chemist certification to the SUPERVISOR within 4 hours.

3.4 The tanks listed in 1.2 shall be cleaned to achieve the cleanliness level of Society for Protective Coatings Surface Preparation Standard No.1 (SSPC-SP 1) of 2.6.

3.5 Accomplish the requirements of 009-09 of 2.1 for the method of cleaning, safety/health guidelines, handling, removal and DISPOSAL OF RESIDUALS in the tanks listed in 1.2 in accordance with reference 2.4.

Include the method of cleaning with solvents. Each procedure shall include the following:

3.5.1 Type of cleaning equipment and holding tanks to be utilized during the cleaning evolution of tanks.

3.5.2 Method of ensuring safe operation of cleaning equipment.

3.5.3 Method utilized and acceptance criteria to ensure complete removal of thick sludge and surface contaminants.

3.5.4 Personnel safety/training of procedure and hazardous

communication prior to start of work.

3.5.5 Providing the service of a qualified safety/environmental coordinator to accomplish work required in this work item.

3.5.6 Method of providing ventilation in each tank during cleaning.

3.5.7 Method of isolating/containing work space and change area by the use of containments.

3.5.8 Method of disposal of sludge and waste water.

3.5.9 Method where all surfaces of the tanks listed in 1.2 shall be cleaned free of waste and calcium deposits. Removal shall be accomplished by light scraping and power washing to a degree that enables inspection of the top layer of paint. In addition, hand wiping with rags and non-petroleum based solvents/cleaners may be required to achieve SSPC-SP1 cleanliness. Particular attention shall be paid to tank cover hangers, TLI hangers, fittings, tubing, anodes, tank corner welds and underside of horizontal channels.

3.5.10 Non-ionic detergent and degreasing agents shall be specified by the Contractor and meet all applicable mil-spec requirements. A final low pressure detergent wash shall be accomplished as required.

3.5.11 Contractor shall prohibit the wearing of any "yellow" colored clothing or coveralls and utilizing any "yellow" colored containers, equipment etc.

3.5.12 Contractor shall ensure current physical examinations (within one year), including current dysentery inoculations (within 6 months) for all personnel working in the tank listed in 1.2 in accordance

3.5.13 Subject tanks interior surfaces are defined as: "all

structural members, bulkheads, piping, piping penetrations, hangers and tank cover's interior side".

(V)(G) "SLUDGE REMOVAL"

3.5.14 Visual inspection for sludge removal. Accept/reject criteria:

Surfaces shall be completely free of solids, semi-solid residuals, oil, and oily film in tank surfaces. Inspection mirror shall be used to inspect the underside of horizontal channels and hard to reach areas.

3.5.15 Ensure tank internal components are not damaged during cleaning evolutions.

3.6 Loose, flaking and blistering paint is sufficient cause to suspect underlying sludge and will be removed as part of the cleaning process.

3.6.1 Approximately 100 to 200 square feet of loose and flaking epoxy paint on the steel structure of each tank listed in 1.2 will require removal to the steel substrate.

3.6.2 Submit one legible copy, in approved transferrable media, of a report with the results of 3.6.1 to the SUPERVISOR within 24 hours of completion of paint removal.

3.6.2.1 Report shall include the approximate square footage of paint removed from each tank listed in 1.2.

(I)(G) "DISINFECTION /SANITIZATION"

3.7 Accomplish the disinfection / sanitization of the tanks listed in 1.2.

3.7.1 Spray down the interior of the tanks listed in 1.2 using a solution of ¾ cup of sodium hypochlorite per gallon of water.

3.7.2 Allow disinfection / sanitization solution to sit for a minimum of 5 minutes.

3.7.3 Accomplish a low pressure wash down of the tanks listed in 1.2 to remove remaining disinfection / sanitization solution.

3.8 Upon completion of the requirements of 3.7, completely pump down each tank listed in 1.2 to a dry condition.

(I)(G) "CLEANLINESS"

3.9 Inspect each tank listed in 1.2 for SSPC-SP1 Cleanliness.

3.9.1 Notify the SUPERVISOR for surface cleanliness and profile inspection of tank interior surfaces. Advanced notice of 24 hours is required for inspection notification.

3.9.2 Inspection mirror shall be used to inspect the backside of stiffeners and hard to reach areas.

3.9.3 Submit one legible copy, in approved transferrable media, of a report with the results of 3.9 to the SUPERVISOR within 24 hours of

3.10 Accomplish handling and commercial disposal of tank cleaning residuals and a total of 1,600 gallons of waste water, in accordance with all applicable Federal, state, and local regulations, laws, codes ordinances, and regulations, including the obtaining of licenses and permits.

3.10.1 Accomplish the pump down of pier side waste and transport waste and holding tanks to a secondary location as designated by the

SUPERVISOR.

