1. This solicitation is for the procurement of description in support of customer. It is anticipated that this solicitation will result in an award/order of a Firm, Fixed-Price Service type contract. Payment will be by Government Purchase Card.
2. The entire solicitation shall be filled out, signed, and returned as part of your quote. See block 8 on page 1 of the solicitation for the closing date and time.
3. The deadline for submission of questions regarding the solicitation’s terms, conditions, specifications, and/or the

Therefore, bidders who quote less than the entire effort specified herein may be determined to be unacceptable.

PERFORMANCE WORK STATEMENT
NAVAL SUBMARINE SUPPORT CENTER
GENERAL – All work under this contract shall, unless otherwise indicated, be accomplished in accordance with best commercial standards utilizing materials and methods that are not deleterious to the fabrics. No Dry Cleaning is required under this contract.
PICKUP AND DELIVERY POINTS AND TIMES – the location for pickup and delivery time are set forth below:

For occasions when service locations experience a surge increase in volume of laundry requiring an unscheduled pickup/delivery, request for unscheduled pickup/delivery will be performed by the contractor with a minimum of 1 day prior notification to accommodate the surge in laundry volume.
HANDLING - The contractor shall be responsible for loss or damage for all articles of laundry (Government owned) in his possession. When the laundry is in possession of the Contractor, it shall be adequately stored and protected. Mixing of the dirty and clean articles is prohibited. The laundry facility shall establish airflow and ensure solid physical separation between clean and contaminated laundry articles at the laundry facility. Laundry that has been segregated by the customer shall be returned in the same manner.
VEHICLE / EQUIPMENT – Vehicles used to transport laundry shall be clean, conform to commercial standards, and be equipped to protect against laundry contamination. Clean and contaminated (soiled) laundry articles may be maintained in transport simultaneously, within the same vehicle, as long as it is contained in distinctly separate covered containers. Vehicles shall be subject to examination at any time by the Surveillance Officer.
INVOICING – In addition to invoice clauses set forth and referenced herein, the Contractor shall invoice on a monthly basis by location. Contractor shall provide copies of the delivery tickets/receipts with each invoice.
Monthly invoices and delivery receipts shall show a deduction from gross weight made to allow for the weight of the containers. Billing is to be made based on the minimum laundry volume per pick-up location; or the actual finished clean weight, if greater than the minimum. The contractor will bill and be paid for the number of pounds serviced multiplied by the rate set forth in the schedule.
Delivery Ticket. The contractor shall provide a total weight count in and out. The weight count for soiled in and clean out is documented on the Delivery Receipt. The Delivery receipt will be signed and recorded by the
Government. Empty cart weight will be attached.
The Contractor shall provide a piece count of items processed with all clean laundry returned. Such record shall be typed or computer generated, with items listed on an 8 ½” x 11” sheet or form. Total weight figures (at Contractor’s place of business) for incoming (soiled) and outgoing (finished work) shall also be reflected on the delivery ticket.
Piece count and weight information will be used by the Government to verify invoices for payment as an inventory management tool to identify laundry articles replacement requirements.
LOST / DAMAGED LAUNDRY – Replacement or repair of any lost or damaged laundry shall be in accordance with the clause entitles “252.237-7015 Loss or Damage (weight of Articles)(Dec 1991)”. Unserviceable articles (not fit for use) shall be identified by the contractor prior to packaging and shipment to the customer. This shall be an integral part of the contractor’s Quality Assurance Plan. Articles identified as unserviceable shall be packaged
(bagged) separately from finished work and identified as such and returned to the customer. Final determination as to the disposition of such articles shall be at the discretion of the customer.
LAUNDRY CARTS – The Contractor shall furnish laundry carts to transport laundry to and from the contractor’s facility. All carts shall be cleaned and sanitized prior to being loaded with laundered goods. These carts are property of the contractor and are to be used for delivery purposes only.
PROCESS – The Contractor shall process the laundry in accordance with best commercial standards as follows:
-Perform any necessary pre-wash sorting
-Wash, dry, and fold all items
-Iron all sheets and pillowcases
-Items determined by the Surveillance Officer to be re-laundered (e.g. excessive lint/stains, etc.), will be accomplished at no charge, and shall not exceed 5%, during any monthly period. The Surveillance Officer will track all re-laundry services.
-Clean laundry shall be sorted, weighed and packaged (secured) by like color and kind (i.e. twin sheets, flat, twin sheets fitted, bath towels, hand towels, etc.), by the Contractor in accordance ASTM-D-3951 (most current edition), “Standard for Commercial Packaging”. Sample packaging requirements will be provided to the contractor if required, for employee training and to ensure that specifications are understood and met. Quantities of laundry articles will be packaged as follows:
Bath Mats – 10 per package
Towels – 5 per package
Hand Towels – 10 per package
Wash Cloths – 50 per package
Bed Spreads – 1 per package
Sheets – 10 per package
Pillow Cases – 50 per package
Blankets – 3 per package
Mattress Pads – 5 per package
-Laundry will be returned in sanitized baskets covered with clean (new) plastic covers.
-Any items of concern found on or in laundry (i.e. nametag, pins, etc.), during the receipt, process or return of laundry will be reported to the Surveillance Officer.
-Wool and synthetic articles are to be dried using low heat to prevent deleterious effects. Adequate “cool down” of synthetic textiles and (cotton poly) blends shall be achieved to prevent flash melting. Permanent Press articles shall be removed from dryers “slightly damp”. It is preferred that these articles be further processed using “Steam
Tunnels” or some similar piece of equipment.
DELAYS – The Contractor shall notify the Surveillance Officer when an equipment malfunction or other problems will cause delay in pick-up or delivery of laundry.
WORK STANDARDS - All workmanship, including delivery, shall be of best commercial standards of the laundry industries.
-Fabrics will be clean of all soils, spots, and stains unless unable to remove in the normal process. Fabrics will be free of contamination and processed in accordance to healthcare standards.
-All work will be of a uniform, high-quality, entirely clean, dry and free of excessive lint.
-Laundry articles are to be free of objectionable odors.
-Fabrics will be free of any trace of detergents or laundry processing problems, which might cause irritation to the skin.
ENVIRONMENTAL - The contractor shall comply with all Federal, State, and local jurisdiction, regulatory and administrative code requirements involving Environmental Protection and Hazardous Waste Management.
MISCELLANEOUS - It is emphasized that only the contracting officer has the authority to modify the terms of the contract, therefore, in no event will any understanding, agreement modification, change order or other matters deviating from the terms of the basic contract between the Contractor and any other person be effective or binding on the government.
-When/if in the opinion of the contractor, and effort outside the existing scope of the contract is requested, the contractor shall promptly notify the Contracting Officer in writing. The contractor shall not take action unless the
Contracting Officer has issued a contractual change.
FEDERAL HOLIDAYS - The following days are legally established holidays. There shall be no laundry deliveries or pickups on the Federal Holidays. New Year’s Day (first day of January), Martin Luther King Jr’s Birthday (third
Monday in January), President’s Day (third Monday in February), Memorial Day (last Monday in May), Independence Day (fourth day in July), Labor Day (first Monday in September), Columbus Day (second Monday in
October), Veteran’s Day (eleventh day in November), Thanksgiving Day (fourth Thursday in November) and
Christmas Day (twenty-fifth day in December).

