MINIMUM TECHNICAL REQUIREMENTS

(MTRS) and Justification

METHOD OF EVALUATION
VENDOR’S PROVIDE PAGE # WHERE EACH MTR IS MET IN YOUR TECHNICAL LITERATURE
1
Characteristic: Must be wheelchair accessible Justification: A significant portion of VA patient population is elderly and/or disabled.
Literature Review
2
Characteristic: Must have a minimum of 3 charging wells for hand-held instruments Justification: Hand-held instruments needed are rechargeable battery powered and must be recharged within easy reach of the clinician. Recharging at a distance or other location will impede efficient clinic operations.
Literature Review
3
Characteristic: Must have light source that is a warm tone in color (not bright white). The light source must be an overhead, adjustable arm that can be maneuvered by the clinician to an optimal location for each exam Justification: The clinician must be able to see the eye clearly to make a proper examination. The color used is important as a warm tone makes it easier to read the patient's chart and other necessary materials. The light must be adjustable to accommodate a wide variety of patients and clinicians.
Literature Review
4
Characteristic: Instrument stand refractor arm must have a range of a minimum of 10". Justification: The clinician needs to be able to move the refractor into position in relation to the patient lined up with the vision checker. Any length less than 10" will inhibit proper alignment.
Literature Review
5
Characteristic: The instrument stand must have an integrated control panel with switches that electronically control multiple elements. At a minimum, the panel must be able to control the exam chair to allow for the proper positioning of the patient, the overhead light and the slit lamp arm. This panel must ensure easy, one-handed adjustment of arm positions to accommodate both ambulatory and wheelchair patients. Justification: Controls must be within easy reach of the clinician during the exam to maximize efficiency. The clinician needs to control arm movement with one hand as they will be holding instruments in the other.
Literature Review
6
Characteristic: Must biocompatible the slit lamp quoted in LIN 3. This means the stand can physically hold and connect to the slit lamp including any electrical connections, etc. Justification: The purpose of the stand is to hold the slit lamp. A stand that cannot hold, connect to and support the control of the slit lamp quoted in LIN 3 would be of no use.
Literature Review
7
Characteristic: Spare parts must be readily available for purchase to ensure proper maintenance and timely repair of the stand. Justification: Unavailability of, or delays in obtaining parts will render the stand inoperable and therefore render the exam room unusable for eye exams. This defeats the purpose of the room and would severely impact clinic efficiency and patient access to care.
Literature Review
8
Characteristic: it is preferred that the stand be capable of subsequent add-ons or modifications. For example, at a future date, we may add the capability to control the lights of the exam room itself. This enables the clinician to turn the room lights on or off without having to get up and move across the room. Justification: This preference is solely for clinic efficiency. Any time a clinician has to rise from their seated position at the controls and move across the room impacts efficiency. Efficiency impacts the number of exams that can be conducted in a day, and cumulatively over time. This impacts patient's timely access to care.

1
Characteristic: Must be able to move into any position necessary for the proper examination of the patient, including reclining to a near flat (horizontal) position. Justification: The clinician must be able to maneuver the patient into proper alignment for any given examination. This is also to ensure patients can be maneuvered by means of the powered chair without any physical exertion on the part of the patient or the clinician for safety reasons.
Literature Review
2
Characteristic: The chair must have multiple options for control of the chair including controls on the chair itself, on the control stand (as described in LIN 1), and a foot switch/pedal. The chair mounted switches must be available on both sides of the chair. The chair must be compatible with the instrument stand described in LIN 1. Justification: The clinician must be able to maneuver the patient effectively and efficiently by use of the chair from multiple positions based on the needs of the exam/patient. This includes being able to approach the patient and control the chair from either side, as well as controlling the chair while both hands are full.
Literature Review
3
Characteristic: Chair must have a minimum hydraulic rating of greater than 500 lbs. Justification: A significant portion of VA patient population is elderly overweight or obese and the chair must be able to accommodate and support a variety of patients including those more than 500 lbs.
Literature Review
4
Characteristic: The chair must be able to accommodate and safely support a variety of patients with varying needs to enter/exit the chair and be positioned comfortably during the exam. The chair must have adjustable head rests with at least two points of adjustment. It must have armrests and footrests that can move out of the way to enable patients to enter/exit the chair from either side directly to a walker or wheel chair. Justification: Patients of all sizes, shapes, and physical ability to sit up, etc. must be able to safely enter/exit the chair. The patient must be able to rest comfortably with proper support during the exam. Patients who are not properly supported may move which impacts the precision of the exam and may even be injured.
Literature Review
5
Characteristic: This chair must be compatible with the requirements of the chair mover described in LIN 20. Justification: As the name implies, the chair mover enables clinicians to move the chair out of the way enabling examination of wheelchair-bound patients who are unable to otherwise move into the chair. Inability to move the chair out of the way or attempts by clinicians to move the chair manually may result in injury to the patient or the clinician.
Literature Review
6
Characteristic: Chair materials must be sufficient in terms of materials used, and quality to stand up to heavy use and be easy to clean between patients, many times per day. Justification: Torn chairs cannot be used because they cannot be properly cleaned. Chairs that cannot be properly cleaned put patients and staff at risk because they do not meet infection control standards.
8
Characteristic: Chair is preferred, but not required to have a auto-positioning and/or return to default position feature. Justification: Clinicians do not stay in a single exam room all day. They rotate using multiple rooms as patients are arriving and departing. Multiple clinicians may use an exam room during a given day. Clinicians often have a preferred position of the chair for various eye exams and it is helpful to be able to quickly return the chair to its default position to accommodate quicker cleaning of the chair and room preparation for turnover to the next clinician/patient.