3.10.2 Provide and maintain temporary storage of tank cleaning residuals from the tanks listed in 1.2 while waiting for waste water sample laboratory results for ninety (90) days. Ensure proper secondary containment of liquid waste holding tanks as necessary.

3.11 Take samples of waste water and solid sludge residuals as directed by PHNSY Code 106.3.

3.11.1 Sample containers will be provided by PHNSY Code 106.3.

3.11.2 Ensure samples are tested to certify that disinfection / sanitization of the tanks listed in 1.2 have been satisfactorily accomplished and is safe for personnel.

3.11.3 Samples shall be tested for presence of both E.Coli and Enterococci.

3.11.3.1 E. Coli levels must be less than 235 CFU/100mL

3.11.3.2 Enterococci levels must be less than 61 CFU/100mL

3.12 Upon completion of Work Item in its entirety, submit one legible copy, in approved transferrable media, of a report notifying the GOVERNMENT of job completion to the SUPERVISOR.

3.12.1 The Contractor shall certify the work has been completed, all checkpoints have been accomplished, the requirements of all applicable Standard Items have been met, and all reports have been submitted, received, and answered by the SUPERVISOR.

4. NOTES:

4.1 Contact PHNSY & IMF Code 106.3 for direction and guidance in the handling and disposal of tank cleaning residuals.

4.1.1 The tank listed in 1.2 is a storage tank for human waste.

4.2 Work to be accomplished in Pearl Harbor Naval Shipyard, Industrial Control Area (CIA). Contractor personnel must be U.S. Citizens and have successfully completed security access determination IAW NAVSEAINST 5510.2B.

4.3 Ship's Force will be responsible for the initial chlorination, pump down / blow down and opening of the tanks listed in 1.2 prior to the start of work.

3.1 The contractor shall provide a safety and health plan as applicable to the contract work per the requirements of 2.2 prior to the start of work (i.e. hazard communication program, confine space program, respiratory protection plan, emergency response plan, etc.).

3.1.1 Submit one legible copy, in approved transferrable media, of a report with the requirements of 3.1 to the SUPERVISOR.

3.2 The contractor shall provide all cleaning equipment, holding tank(s), machinery, material, lights, ventilation, transportation, and disposal service to accomplish work required in this work item. The contractor shall ensure that all equipment used does not exceed the height requirements listed in 2.5.

3.2.1 In the event of a partial or total loss of electrical power or compressed air at the dockside source outlets, the contractor shall provide standby portable diesel electric generator(s), air compressor(s), and associated equipment to maintain and restore backup shore power and integrity of all temporary utilities with equal capability of systems disrupted in the performance of his work.

3.2.2 The contractor shall provide 440 volts, 3 phase circuit breakers at dockside for his own equipment. The contractor shall provide step-down transformer/breakers and shall run all his leads from these connections. The contractor is responsible for any modifications required to hook up his leads/make up his connections.

3.2.2.1 All service leads (air, water and electricity etc.)

shall be properly secured and routed in such a manner as to not present a tripping or other safety hazard. The service leads shall be protected by chafing material as required to prevent damage.

3.2.3 The Government will provide source outlets from dockside and hook-ups for air, water and electricity. The contractor shall supply

SSP: HRMC-001-18 2 of 6 ITEM NO: 593-11-002 his own connectors and manifolds and run all his leads and hoses from these connections. The contractor is responsible for all modifications required to hook-up his leads/make up connections.

3.3 Clean, vent, gas free and certify the tank listed in 1.2 "Safe for Workers" and "Safe for Hot Work" in accordance with 2.3.

3.3.1 Submit one legible copy, in approved transferrable media, of certified marine chemist certification to the SUPERVISOR within 4 hours.

3.4 Accomplish the requirements of 009-09 of 2.1 for the method of cleaning, safety/health guidelines, handling, removal and DISPOSAL OF RESIDUALS in the tank listed in 1.2 in accordance with reference 2.4.

Include the method of boiling out (flushing) with trisodium phosphate (TSP). Each procedure shall include the following:

3.4.1 The Contractor shall only use Type 1 (Anhydrous) TSP for the execution of this work item.

3.4.2 Type of cleaning equipment and holding tanks to be utilized during the cleaning evolution of tanks.

3.4.3 Method of ensuring safe operation of cleaning equipment.

3.4.4 Method utilized and acceptance criteria to ensure complete removal of thick sludge, grease and surface contaminants.

3.4.5 Personnel safety/training of procedure and hazardous

communication prior to start of work.

3.4.6 Providing the service of a qualified safety/environmental coordinator to accomplish work required in this work item.