SURVEILLANCE OF FIRM-FIXED-PRICED SERVICES
a. The Surveillance Official(s) shall be responsible for appropriate surveillance of all services to be performed under this contract.
b. When performance is at a government site, the contractor's representative shall contact the government representative named above upon arrival at and departure from the work site. If access to a security area is required, continuous escort service for contractor's representative will be provided by the designated government representative.
c. It is emphasized that only the Contracting Officer has authority to modify the terms of this contract;
therefore in no event will any understanding, agreement, modification, change order, or other matter deviating from the terms of the basic contract between the contractor and any other person be effective or binding on the government. When or if, in the opinion of the contractor, an effort outside of the existing scope of the contract is requested, the Contractor shall promptly notify the contract administrator and/or the
Contracting Officer in writing. No action shall be taken by the contractor under such direction until the contract administrator and/or the Contracting Officer has issued a modification to the contract or has otherwise resolved the issue.

ENTERPRISE-WIDE CONTRACTOR MANPOWER REPORTING APPLICATION
(ECMRA) (IAW 10 USC 235 and 2330a)
The contractor shall report contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for laundry services in support of Naval Submarine Support Center. Contracted services excluded from reporting are based on Product Service Codes (PSCs). The excluded PSCs are:
(1) W, Lease/Rental of Equipment;
(2) X, Lease/Rental of Facilities;
(3) Y, Construction of Structures and Facilities;
(4) D, Automatic Data Processing and Telecommunications, IT and Telecom- Telecommunications
Transmission (D304) and Internet (D322) ONLY;
(5) S, Utilities ONLY;
(6) V, Freight and Shipping ONLY
The contractor is required to completely fill in all required data fields using the following web address https://doncmra.nmci.navy.mil. Reporting inputs will be for the labor executed during the period of performance https://doncmra.nmci.navy.mil/ during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year.

Defense Biometric Identification System (DBIDS):
Commander, Navy Installations Command (CNIC), has established the Defense Biometric Identification System
(DBIDS) to improve management and force protection at Navy installations. The Navy has transitioned from using the Navy Commercial Access Control System (NCACS) to the DBIDS for contractors and vendors requiring access to an installation. DBIDS is a standardized process for granting unescorted access privileges to vendors, contractors, suppliers, and service providers not otherwise entitled to the issuance of a Common Access Card (CAC) who seek access to and can provide justification to enter Navy installations and facilities. Visiting vendors may obtain daily passes directly from the individual Navy Installations by submitting identification credentials for verification and undergoing a criminal screening/background check. Alternatively, if the vendor so chooses, it may voluntarily elect to obtain long-term credentials through enrollment registration, backing vetting, screening, issuance of credentials through a designated independent contractor DBIDS service provider.

