Performance Work Statement

LINEN AND UNIFORM SERVICES

NUTRITION MANAGEMENT DEPARTMENT

NAVAL MEDICAL CENTER SAN DIEGO

1.0 BACKGROUND:

1.1 General: The purpose of this contract is to provide continual laundry services needed at the Naval Medical Center's Nutrition Management Department. The contractor shall furnish all management, labor, transportation, pickup and delivery services in accordance with the term and conditions of the Performance Work Statement (PWS).

2.0 SCOPE:

2.1 Scope of Work: The contractor will pick up soiled linen and perform laundry services which include but are not limited to wash, sort, fold wrap in plastic and deliver to Nutrition Management Department at the Naval Medical Center San Diego.

3.0 SPECIFIC DUTIES:

3.1 Pick-up/Delivery: Transportation of clean/soiled linen will be in carts provided by the contractor. These carts must be clean and maintained in a safe condition. The contractor shall have adequate carts on hand to transport linens and shall provide one

(1) cart on site at the Nutrition Management Department for use at all times. Pick-up of all items is required every Tuesday between 0700 and 1000. Drop-off of all items is required every Thursday between 0700 and 1000.

3.2 Laundering Requirement: The contractor is required to adhere to folding and quantity packaging of laundered items. Plastic wrapping should be of a reasonable thickness and quality to prevent heat softening or melting, which causes unsafe handling. Laundered materials will be sorted by like item and color. Each delivery of clean linen will contain an itemized list of its contents and the net weigh and/or quantity. These lists will be used to verify invoices for payment and to advise the government of items received as an on-going inventory of the available items.

3.2.1 Item Description:

1. Chef Smock: Launder according to washing instructions on product.

Stains shall be treated to the best of the contractor, using a stain remover agent. Two creases on front and back smock, 8 inches from each side seam. Medium starch and no folding. Use hanger.

2. Chef Pants: Launder according to washing instructions on product.

Stains shall be treated to the best abilities of the contractor, using a stain remover agent. Hot press, light starch, no folding. Use hanger.

3. Aprons: Launder according to washing instructions on product. Stain shall be treated to the best abilities of the contractor, using a stain remover agent. Hot press and light starch. Package no more than five

(5) each, together with plastic.

4. Chef Hats: Launder according to washing instructions on product.

Stain shall be treated to the best abilities of the contractor, using a stain remover agent. Hot press and light starch. Package no more than ten

(10) each, together with plastic.

5. Tablecloths: Launder according to washing instructions on product.

White table cloths may be bleached for harsher stains. All other colors shall be treated for stain removal by the best abilities of the contractor, using a stain remover agent. Hot press and medium starch. Minimum folding to avoid creases. Hang on large hanger.

6. Cloth Na pk.ins: Launder according to washing instructions on product.

White cloth napkins may be bleached for harsher stains. All other colors shall be treated for stain removal by the best abilities of the contractor, using a stain remover agent. Hot press and light starch. Fold in half.

Package no more than thirty (30) each, together with plastic.

7. Cleaning Rags: Launder according to washing instructions on product.

Fold in half. Package no more than one hundred (100) each, together with plastic.

3.3 Unserviceable Articles: Worn, tom and excessively stained linen items shall be removed from the service by the contractor. These items shall be returned to the government laundered, folded and separated from all other linen. The contractor shall be paid for the laundered item. ·

3.4 Miscellaneous Items: Miscellaneous items received for laundering that are not listed in Item Description shall not be laundered and returned to the government. There shall be no charge or payment for such items.

3.5 Special Need or Requests: The government may, on occasion request that the contractor provide a special delivery of linen to meet immediate needs due to the shortages of items and /or emergencies that cannot wait until the regular scheduled delivery. Additionally, special handling oflinen items may be required or requested at times. Examples include but are not limited to the following: to provide "detergent free" linen packets, special handling and delivery of new item, etc. These request are minimal and /or infrequent, twelve (12) times or less per year.

4.0 QUALITY ASSURANCE:

4.1 Quality of Service: The services specified in this Performance Work Statement shall be performed in accordance with established principles and ethics of the contract personnel.

4.2 Contractor Quality Control: The contractor shall establish and maintain a Quality Control Program and/or plan to ensure the requirements of the contract are maintained on a continued basis.

4.3 Quantity Assurance Surveillance Plan (QASP)

5.1 Federal Holidays: contract services will not be required on federally established holidays that are on a Tuesday or Thursday. Pick-up and /or delivery will be moved to the next business day.

6.0 PERSONNEL:

6.1 Contractor Representative: the contractor shall designate a representative for contract · issues. The name of the person shall be designated in writing and provided to the contracting office. The contractor representative shall have authority for the contractor on all contract matters relating to daily operation of this contract.

6.2 Employees: Contractor personnel shall maintain a neat appearance and be easily recognizable. This may be accomplished by wearing distinctive clothing bearing the name of the company or by wearing appropriate badge which contains the company name.
