CHAPLAIN RELIGIOUS ENRICHMENT DEVELOPMENT OPERATION (CREDO) PROGRAM
CREDO FACILITATOR
1) Background:
a) CREDO is a Secretary of the Navy Flagship program which has a 45 year history of proactively delivering specialized pastoral/spiritual care to address issues affecting readiness and resilience.
b) The facilitation of professional pastoral care services required by this Performance Work Statement (PWS) is similar to Department of the Navy and other Department of Defense clergy support services with the exception that the focus of effort is on facilitation, testing, and evaluation of CREDO programs. The CREDO facilitator support services and related manpower are needed to expand the access to CREDO pastoral care programs which sustain the fleet, enable the war fighter, and support their families.
2) Scope:
a) The emphasis of this requirement is on the development, implementation, execution and evaluation of CREDO workshops and retreats. This requirement will involve approximately 70% workshop leadership, 20% retreat facilitation, and 10% other activities.
b) The contractor shall be responsible for ensuring the accuracy, timeliness, and completion of retreats, workshops, assessments, and reports in support of the CREDO program as required in PWS section 4 - Deliverables.
c) This is not a Chaplain position; however, the contractor shall be a credentialed subject matter expert with the knowledge, skills and abilities of seminary-trained professional clergy. Two years of professional experience leading CREDO programs is required. Since participants may suffer from varying degrees of operational stress, post-traumatic stress, battle fatigue, and other forms of visible and invisible wounds; this level of expertise and experience is critical to creating a safe emotional space for CREDO participants in varied CREDO program formats such as a weekend retreat away from base in civilian clothes or a workshop on base. This position requires extensive familiarity with CREDO, its programs and mission.
d) The contractor shall assist NDW Religious Programs Office as a professional facilitator and provide support services to the NDW CREDO staff for command administrative consulting support to meet its mission of strengthening Fleet, Fighter, and Family. This effort includes:
i) Identifying and defining additional capabilities and initiatives that can be delivered by CREDO under the four Chaplain Corps professional capabilities: Provide, Facilitate, Care, and Advise.
ii) Making appropriate referrals to a chaplain or other helping professionals within the Navy system when trauma and traumatic loss needs arise, especially with wounded, ill and injured service members and their families.
iii) Using specific reporting means to capture measures of performance and measures of effectiveness regarding the impact of CREDO programs in order for Echelon I, II, III and IV commands and other stakeholders to have ready access to professional business processes, data and trend analysis.
iv) Promoting knowledge sharing between other caring professions within the Navy system through collaborative ventures that increase professional cross-pollination in prevention and intervention pastoral care services for Sailors and their families.
v) Providing timely and accurate promotional materials, flyers and advertisements, to promote workshops and events and distribute through Navy media and electronic format including Facebook.
e) Contractor shall maintain professional certifications, credentials and ethical standards required in the field of pastoral care, counseling, and/or social work, and immediately report moral breaches to the Government Technical Point of Contact (TPOC) and/or the CREDO Director.
f) Contractor shall comply with station and government policies and regulations regarding identification badges, parking, professional decorum, and Information Technology Information Assurance.
g) Contractor must be flexible in regard to scheduling and be available to deliver training during early morning, daytime, evening, and weekend hours. Work schedules will vary from week to week depending on the availability of military personnel to participate in CREDO Workshops and CREDO Retreats.
h) Except where specified in PWS section 4(m), all cost associated with additional contractor training including tuition, fees, lodging and travel are born by the contractor.
3) Qualifications:
Minimum qualifications, experience and skills required of contractor's employee facilitators that are necessary to adequately perform the stated requirements:
a) Two years of full-time professional experience leading CREDO programs is required.
b) Masters of Divinity degree or a Master’s Degree with a minimum of 60 units in a related field that is accredited by Association of Theological Schools. A social worker must be licensed and certified by a state to independently practice social work at the master’s degree level. A mental health counselor or professional counselor must be certified by a governing body in their field.
c) Must have completed a minimum of one (1) unit of Clinical Pastoral Education (CPE) or equivalent clinical training experience; or successfully completed clinical program(s) accredited by the Association for Clinical Pastoral Education (ACPE), the College of Pastoral Supervision and Psychotherapy (CPSP), or the National Association of Catholic Chaplains.
d) Must have a minimum of five (5) verifiable years of full-time pastoral experience in an ecclesiastical setting, a hospital, bureau of prisons, or college campus, or equivalent professional experience.
e) Must have verification of completed Myers-Briggs Type Indicator (MBTI) practitioner certification and provide evidence of successfully delivering MBTI to a military environment.
f) Must have received Applied Suicide Intervention Skills Training – Training for Trainers (ASIST T4T) and safeTALK – Training for Trainers (safeTALK T4T). Both trainings are offered by the company LivingWorks.
