5.0 NOTES

5.1 The contractor and all subcontractors, regardless of tier must consult the General Technical Requirements (GTR) to determine applicability to this work item. In performance of this work item, the contractor, and all subcontractors regardless of tier must comply with the requirements of all applicable GTRs including but not limited to GTRs 1 through 7, 22, 23, 24, 28, and 29.

5.2 All work listed under the Statement of Work shall be completed on EACH of the following T-AKE Class vessels during the listed Period of Performance except where otherwise noted:

5.3 Performance Location and Period of Performance dates provided are best known locations and dates at time of submission of these requirements and are subject to change. A Condition Report will be submitted to the OMT REP and Contracting Officer requesting modification of the contract due

to travel costs.

5.4 Provide an access list of all personnel boarding the ship. The list shall include the following:

5.4.1 Persons full name

5.5 The Contractor is responsible for proper and timely submittal of all access requirements to include, but not limited to, EPIC and base/port access.

5.6 The following is mandatory training required to be completed NO LATER THAN 3 (three) weeks prior to travel to Diego Garcia:

5.6.1 Complete ISOPREP via the DD Form 1833 or online. This information stays in the contractor's hands. DO NOT FORWARD to Military Sealift Command. NOTE: DO NOT FILL IN RED SECTION of DD Form 1833.

5.6.2 Complete Antiterrorism Level I Training (must be completed within 12 months of travel) https://atlevel1.dtic.mil/at training is valid for 1 year.

Time of training is approximately 1 hour, so print certificate and record completion date upon completion of training. This certificate can't be reprinted if lost.

5.6.3 Survival, Evasion Resistance, and Escape (SERE) Course 100.1 (must be completed within 12 months of travel) https://jkodirect.jten.mil/Atlas2 /faces/page/login/Login.seam training is valid for 3 years. Time of training is approximately 3 hours, so print certificate and record completion date upon completion of training. This certificate can't be reprinted if lost.

5.6.5 Provide the Port Engineer the following information, so he may complete the Aircraft and Personnel Automated Clearance System (APACS) for Island Clearance and schedule the AMC Flight from either TBA or TBA.

a. Provide date ISOPREP DD Form 1833 was completed/reviewed.

b. Provide date of Antiterrorism Level I training

c. Provide date of SERE training

d. Supervisors job title

5.7 Include the cost of the AMC flight in the proposal, as required. Costs are ONE-WAY: $791 to/from TBA and $975 to/from TBA.

6.0 QUALITY ASSURANCE REQUIREMENTS: None additional

7.0 STATEMENT OF WORK REQUIRED

7.1 Provide the service of one OEM authorized field service technical representative. The tech rep will be experienced with Appleton Marine Cranes listed in 3.0.

7.2 Provide and operate equipment and supply all services and assistance to accomplish the inspection and servicing of the cargo cranes identified in section 3.0 using References 2.1 through 2.3 as guidance.

7.3 OEM authorized field service technical representative(Ships Force To Assist 7.3.3 through 7.3.40)

7.3.1 Travel to the locations described in section 5.0.

7.3.2 Notify OMT REP prior to starting any work.

7.3.3 Perform operational checks on each crane.

7.3.4 Perform annual safeties and shutdowns of each crane.

7.3.5 Inspect wire rope on each crane.

7.3.6 Inspect the swing drive gearbox oil level on each crane.

7.3.7 Inspect the winch oil level on each crane.

7.3.8 Inspect the bearing gear and pinion teeth for lubrication and excessive wear on each crane.

7.3.9 Inspect hydraulic hoses on each crane.

7.3.10 Inspect electric cables on each crane.

7.3.11 Visually inspect the main boom heel pin and bushings on each crane.

7.3.12 Inspect the cylinder pins and sheave pins on each crane.

7.3.13 Inspect the sheaves on each crane.

7.3.14 Inspect the Electric Motor on each crane.

7.3.15 Verify correct HPU System Pressure on each crane.

7.3.16 Visually Inspect the Flood Light Mounting Brackets on each crane.

7.3.41 Provide hands on technical trouble shooting training to the crew throughout the testing evolution.

7.4 Coordinate with ship's force to have associated equipment locked and/or tagged out per ship's lock out/tag out policy.

7.5 Maintenance and Inspection (To Be Conducted By Ships Force)

7.5.1 Lubricate the luff cylinder rods on each crane.

7.5.2 Lubricate the bearing gear and pinion teeth on each crane.

7.5.3 Replace the Return Filter Element on each crane.

7.5.4 Inspect and Replace Windshield Wiper Blade as required on each crane.

7.5.5 Replace the EEPROM backed flash memory battery.

7.5.6 Replace the Swing Drive Gear Oil on each crane.

7.5.7 Replace the Winch Gear Oil on each crane.

7.5.8 Replace the Hydraulic Oil on each crane.

7.5.9 When the maintenance is complete, coordinate with ship's force to have associated equipment unlocked and/or tags cleared per ship's lock out /tag out policy.

7.5.10 Prep the surface prior to obtaining results of Rocking Test. Surfaces between the Slew Ring and the Crane foundation to be cleaned and free of grease, cosmoline, or dirt on each crane.

7.6 Return equipment to readiness condition.

7.7 Reports

7.7.1 When inspection, service and testing reveal any deficiency a condition report is to be submitted to the OMT REP with recommended repairs and parts required. Submit one electronic copy in Portable Document Format (PDF).

7.7.2 Submit a report that includes "as released" condition of the cargo crane. Submit one electronic copy in Portable Document Format (PDF).

7.7.3 All reports and checklists are to be completed and signed by the person who carried out the inspection and maintenance work and countersigned by the Company's representative.

7.8 Coating/Lagging

7.8.1 Prepare, prime, coat and restore all new, soiled, and disturbed surfaces to match surrounding surfaces.

7.8.2 Renew lagging where damaged or disturbed.

7.9 Manufacturer's Representative

7.9.1 Provide the services of an OEM authorized field service representative to accomplish all work and testing on the cargo crane identified in paragraph 3.0 ensuring compliance with the manufacturer's standards and performance specifications.

7.9.2 This work item requires maintenance and repair actions to be performed on a MSC ship critical safety item. Only original equipment manufacturer (OEM) authorized technical field service providers or MSC qualified non-OEM technical field service providers and OEM authorized or MSC qualified non-OEM parts shall be used to accomplish the requirements of this work item for this ship critical safety item including oversight and guidance on all aspects of equipment as-found condition inspection, removal, disassembly, reassembly, repairs, modifications, reinstallation, and testing as applicable. The technical service provider is to attend dock and sea trials.

7.9.5 Provide the OMT REP with a signed letter of qualification from the Original Equipment Manufacturer and certificate of training for the field service representative for the equipment listed in paragraph 3.0 prior to the start of any work.

7.10.1 Provide electronic copies of all revised and/or updated technical documentation including drawings, technical manuals, service bulletins, parts lists, certification and test data associated with the work performed or parts supplied to the OMT REP.
