STATEMENT OF WORK

STATEMENT OF WORK: Remove and Dispose of Unbound Sand

Scope:

NUWC-Keyport has a system that makes sand casting molds directly from Computer Aided

Design (CAD) files. Each time the system is run, a certain amount of waste sand is produced.

The sand is mixed with a small amount of activator. Mixed in with the sand is waste garnet from a waterjet cutter at Keyport. NUWC-Keyport requires pick up and proper disposal of this material from NUWC-Keyport.

Objective:

This task is for a contractor to come to Keyport, pick up waste sand and garnet and dispose of the waste properly.

Statement of Work Tasks:

The purpose of this Statement of Work (SOW) is to define all tasks required to support disposal and/or recycling of unbound sand mixed with activator. The mix ratio is approximately 99.8% sand and .2% activator, giving the sand a pH of 2.4. The waste garnet is mostly garnet with some metals filings. Both materials have been tested and are not classified as hazardous waste.

The primary objectives of the work performed under this SOW are the following:

1.0 Task 1: Pick Up Waste

Contractor to drop off 6 cubic yard capacity front load dumpster, with a minimum weight capacity of 4 tons (8,000 lbs.), placed at the north end of Bldg. 1. Dumpster is to have five solid sides that form a container to minimize leakage from dumpster. Contractor to provide a minimum of a 2 hour warning prior to dropping off container so Keyport can make sure area for container is clear or make arrangements to meet contractor to show contractor alternate location a short distance from usual location. Once the container is full, Keyport will contact contractor & contractor will empty container within three working days. Estimated number of service calls is one per month.

2.0 Task 2: Disposal and/or recycling of material

Contractor shall dispose of and/or recycle waste properly.

This is not-to-exceed (NTE) Purchase Order with Firm Fixed Pricing. Do not perform services beyond the NTE amount without prior approval from the Contracting Officer.

52.204-17 OWNERSHIP OF CONTROL OF OFFEROR (NOV 2014)

(a) Definitions. As used in this provision--

Commercial and Government Entity (CAGE) code means—

(1) An identifier assigned to entities located in the United States or its outlying areas by the Defense Logistics

Agency (DLA) Contractor and Government Entity (CAGE) Branch to identify a commercial or government entity, or

(2) An identifier assigned by a member of the North Atlantic Treaty Organization (NATO) or by the NATO Support

Agency (NSPA) to entities located outside the United States and its outlying areas that the DLA Contractor and

Government Entity (CAGE) Branch records and maintains in the CAGE master file. This type of code is known as an NCAGE code.

52.209-11 REPRESENTATION BY CORPORATIONS REGARDING DELINQUENT TAX LIABILITY OR A

(a) As required by sections 744 and 745 of Division E of the Consolidated and Further Continuing Appropriations

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph

Place of manufacture means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.

(b) For statistical purposes only, the offeror shall indicate whether the place of manufacture of the end products it expects to provide in response to this solicitation is predominantly--

CONFIDENTIALITY AGREEMENTS—REPRESENTATION (DEVIATION 2016-O0003)(OCT 2015)

(a) In accordance with section 101(a) of the Continuing Appropriations Act, 2016 (Pub. L. 114-53) and any subsequent FY 2016 appropriations act that extends to FY 2016 funds the same restrictions as are contained in section 743 of division E, title VII, of the Consolidated and Further Continuing Appropriations Act, 2015 (Pub. L.

113-235), none of the funds appropriated (or otherwise made available) by this or any other Act may be used for a contract with an entity that requires employees or subcontractors of such entity seeking to report fraud, waste, or abuse to sign internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contactors from lawfully reporting such waste, fraud, or abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information.

(b) The prohibition in paragraph (a) of this provision does not contravene requirements applicable to Standard Form

312, Form 4414, or any other form issued by a Federal department or agency governing the nondisclosure of classified information.

(c) Representation. By submission of its offer, the Offeror represents that it does not require employees or subcontractors of such entity seeking to report fraud, waste, or abuse to sign or comply with internal confidentiality agreements or statements prohibiting or otherwise restricting such employees or contactors from lawfully reporting such waste, fraud, or abuse to a designated investigative or law enforcement representative of a Federal department or agency authorized to receive such information.