Additionally, candidate must be a current and active ASIST and safeTALK trainer registered with LivingWorks.
Otherwise, if selected, candidate will complete both ASIST T4T and safeTALK T4T within 90 days of acceptance of contract at the contractor’s expense (expenses include travel, lodging, tuition and misc. associated with ASIST T4T and safeTALK T4T trainings).
g) Candidate must be certified in facilitating The Prevention and Relationship Education Program (PREP) by PREP, Inc. Otherwise, if selected, candidate will complete PREP facilitator certification within 90 days of acceptance of contract at the contractor’s expense (expenses include travel, lodging, tuition and misc. associated with PREP facilitator training).
h) Must have five (5) years of military service (received no less than an honorable discharge or administrative discharge for medical reasons), or equivalent experience as a military family member, or equivalent experience as a civilian in the employment of DoD, who would be familiar with the experiences of life of service personnel.
(DD214 verification required of former military personnel.)
i) Must have demonstrated capability to work in multi-disciplinary team environment, with health care providers, community educators, social workers and sexual assault victim advocates.
j) Must have verifiable curriculum development and implementation experience in a military environment utilizing an evidence-based curriculum devolvement model.
k) Must have current security accessibility to all military installations in the National Capital area.
4) Deliverables:
a) Contractor will facilitate CREDO Workshops. CREDO Workshops may be held anywhere within a 60-mile radius of the Washington Navy Yard in order to serve tenant and/or installation commanders. Quantities are total per contract and not per base location. Commanders may direct military personnel in their commands to attend these workshops in support of command objectives and may also invite one of the Team Building, Marriage, or Suicide Prevention workshops to be command-specific. Curricula are standardized and will be provided by NDW CREDO.
Examples are MBTI workshop, PREP, safeTALK, and ASIST.
b) Other additional workshops may be developed at the discretion of the NDW CREDO director to meet the ever-changing needs of service personnel and commands. Contractor will contribute ideas and professional expertise in the development of additional workshops.
c) Contractor will facilitate CREDO Retreats. CREDO Retreats are typically scheduled from Friday afternoons through Sunday mornings, but may be scheduled on different days of the week depending on the needs of the Navy and supported commands. CREDO retreats may be held anywhere within a 60 mile radius of the Washington Navy Yard. Quantities are total per contract and not per base location.
Curricula are standardized and will be provided by NDW CREDO.
d) Retreats and workshops are pre-scheduled throughout the year. Workshops during the week vary from 2 to 8 hours per day, not including preparation time. Contractor must be flexible in regard to scheduling and be available to deliver training during early morning, daytime, evening, and weekend hours. Work schedules will vary from week to week depending on the availability of military personnel to participate in CREDO Workshops and CREDO Retreats. Any changes in scheduling will have a 7 day notification.
e) Contractor will assist in advertising CREDO programs by staffing information tables, delivering information briefings about CREDO, and distributing flyers and other promotional materials. Contractor may be photographed or video-recorded while conducting CREDO events. Photographs or video-recordings will be used to advertise CREDO programs.
f) Contractor will communicate with retreat and workshop participants by email and telephone before and after certain events (as directed) for pre-event check-ins, post-event follow-ups, recruiting, and other related activities.
g) Contactor will facilitate workshops virtually via live and pre-recorded methods when directed by the NDW CREDO Director. This may include audio and video or audio only. Training may be delivered over the telephone or other electronic methods approved by the NDW CREDO Director.
h) In the performance of their contracted hours and with the NDW CREDO Director’s approval, the contractor may produce (or participate in the production of) printed, photographed, and video recorded advertisements and inspirational messages consistent with the mission of NDW CREDO and the Navy Chaplain Corps. Any such products will be the exclusive property of NDW CREDO and the Navy.
i) In the performance of their contracted hours and with the NDW CREDO Director’s approval, the contractor may develop (or participate in the development of) new content and curriculum for future CREDO Retreats and CREDO Workshops. Any scripts, lesson plans, recordings, audio and visual aids, or other work-product generated will be the exclusive property of NDW CREDO and the Navy.
j) In order to conduct CREDO activities, the contractor will be required to physically arrange training spaces;
transport, set-up, and takedown presentation equipment; transport promotional and training materials; and engage in other logistical tasks. Some training sites will have support staff that will assist with these tasks, but some will not.
