REQUEST FOR QUOTATION - FA442723Q0032
1. The 60th Contracting Squadron is soliciting quotations for the items listed below. This is a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This Combo constitutes the only solicitation. Additionally, the Government will utilize simplified procedures in accordance with FAR Part 13.

3. The solicitation document and incorporated provisions and clause are those in effect through Federal Acquisition Circular 2023-01 effective 30 Dec 2022, Defense Federal Acquisition Regulation effective 31 Jan 2023, and Air Force Acquisition Regulation Supplement effective 20 Dec 2022.
4. The contractor shall provide all equipment, personnel, transportation, materials, installation and other items and all other items necessary to provide the Blood Irradiator as defined in the Statement of Need. Reference Attachment 1 – Statement of Work.

5. FAR 52.212-1, Instructions to Offerors – Commercial Items (Jun 2020) applies to this acquisition and the following addendum applies:
Paragraph (a) first paragraph revised as follows: “The NAICS code and small business size standard for this acquisition appear above”.
Paragraph (b)(2) revised as follows:
· All questions regarding this acquisition are to be directed ONLY to the 60th Contracting Squadron personnel. Questions will be accepted until 12:00 pm Pacific Standard Time, Friday 3 March 2023. Questions shall be in writing and sent by email to gabriel.sanderson@us.af.mil. Offerors shall provide quotes no later than 12:00 pm PST, 10 March 2023 to the 60th Contracting Office via email to: gabriel.sanderson@us.af.mil.
· Offers received after the established response date/time will not be accepted
· If you do not receive an email confirming receipt/acknowledgment of your quote, please contact the POCs below to ensure your quote was delivered
· A site visit for potential venders will take place at 09:00AM PST, 1 March 2023. Meeting location will be outside the Travis AFB Visitor Center, Bldg. 599, Air Base Pkwy Travis AFB, CA 94535
· Please provide the following information to the specialist listed by 12:00PM PST, 24 February 2023:

Paragraph (b)(4) revised as follows: “Technical description of all items being offered: Limited to no more than five (5) pages. The contractor must demonstrate that the proposed equipment to include install meets all the minimum requirements in the attached Statement of Work”
Paragraph (b)(6) revised as follows: “The price and any discount terms for this acquisition should be populated in the table above (ref. Para 4).”
Paragraph (b)(9) revised as follows: “Acknowledgement of Solicitation Amendments: The contractor will be required to acknowledge any amendment(s) via email.”
Paragraph (b)(11): revised as follows: “Contractors must acknowledge in the table above specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation.”
Paragraph (c) Period for acceptance of offers: “The vendor agrees to hold the prices in its quote firm for 90 calendar days from the date specified for receipt of quotes, unless another time period is specified in an addendum to the quote.”
Paragraph (h) revised as follows: “The Government will not accept any offers or quantities other than the established items in the table found above and Statement of Work.”
6. FAR 12.602 contract award will be made Simplified Acquisition Procedures in accordance with FAR
13.106. Vendor’s quotes will be evaluated based upon the following:
Technical: All quotes must be found technically acceptable to be eligible for award. In order to be deemed technically acceptable, the following evaluation criteria must be met:
The quoted items must be in compliance with all parameters identified in the Statement of Work specifications.
Price: An offeror’s quoted prices will be determined by the unit price of the CLIN.
Delivery/Install: An offeror must provide delivery/install terms expressed in number of days in the table found above.

7. 52.225-18 – Place of Manufacture.
As prescribed in 25.1101(f), insert the following solicitation provision:
Place of Manufacture (Aug 2018)
(a) Definitions. As used in this provision:
“Manufactured end product” means any end product in product and service codes (PSC) 1000-9999, except—
(1) FSC 5510, Lumber and Related Basic Wood Materials;
(2) Product or Service Group (PSG) 87, Agricultural Supplies;
(3) PSG 88, Live Animals;
(4) PSG 89, Subsistence;
(5) PSC 9410, Crude Grades of Plant Materials;
(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;
(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;
(8) PSC 9610, Ores;
(9) PSC 9620, Minerals, Natural and Synthetic; and
(10) PSC 9630, Additive Metal Materials.
“Place of manufacture” means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.
(b) For statistical purposes only, the offeror shall indicate whether the place of manufacture of the end products it expects to provide in response to this solicitation is predominantly—
(1) [ ] In the United States (Check this box if the total anticipated price of offered end products manufactured in the United States exceeds the total anticipated price of offered end products manufactured outside the United States); or
(2) [ ] Outside the United States.