1
Characteristic: Must have LED illumination Justification: LED lights are more durable and reliable over the long-term and fail far less frequently than other types of illumination such as halogen lights. Any time the light is down the slit lamp, and therefore the entire exam room is unusable. This impacts efficiency and patient access to care.

Literature Review

2
Characteristic: Must have versatile light-filtering options including red-free, blue, green and grey filtering. Justification: Various exams require the clinician to filter out certain light spectrums (colors).

Literature Review

3
Characteristic: Must have a minimum of 5 Galilean magnification settings (6x, 10x, 16, 25x and 40x). Justification: Various exams require the clinician to set varying degrees of magnification. It is critical that all devices across the service line have the same magnifications setting available to ensure exams performed in one exam room can be comparted to those conducted in another room without variation caused by differing magnification settings.

Literature Review

4
Characteristic: Must be capable of producing high-quality imaging identical to that of the other slit-lamps already in use in the clinic and the affiliated medical university. This is facilitated by certain features of the slit-lamp including slit width and length, rotatability, as well as adjusting eye pieces. The lamp must be able to support a slit width of 0mm to 14mm and a length of 1mm to 14mm. The lamp must also support image rotatability of 0 to 90 degrees. Must support a range of adjusting eye pieces +8 to -8 diopters. Justification: Exams conducted with this device must produce images commensurate with images produced by the other slit-lamps in use within the clinic and the affiliated medical university where the residents (physicians training to become Ophthalmologists are trained). Variations in image quality and precision impact patient care because an image may vary from one exam room to another. For example, a defect within the eye visible in one system, but not visible in another, may lead to missed diagnosis and other complications.

Literature Review

5
Characteristic: Must be able to attach beam splitters and observer tubes accessories. Justification: These accessories enable a second person to see what is happening during the exam. This is an essential function as the RHJ VA HCS is an educational facility and teaches residents (physicians training to specialize in Ophthalmology) to see the eye while the attending physician (instructor) performs the eye exam. Without these devices it would not be possible to teach Ophthalmology.

Literature Review

6
Characteristic: Must have a warranty of a minimum of 10 years. Justification: This is one of the main examination tools used by clinicians during an eye exam. It will be used many times per day and must be able to withstand the wear and tear of daily use. When the device is inoperable, this renders the exam room useless which reduces the number of exam rooms available thereby reducing patient efficiency and flow which impacts the number of patients seen per day. Patient seen per day determines the availability of patient slots for veterans to be seen in clinic. In short, if the machine is down, fewer veterans will be seen timely and more veterans will have to wait longer for care.

1
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have a battery capable of operating for 10 hours without charging Justification: To support a clinic, the device must operate throughout the entire duration of the clinic (anywhere between 4 - 10 hours). If the device cannot last the clinic day it will need to charge and will be unavailable for clinician use during the clinic

Literature Review

2
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have 10-year warranty covering material, processing, construction, and accidental damage in use (such as a clinician dropping and breaking the device). Justification: These devices will be used all day, every day in clinic and need to hold up to rigorous use. And in the event a repair or replacement is needed it must happen quickly or clinical operations will be impacted.
Literature Review
3
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have a yellow, blue and red-free filter. Justification: Filtering various colors is an integral part of the eye exam. The device must be able to support this important aspect of eye exams.