52.204-9 PERSONAL IDENTITY VERIFICATION OF CONTRACTOR PERSONNEL (JAN 2011)
(a) The Contractor shall comply with agency personal identity verification procedures identified in the contract that implement Homeland Security Presidential Directive-12 (HSPD-12), Office of Management and Budget (OMB) guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201.
(b) The Contractor shall account for all forms of Government-provided identification issued to the Contractor employees in connection with performance under this contract. The Contractor shall return such identification to the issuing agency at the earliest of any of the following, unless otherwise determined by the Government:
(1) When no longer needed for contract performance.
(2) Upon completion of the Contractor employee's employment.
(3) Upon contract completion or termination.
(c) The Contracting Officer may delay final payment under a contract if the Contractor fails to comply with these requirements.
(d) The Contractor shall insert the substance of this clause, including this paragraph (d), in all subcontracts when the subcontractor`s employees are required to have routine physical access to a Federally-controlled facility and/or routine access to a Federally-controlled information system. It shall be the responsibility of the prime Contractor to return such identification to the issuing agency in accordance with the terms set forth in paragraph (b) of this section, unless otherwise approved in writing by the Contracting Officer.

52.204-24 Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment
(AUG 2019)
(a) Definitions. As used in this provision—
“Covered telecommunications equipment or services,” “Critical technology,” and “Substantial or essential component” have the meanings provided in clause 52.204-25, Prohibition on Contracting for Certain
Telecommunications and Video Surveillance Services or Equipment.
(b) Prohibition. Section 889(a)(1)(A) of the John S. McCain National Defense Authorization Act for Fiscal Year
2019 (Pub. L. 115-232) prohibits the head of an executive agency on or after August 13, 2019, from procuring or obtaining, or extending or renewing a contract to procure or obtain, any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. Contractors are not prohibited from providing—
(1) A service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or
(2) Telecommunications equipment that cannot route or redirect user data traffic or permit visibility into any user data or packets that such equipment transmits or otherwise handles.
(c) Representation. The Offeror represents that—
It ___ will, ____ will not provide covered telecommunications equipment or services to the Government in the performance of any contract, subcontract or other contractual instrument resulting from this solicitation.
(d) Disclosures. If the Offeror has responded affirmatively to the representation in paragraph (c) of this provision, the Offeror shall provide the following information as part of the offer
(1) All covered telecommunications equipment and services offered (include brand; model number, such as original equipment manufacturer (OEM) number, manufacturer part number, or wholesaler number; and item description, as applicable);
(2) Explanation of the proposed use of covered telecommunications equipment and services and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b) of this provision;
(3) For services, the entity providing the covered telecommunications services (include entity name, unique entity identifier, and Commercial and Government Entity (CAGE) code, if known); and
(4) For equipment, the entity that produced the covered telecommunications equipment (include entity name, unique entity identifier, CAGE code, and whether the entity was the OEM or a distributor, if known).

(a) Definitions. As used in this provision, “covered telecommunications equipment or services” has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video
Surveillance Services or Equipment.
(b) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management (SAM)
(https://www.sam.gov) for entities excluded from receiving federal awards for “covered telecommunications equipment or services.”
(c) Representation. The Offeror represents that it _____ does ______ does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument.
52.209-11 REPRESENTATION BY CORPORATIONS REGARDING DELINQUENT TAX LIABILITY OR A
FELONY CONVICTION UNDER ANY FEDERAL LAW (FEB 2016)
(a) As required by sections 744 and 745 of Division E of the Consolidated and Further Continuing Appropriations
Act, 2015 (Pub. L. 113-235), and similar provisions, if contained in subsequent appropriations acts, the Government will not enter into a contract with any corporation that--
(1) Has any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability, where the awarding agency is aware of the unpaid tax liability, unless an agency has considered suspension or debarment of the corporation and made a determination that suspension or debarment is not necessary to protect the interests of the Government; or
(2) Was convicted of a felony criminal violation under any Federal law within the preceding 24 months, where the awarding agency is aware of the conviction, unless an agency has considered suspension or debarment of the corporation and made a determination that this action is not necessary to protect the interests of the Government.
(b) The Offeror represents that--

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a Firm Fixed Price purchase order resulting from this solicitation to the responsible vendor whose quote conforming to the solicitation will be most advantageous to the https://www.sam.gov/
Government, price and other factors considered. The following factors in order of relative importance shall be used to evaluate offers:
(i) Technical; EXCEPTIONS: All vendors MUST note any exception from the solicitation’s specifications, terms and conditions, clauses and provisions and/or performance work statement, if any. It will be assumed that any quote without mention of any exceptions will take no exceptions.
(ii) Price (inclusive of transportation charges); Single Award evaluation per the paragraph entitled
"Single Award for All Items";
(iii) Delivery; Ability to meet or exceed the stated delivery date or period of performance
(iv) Past Performance - Evaluation of past performance will be based on information obtained from
PPIRS-SR as well as any other sources which may have relevant information. A quote/offer with a low PPIRS-SR rating (red or yellow for quality and/or less than a 70 weighted delivery score) may not represent the most advantageous quote/offer to the Government.
b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) Award:
1) Award will be made to the quote/offer that represents the best value to the Government.
2) The Government intends to award a Purchase Order as a result of this solicitation. Written acceptance by the contractor is not required.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