k) Contractor will provide their own transportation at the contractor’s own expense to the various sites where the contractor is assigned to provide retreats, workshops, information tables, or perform any other CREDO activity.
l) Program Assessments submitted by the contractor as instructed by CREDO staff will include timely and accurate reports for each retreat, workshop or event; executive summaries, memos, briefs, meeting notes, and after-action reports; and participant feedback forms.
m) The NDW CREDO director may require contractor to receive training for additional curricula not already specified in this contract. When this is the case, training attendance will count toward the performance of the contractor’s work hours. Additionally, the costs of the training (registration and curriculum) will be paid by NDW CREDO. For locally conducted training, contractor will provide their own transportation at the contractor’s own expense.
5) Hours of Operations:
a) Work hours for this contractor shall be a minimum of 35 hours per week and not to exceed 40 hours per week for 48 weeks per year. Daily travel time to the contractor’s initial worksite and from the contractor’s final worksite of the day (whether it is the administrative offices for NDW CREDO, a CREDO Retreat site, a CREDO Workshop site, or other designated site within the NCR) is not counted towards the minimum 35 weekly work hours. Work schedules will vary from week to week depending on scheduled programing.
b) Four weeks of no work per contract year shall be approved by the Regional CREDO Director.
c) Contractor is expected to attend coordination meetings during regular office hours in addition to delivering programming.
d) There will be no work performed nor meetings held during Federal holidays as promulgated by the Installation Commander and the Office of Personnel Management (OPM).
6) Places of Performance:
a) CREDO Workshops and Retreats may be held anywhere within a 60-mile radius of the Washington Navy Yard.
b) Settings may include retreat facilities, offices, classrooms, recreational facilities, chapels, or other appropriate settings within the region.
7) Security Requirements:
Unclassified. Contractor must comply with the following directive:
Contractor Unclassified Access to Federally Controlled Facilities, Sensitive Information, Information Technology (IT) Systems or Protected Health Information (April 2017)
Homeland Security Presidential Directive (HSPD)-12, requires government agencies to develop and implement Federal security standards for Federal employees and contractors. The Deputy Secretary of Defense Directive-Type Memorandum (DTM) 08-006 – “DoD Implementation of Homeland Security Presidential Directive – 12 (HSPD- 12)” dated November 26, 2008 (or its subsequent DoD instruction) directs implementation of HSPD-12. This clause is in accordance with HSPD-12 and its implementing directives.
APPLICABILITY
This text applies to contractor employees requiring physical access to any area of a federally controlled base, facility or activity and/or requiring access to a DoN or DoD computer/network/system to perform certain unclassified sensitive duties. This clause also applies to contractor employees who access Privacy Act and Protected Health Information, provide support associated with fiduciary duties, or perform duties that have been identified as National Security Position, as advised by the command security manager. It is the responsibility of the responsible security officer of the command/facility where the work is performed to ensure compliance.
Each contractor employee providing services at a Navy Command under this contract is required to obtain a Department of Defense Common Access Card (DoD CAC). Additionally, depending on the level of computer/network access, the contract employee will require a successful investigation as detailed below.
ACCESS TO FEDERAL FACILITIES
Per HSPD-12 and implementing guidance, all contractor employees working at a federally controlled base, facility or activity under this clause will require a DoD CAC. When access to a base, facility or activity is required contractor employees shall in-process with the Command’s Security Manager upon arrival to the Command and shall out-process prior to their departure at the completion of the individual’s performance under the contract.
ACCESS TO DOD IT SYSTEMS
In accordance with SECNAV M-5510.30, contractor employees who require access to DoN or DoD networks are categorized as IT-I, IT-II, or IT-III. The IT-II level, defined in detail in SECNAV M-5510.30, includes positions which require access to information protected under the Privacy Act, to include Protected Health Information (PHI).
All contractor employees under this contract who require access to Privacy Act protected information are therefore categorized no lower than IT-II. IT Levels are determined by the requiring activity’s Command Information Assurance Manager.
Contractor employees requiring privileged or IT-I level access, (when specified by the terms of the contract) require a Single Scope Background Investigation (SSBI) or T5 or T5R equivalent investigation , which is a higher level investigation than the National Agency Check with Law and Credit (NACLC)/T3/T3R described below. Due to the privileged system access, an investigation suitable for High Risk national security positions is required. Individuals who have access to system control, monitoring, or administration functions (e.g. system administrator, database administrator) require training and certification to Information Assurance Technical Level 1, and must be trained and certified on the Operating System or Computing Environment they are required to maintain.