3.4.7 Method of providing ventilation in each tank during cleaning.

3.4.8 Method of isolating/containing work space and change area by the use of containments.

3.4.9 Method of disposal of sludge, grease, and waste water.

3.4.10 Method where all surfaces of each tank listed in 1.2 shall be cleaned free of waste and calcium deposits. Removal shall be accomplished by light scraping and power washing to a degree that enables inspection of the top layer of paint. Particular attention shall be paid to tank cover hangers, TLI hangers, fittings, tubing, tank corner welds and underside of horizontal channels.

3.4.11 Non-ionic detergent and degreasing agents shall be specified by the Contractor and meet all applicable mil-spec requirements. A final low pressure detergent wash shall be accomplished as required.

3.4.12 Contractor shall prohibit the wearing of any "yellow" colored clothing or coveralls and utilizing any "yellow" colored containers, equipment etc.

3.4.13 Contractor shall ensure current physical examinations (within one year), including current dysentery inoculations (within 6 months) for all personnel working in the tank listed in 1.2 in accordance with 2.4.

3.4.14 Subject tank interior surfaces are defined as: "all

structural members, bulkheads, piping, piping penetrations, hangers and tank cover's interior side".

(V)(G) "GREASE REMOVAL"

3.4.15 Visual inspection for grease removal. Accept/reject criteria:

Surfaces shall be completely free of solids, semi-solid residuals, oil, and grease film in tank surfaces. Inspection mirror shall be used to inspect the underside of horizontal channels and hard to reach areas.

3.4.16 Ensure tank internal components are not damaged during cleaning evolutions.

3.5 Method where cleaning using TSP boil out is accomplished in the tank listed in 1.2 shall be incorporated into the Process Control Procedure

(PCP).

3.5.1 Set up an emergency eye wash facility or portable eye wash station within 10 seconds walk from the mixing tank.

3.5.2 Ensure that the opening to the tank listed in 1.2 is blanked.

3.5.3 Install a steam dispersing line, or a heat circulating line inside the tank listed in 1.2.

3.5.4 Connect a drain pipe from the temporary boiling cover to approximately six inches from the bottom of the tank listed in 1.2.

Connect the steam dispersing pipe to the boiling cover. Install boiling cover and cover gasket.

3.5.5 Connect a hose from the appropriate boiling cover nipple overboard to provide a vent to the Sanitary Tank. Ensure that there are no obstructions, kinks or sharp bends in this line and that it vents properly.

3.5.6 Check and secure all openings where cleaning operations are to be accomplished in order to minimize contamination to other areas.

(I) "TSP VERIFICATION"

3.5.7 Fill a pre-heating tank located dockside with 2,000 gallons of clean, fresh water. Add 300 pounds of TSP and 18 gallons of non-ionic detergent. Heat the solution between 140 degrees F to 160 degrees F. The Contractor shall ensure that only Type 1 (Anhydrous) TSP has been used.

3.5.7.1 Do not add TSP to hot water. Violent reactions occur

when TSP is added to hot water.

3.5.7.2 Ensure all personnel entering the area have proper

Personal Protective Equipment (PPE).

3.5.7.3 Submit one legible copy, in approved transferrable

media, of a report listing the results of 3.5.7 to the

SUPERVISOR.

3.5.8 Pump the heated solution from the dockside tank through the appropriate nipple, using a pump suitable for alkalis.

3.5.9 Continue filling with clean fresh water heated to 140 degrees F to 160 degrees F until the tank listed in 1.2 is filled to within six inches of the top of the tank. Determine the water level with a

3.5.10 Connect heating line to the one-inch boiling cover nipple.

3.5.11 Open the Heat supply valve in small increments until a sufficient amount of heat is entering the tank listed in 1.2 to heat and circulate the water. Continue to feed heat into the tank, keeping the solution between 140 degrees F to 160 degrees F for a minimum of 16 hours.

3.5.12 Monitor the water level in the tank listed in 1.2 with a dipstick or floater (remove the pipe cap as necessary). Maintain a level within six inches of the tank top. If the water level rises to less than six inches from the tank top, lower it by pumping out some of the solution. If the water level drops to more than six inches from the tank top, secure operations and check for leaks. After correcting any leaks, readjust the water level in the tank to its proper level.

3.5.13 At the termination of the boiling period, pump down the tank listed in 1.2 using a pump suitable for alkalis. Discharge the contents into the spent liquid holding tank.

3.5.13.1 Accomplish pH testing of the Trisodium Phosphate

Anhydrous waste as directed by the SUPERVISOR. Acceptable pH level: less than 12.5.

3.5.13.2 Submit one legible copy, in approved transferrable

media, of a report listing the results of 3.5.13.1 to the

SUPERVISOR.

3.5.14 Fill the tank listed in 1.2 to within six inches from the tank top with clean, fresh water heated to 140 degrees F to 160 degrees F.

