STATEMENT OF WORK
1.0 BACKGROUND
1.1 Per federal Occupational Safety and Health Administration (OSHA) regulations (29 CFR 1910.136), NAVSUP Fleet Logistics Center Puget Sound (FLCPS) is required to provide safety footwear to its civilian personnel engaged in foot-hazardous duties.
1.2 This Performance Work Statement defines the process for FLCPS civilian personnel to obtain safety shoes or boots, and for contractor invoicing and payment.
2.0 OBJECTIVE
2.1 General requirements. 29 CFR 1910.136 requires that employers ensure that their employees wear protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, objects piercing the sole, or electrical hazard, such as a static-discharge or electric-shock hazard.
2.2 Safety Footwear Requirements: Safety footwear must meet American Society of Testing Material (ASTM)
F2412-05 standards for foot protection and F2413-05 specifications for performance requirements.
3.0 SCOPE This contract is for provision of safety footwear to civilian employees of FLCPS, and can only be exercised through use of a NAVSUP FLCPS Safety Shoe Purchase Form.
3.1 Civilian personnel will present the Safety Shoe Purchase Form as authorization to purchase and select safety shoes from the contractor's inventory (except as noted below for unusual styles or sizes).
3.2 The contractor will provide safety shoes to FLCPS civilian employees up to $200.00 the dollar limit listed on the Safety Shoe Purchase Form. If the price exceeds the purchase limit on the form, the contractor will collect any funds exceeding the limit directly from the employee at the time of purchase.
3.3 Contractor will have employee sign and date the Safety Shoe Purchase Form.
4.0 Contractor Inventory. The contractor will stock several shoe/boot styles, colors, in a normal range of sizes, so employees can try on shoes and decide if they meet their needs. The contractor will carry sufficient stock to ensure that most employees can purchase shoes and wear them immediately. If the desired shoes/sizes are not in stock, the contractor will obtain the required items within five calendar days, notify the employee when they are available for pick-up, and include the sale on an invoice only after the sale is finalized.
5.0 Place of Performance. The contractor will maintain a public business establishment located in Kitsap
County, Washington or Bellingham, Washington to allow easy access for personnel who work in NAVSUP FLCPS locations in Bremerton, Bangor, Keyport, Whidbey Island, Everett, and Manchester, Washington.
6.0 Period of Performance: Basic Year, plus four option years, as listed below. NAVSUP FLCPS is under no obligation to exercise the options.
Basic Period of Performance: 1 July 2018 – 30 June 2019
Option One: 1 July 2019 – 30 June 2020
Option Two: 1 July 2020 – 30 June 2021
Option Three: 1 July 2021 – 30 June 2022
Option Four: 1 July 2022 - 30 June 2023