Access to sensitive IT systems is contingent upon a favorably adjudicated background investigation. When access to IT systems is required for performance of the contractor employee’s duties, such employees shall in-process with the Navy Command’s Security Manager and Information Assurance Manager upon arrival to the Navy command and shall out-process prior to their departure at the completion of the individual’s performance under the contract.
Completion and approval of a System Authorization Access Request Navy (SAAR-N) form is required for all individuals accessing Navy Information Technology resources. The decision to authorize access to a government IT system/network is inherently governmental. The contractor supervisor is not authorized to sign the SAAR-N;
therefore, the government employee with knowledge of the system/network access required or the COR shall sign the SAAR-N as the “supervisor”.
The SAAR-N shall be forwarded to the Command’s Security Manager at least 30 days prior to the individual’s start date. Failure to provide the required documentation at least 30 days prior to the individual’s start date may result in delaying the individual’s start date. When required to maintain access to required IT systems or networks, the contractor shall ensure that all employees requiring access complete annual Information Assurance (IA) training, and maintain a current requisite background investigation. The Contractor’s Security Representative shall contact the Command Security Manager for guidance when reinvestigations are required.
INTERIM ACCESS
The Command's Security Manager may authorize issuance of a DoD CAC and interim access to a DoN or DoD unclassified computer/network upon a favorable review of the investigative questionnaire and advance favorable fingerprint results. When the results of the investigation are received and a favorable determination is not made, the contractor employee working on the contract under interim access will be denied access to the computer network and this denial will not relieve the contractor of his/her responsibility to perform.
DENIAL OR TERMINATION OF ACCESS
The potential consequences of any requirement under this clause including denial or termination of physical or system access in no way relieves the contractor from the requirement to execute performance under the contract within the timeframes specified in the contract. Contractors shall plan ahead in processing their employees and subcontractor employees. The contractor shall insert this clause in all subcontracts when the subcontractor is permitted to have unclassified access to a federally controlled facility, federally-controlled information system/network and/or to government information, meaning information not authorized for public release.
CONTRACTOR’S SECURITY REPRESENTATIVE
The contractor shall designate an employee to serve as the Contractor’s Security Representative. Within three work days after contract award, the contractor shall provide to the requiring activity’s Security Manager and the Contracting Officer, in writing, the name, title, address and phone number for the Contractor’s Security Representative. The Contractor’s Security Representative shall be the primary point of contact on any security matter. The Contractor’s Security Representative shall not be replaced or removed without prior notice to the Contracting Officer and Command Security Manager.

Navy security policy requires that all positions be given a sensitivity value based on level of risk factors to ensure appropriate protective measures are applied. Contractor employees under this contract are recognized as Non- Critical Sensitive [ADP/IT-II] positions when the contract scope of work require physical access to a federally controlled base, facility or activity and/or requiring access to a DoD computer/network, to perform unclassified sensitive duties. This designation is also applied to contractor employees who access Privacy Act and Protected Health Information (PHI), provide support associated with fiduciary duties, or perform duties that have been identified as National Security Positions. At a minimum, each contractor employee must be a US citizen and have a favorably completed NACLC or T3 or T3R equivalent investigation to obtain a favorable determination for assignment to a non-critical sensitive or IT-II position. The investigation consists of a standard NAC and a FBI fingerprint check plus law enforcement checks and credit check. Each contractor employee filling a non-critical sensitive or IT-II position is required to complete:
SF-86 Questionnaire for National Security Positions (or equivalent OPM investigative product) Two FD-258 Applicant Fingerprint Cards (or an electronic fingerprint submission) Original Signed Release Statements
Failure to provide the required documentation at least 30 days prior to the individual’s start date shall result in delaying the individual’s start date. Background investigations shall be reinitiated as required to ensure investigations remain current (not older than 10 years) throughout the contract performance period. The Contractor’s Security Representative shall contact the Command Security Manager for guidance when reinvestigations are required.
Regardless of their duties or IT access requirements ALL contractor employees shall in-process with the Command’s Security Manager upon arrival to the command and shall out-process prior to their departure at the completion of the individual’s performance under the contract. Employees requiring IT access shall also check-in and check-out with the Navy Command’s Information Assurance Manager. Completion and approval of a System
Authorization Access Request Navy (SAAR-N) form is required for all individuals accessing Navy Information Technology resources. The SAAR-N shall be forwarded to the Navy Command’s Security Manager at least 30 days prior to the individual’s start date. Failure to provide the required documentation at least 30 days prior to the individual’s start date shall result in delaying the individual’s start date.