3.5.15 Pump down the tank listed in 1.2 to low suction.

3.5.16 Remove all unnecessary hoses and vent tank. Allow tank to cool down for a minimum of 12 hours.

3.6 Accomplish handling and disposal of tank cleaning residuals, for a total of 5,000 gallons, in accordance with all applicable Federal, state, and local regulations, laws, codes ordinances, and regulations, including the obtaining of licenses and permits.

3.6.1 Accomplish the pump down of pier side waste and transport the waste and holding tanks to a secondary location as designated by the

SUPERVISOR.

3.6.2 Provide and maintain temporary storage of tank cleaning residuals from the tank listed in 1.2 while awaiting waste water sample laboratory results for ninety (90) calendar days. Ensure proper secondary containment of liquid waste holding tanks as necessary.

(I)(G) "CLEANLINESS"

3.7 Inspect the tank listed in 1.2 for cleanliness. Accept/reject criteria:

Cleanliness is defined as no visible or detectable by touch of oil, grease, debris or any surface contaminants.

3.7.1 Notify the SUPERVISOR for surface cleanliness and profile inspection of tank interior surfaces. Advance notice of one work day is required for inspection notification.

3.7.2 Inspection mirror shall be used to inspect the underside of horizontal channels and hard to reach areas.

3.8 Take samples of TSP boil out liquid waste, rinse water with detergent, and of solid grease residuals with PHNSY Code 106.3.

3.9 Transport and dispose generated waste in 3.6 and 3.8 to the disposal waste facility identified in 4.2

3.9.1 While awaiting disposal, the contractor shall inspect the tank daily for discrepancies for ninety (90) calendar days.

3.10 Upon completion of Work Item in its entirety, submit one legible copy, in approved transferrable media, of a report notifying the GOVERNMENT of job completion to the SUPERVISOR.

3.10.1 The Contractor shall certify the work has been completed, all checkpoints have been accomplished, the requirements of all applicable Standard Items have been met, and all reports have been submitted, received, and answered by the SUPERVISOR.

4. NOTES:

4.1 Contact PHNSY & IMF Code 106.3 for direction and guidance in the handling and disposal of tank cleaning residuals.

4.2 The Solid Waste Management Permit has been modified recently and now the permittee (PVT Land Company, Ltd.) allows the disposal of tank heating only in their solidification facility for liquid wastes.

Contractor(s) and their subcontractor(s) shall comply with applicable Federal, state and local laws, codes, ordinances and regulations, including the obtaining of licenses and permits in connection with industrial waste handling, transporting and disposal. The TSP Boilout portion shall be handled, transported and disposed at: PVT Land Company, Ltd. located at 87-2020 Farrington Highway Waianae, HI 96792.

4.2.1 All hazardous waste generated, including grease sludge and contaminated cloth rags, will be turned in to PHNSY Hazardous Waste Branch, BLDG.1663.

4.3 Work to be accomplished in Pearl Harbor Naval Shipyard, Industrial Control Area (CIA). Contractor personnel must be U.S. Citizens and have successfully completed security access determination IAW NAVSEAINST 5510.2B

4.4 Ship's Force will be responsible for the initial chlorination, pump down / blow down, and opening of the tank listed in 1.2 prior to the start of work.

Surfaces shall be completely free of solids, semi-solid residuals, oil, and oily film in tank surfaces. Inspection mirror shall be used to inspect the underside of horizontal channels and hard to reach areas.

3.5.15 Ensure tank internal components are not damaged during cleaning evolutions.

3.6 Loose, flaking and blistering paint is sufficient cause to suspect underlying sludge and will be removed as part of the cleaning process.

3.6.1 Approximately 30 to 50 square feet of loose and flaking epoxy paint on the steel structure of each tank listed in 1.2 will require removal to the steel substrate.

3.6.2 Submit one legible copy, in approved transferrable media, of a report with the results of 3.6.1 to the SUPERVISOR within 24 hours of completion of paint removal.

3.6.2.1 Report shall include the approximate square footage of paint removed from each tank listed in 1.2.

(I)(G) "DISINFECTION /SANITIZATION"

3.7 Accomplish the disinfection / sanitization of the tanks listed in 1.2.

3.7.1 Spray down the interior of the tanks listed in 1.2 using a solution of ¾ cup of sodium hypochlorite per gallon of water.

3.7.2 Allow disinfection / sanitization solution to sit for a minimum of 5 minutes.

3.7.3 Accomplish a low pressure wash down of the tanks listed in 1.2 to remove remaining disinfection / sanitization solution.

3.8 Upon completion of the requirements of 3.7, completely pump down each tank listed in 1.2 to a dry condition.

3.9 Inspect each tank listed in 1.2 for SSPC-SP1 Cleanliness.