The contractor shall ensure that each contract employee requiring access to IT systems or networks complete annual Information Assurance (IA) training, and maintain a current requisite background investigation. Contractor employees shall accurately complete the required investigative forms prior to submission to the Command Security Manager. The Command’s Security Manager will review the submitted documentation for completeness prior to submitting it to the Office of Personnel Management (OPM); Potential suitability or security issues identified may render the contractor employee ineligible for the assignment. An unfavorable determination is final (subject to SF- 86 appeal procedures) and such a determination does not relieve the contractor from meeting any contractual obligation under the contract. The Command’s Security Manager will forward the required forms to OPM for processing. Once the investigation is complete, the results will be forwarded by OPM to the DoD Central Adjudication Facility (CAF) for a determination.
If the contractor employee already possesses a current favorably adjudicated investigation, the contractor shall submit a Visit Authorization Request (VAR) via the Joint Personnel Adjudication System (JPAS) or a hard copy VAR directly from the contractor’s Security Representative. Although the contractor will take JPAS “Owning” role over the contractor employee, the Command will take JPAS "Servicing" role over the contractor employee during the hiring process and for the duration of assignment under that contract. The contractor shall include the IT Position Category per SECNAV M-5510.30 for each employee designated on a VAR. The VAR requires annual renewal for the duration of the employee’s performance under the contract.
BACKGROUND INVESTIGATION REQUIREMENTS AND SECURITY APPROVAL PROCESS FOR
CONTRACTORS ASSIGNED TO OR PERFORMING NON-SENSITIVE DUTIES
Contractor employee whose work is unclassified and non-sensitive (e.g., performing certain duties such as lawn maintenance, vendor services, etc. ...) and who require physical access to publicly accessible areas to perform those duties shall meet the following minimum requirements:
Must be either a US citizen or a US permanent resident with a minimum of 3 years legal residency in the United States (as required by The Deputy Secretary of Defense DTM 08-006 or its subsequent DoD instruction) and Must have a favorably completed National Agency Check with Written Inquiries (NACI) or T1 investigation equivalent including a FBI fingerprint check prior to installation access.
To be considered for a favorable trustworthiness determination, the Contractor’s Security Representative must submit for all employees each of the following:
SF-85 Questionnaire for Non-Sensitive Positions Two FD-258 Applicant Fingerprint Cards (or an electronic fingerprint submission) Original Signed Release Statements The contractor shall ensure each individual employee has a current favorably completed National Agency Check with Written Inquiries (NACI) or T1 equivalent investigation, or ensure successful FBI fingerprint results have been gained and investigation has been processed with OPM

* Consult with your Command Security Manager and Information Assurance Manager for local policy when IT-III (non-sensitive) access is required for non-US citizens outside the United States.
8) Government Furnished Information:
a) The Government Furnished Information (GFI) shall include relevant proprietary material produced by various organizations in support of analytical and consulting services to discern key core competencies onhand/required for executing identified CREDO core functions. CREDO retreat lesson plans and modules will be provided to the contractor.
b) The deliverables and program assessments are property of CNIC: All raw data as well as resulting tables, matrixes, reports, manuals, audio/visual aids and other such reports generated shall become the property of CNIC Force Chaplain. Products to be in report, print, presentation, or other format as approved by CNIC Force Chaplain or designated representative.
9) Government Furnished Property, Material, and Services:
The Government will furnish necessary workspace to include desk space, telephones, and computers.
10) Inspection and Acceptance The contractor shall review all assigned tasks with the Government TPOC and Contract Subject Matter Expert designate, prior to commencing the work and on a quarterly basis thereafter. Inspection and acceptance of services to be furnished hereunder shall be made, upon completion of the services, by the Government TPOC or designate.

The contractor shall report contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Navy via a secure data collection site. (2) X, Lease/Rental of Facilities;
(3) Y, Construction of Structures and Facilities;

52.204-24 REPRESENTATION REGARDING CERTAIN TELECOMMUNICATIONS AND VIDEO
SURVEILLANCE SERVICES OR EQUIPMENT (AUG 2020)
The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it "does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument" in the provision at 52.204-26, Covered Telecommunications Equipment or Services--Representation, or in paragraph (v) of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items.
(a) Definitions. As used in this provision-
Backhaul, covered telecommunications equipment or services, critical technology, interconnection arrangements, reasonable inquiry, roaming, and substantial or essential component have the meanings provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.
(b) Prohibition.
(1) Section 889(a)(1)(A) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019 (Pub. L.
115-232) prohibits the head of an executive agency on or after August 13, 2019, from procuring or obtaining, or extending or renewing a contract to procure or obtain, any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. Nothing in the prohibition shall be construed to--
(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or
(ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles.
(2) Section 889(a)(1)(B) of the John S. McCain National Defense Authorization Act for Fiscal Year 2019 (Pub. L.
115-232) prohibits the head of an executive agency on or after August 13, 2020, from entering into a contract or extending or renewing a contract with an entity that uses any equipment, system, or service that uses covered telecommunications equipment or services as a substantial or essential component of any system, or as critical technology as part of any system. This prohibition applies to the use of covered telecommunications equipment or services, regardless of whether that use is in performance of work under a Federal contract. Nothing in the prohibition shall be construed to--
(i) Prohibit the head of an executive agency from procuring with an entity to provide a service that connects to the facilities of a third-party, such as backhaul, roaming, or interconnection arrangements; or
(ii) Cover telecommunications equipment that cannot route or redirect user data traffic or cannot permit visibility into any user data or packets that such equipment transmits or otherwise handles.
(c) Procedures. The Offeror shall review the list of excluded parties in the System for Award Management (SAM) (https://www.sam.gov) for entities excluded from receiving federal awards for "covered telecommunications equipment or services."
(d) Representations. The Offeror represents that--
(1) It [ ] will, [ ] will not provide covered telecommunications equipment or services to the Government in the performance of any contract, subcontract or other contractual instrument resulting from this solicitation. The Offeror shall provide the additional disclosure information required at paragraph (e)(1) of this section if the Offeror responds "will" in paragraph (d)(1) of this section; and
(2) After conducting a reasonable inquiry, for purposes of this representation, the Offeror represents that--
It [ ] does, [ ] does not use covered telecommunications equipment or services, or use any equipment, system, or service that uses covered telecommunications equipment or services. The Offeror shall provide the additional disclosure information required at paragraph (e)(2) of this section if the Offeror responds "does" in paragraph (d)(2) of this section.
(e) Disclosures.
(1) Disclosure for the representation in paragraph (d)(1) of this provision. If the Offeror has responded "will" in the representation in paragraph (d)(1) of this provision, the Offeror shall provide the following information as part of the offer:
(i) For covered equipment--
(A) The entity that produced the covered telecommunications equipment (include entity name, unique entity identifier, CAGE code, and whether the entity was the original equipment manufacturer (OEM) or a distributor, if known);
(B) A description of all covered telecommunications equipment offered (include brand; model number, such as OEM number, manufacturer part number, or wholesaler number; and item description, as applicable); and
(C) Explanation of the proposed use of covered telecommunications equipment and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b)(1) of this provision.
(ii) For covered services--
(A) If the service is related to item maintenance: A description of all covered telecommunications services offered (include on the item being maintained: Brand; model number, such as OEM number, manufacturer part number, or wholesaler number; and item description, as applicable); or
(B) If not associated with maintenance, the Product Service Code (PSC) of the service being provided; and explanation of the proposed use of covered telecommunications services and any factors relevant to determining if such use would be permissible under the prohibition in paragraph (b)(1) of this provision.
(2) Disclosure for the representation in paragraph (d)(2) of this provision. If the Offeror has responded "does" in the representation in paragraph (d)(2) of this provision, the Offeror shall provide the following information as part of the offer:
(i) For covered equipment--

52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (DEVIATION 2018-O0018) (JUN 2020)
(a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code(s) and small business size standard(s) for this acquisition appear elsewhere in the solicitation. However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees.
(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show--
(1) The solicitation number;
(2) The time specified in the solicitation for receipt of offers;
(3) The name, address, and telephone number of the offeror;
(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;
(5) Terms of any express warranty;
(6) Price and any discount terms;
(7) "Remit to" address, if different than mailing address;
(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);
(9) Acknowledgment of Solicitation Amendments;
(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and
(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.
(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing.
(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions, including alternative line items (provided that the alternative line items are consistent with subpart 4.10 of the Federal Acquisition Regulation), or alternative commercial items for satisfying the requirements of this solicitation.
Each offer submitted will be evaluated separately.
(f) Late submissions, modifications, revisions, and withdrawals of offers:
(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.
(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and--
(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or
(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or
(C) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or…