Literature Review

4
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have a minimum interpupillary distance or range of 46mm to 74mm. Justification: This distance is a critical aspect of an accurate eye exam. The device must be able to support this important aspect of eye exams

Literature Review

5
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have a minimum working distance of 400mm. Justification: This distance is a critical aspect of an accurate eye exam. The device must be able to support this important aspect of eye exams
Literature Review
6
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have LED illumination. LEDs are more reliable over the longer term leading to fewer episodes of equipment failure Justification: It is important to minimize equipment failures since such failures mean the equipment will be unusable until repaired. This in turn impacts clinic efficiency and ultimately, patient access to care.
Literature Review
7
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have a battery charge status indicator. Justification: The device cannot run out of power unexpectedly in the middle of an exam which would lead to failure of the exam and potentially a safety issue.
Literature Review
8
Characteristic: The rechargeable hand-held binocular indirect ophthalmoscope (BIO) must have a wall-mounted battery charger. Justification: Surface space in exam rooms is limited and must meet infection control standards. Devices charging on horizontal surfaces impede their cleaning and are more prone to accidental damage.
Literature Review
9
Characteristic: Hands-free operation is preferred, but not required Justification: Clinicians hands are holding other instruments or adjusting settings of equipment and devices that can be operated hands-free facilitate a smoother, more efficient clinic by enabling the clinician to continue making adjustments with both hands while operating the BIO.

1
Characteristic: The acuity system must be digital. Justification: Digital screens offer LED backlight and randomization of targets as opposed to older, out-of-date methods such as a paper chart or projectors with slides. Exam rooms have differences in size, shape, etc. Digital systems are more easily adapted to a given room, or change the background, etc. Older, mirror/projector-based systems mainly do not provide this capability.

Literature Review

2
Characteristic: The acuity system must have at least a 24" color LED backlit display Justification: A large display is helpful to see the eye in a large enough image for clinicians to make accurate assessments. It also facilitates discussions of the images between the clinicians and the patient and the attending physician (instructor) and the resident physician (trainee)

Literature Review

3
Characteristic: The acuity system must have pediatric fixation video capabilities Justification: Many "pediatric" capabilities are useful in examining, diagnosing and educating patients such as those with poor reading and comprehension skills or patients with neurological patients which cannot describe what they are seeing if it is a letter or a number, but can if it’s a picture.
Literature Review
4
Characteristic: The acuity system must have connect to digital phoropters. Justification: The device must connect with and be compatible with phoropters which are already in use in the clinic, to facilitate eye examinations.
Literature Review
5
Characteristic: The acuity system must have astigmatic test charts Justification: These are necessary for complete eye exams capable of detecting and examining astigmatism

Literature Review

6
Characteristic: The acuity system must have LEA symbols and LEA numbers Justification: These are necessary for complete eye exams for patients with poor reading skills

Literature Review

7
Characteristic: The acuity system must have contrast testing Justification: These are necessary for complete eye exams

Literature Review

8
Characteristic: The acuity system must have parent education capabilities Justification: These facilitate discussions with care providers and patients' family members who may need to understand the issue because the patient may be incapable of understanding (i.e. patients with dementia). Furthermore, some "pediatric" education can apply to adults, mainly those with eye misalignment.

Literature Review

9
Characteristic: Must be wall-mounted and come with wall brackets Justification: Exam rooms are of a certain size and there is a lot of equipment in each one, in addition to any people who may be in the room. Floor space is at a premium. Wall-mounted systems save space and allow the device to be stowed out of the way when not in use

Literature Review

10
Characteristic: Must be operable by remote control Justification: The clinician must be able to operate the device while maintaining their position while examining the patient. Without a remote, the clinician would have to stop looking at the eye through equipment and get up from their position to operate the device. This constitutes a disruption of the exam and leads to clinic inefficiency.

Literature Review

11
Characteristic: Is preferred, but not required to be capable of importing videos and images Justification: Using a wide variety of images and videos provides the patient with many options to view and enables the clinician to better detect and diagnose eye issues.

Characteristic: Must have 15" circular seat dimension Justification: This size accommodates the greatest variety and size of the clinicians who will be using them on a daily basis.
Literature Review
2
Characteristic: Must swivel 360° Justification: During the course of an exam day, the clinician must frequently reposition and swivel around between various pieces of equipment and patients, etc. Having a stool that can swivel fully 360° enables the clinician to move without being disrupted by a stool that won't turn where they need it to.
Literature Review
3
Characteristic: Must have 5 legs for stability Justification: Safety guidelines demonstrate the 5-legged stools are far less likely to overturn when occupied than those with fewer legs. This is required for the safety of the clinicians who will be using the stools on a daily basis.
Literature Review
4
Characteristic: Must have a vertical lift of 6.25" Justification: Lifting to this height enables clinicians to reposition vertically during the exam in order to reach all the equipment in use during an exam.
Literature Review
5
Characteristic: Must have must have a seat height of 18" to 25" Justification: This height range ensures the comfortable seating of clinicians of all sizes. Clinicians do not stay in the same room all day. They move in and out of a given eye lane room as one becomes available for the next patient and then must adjust their seating accordingly.
Literature Review
6
Characteristic: Seat cushion must be made (covered with) material that are easy to clean and maintain in accordance with infection control procedures Justification: The stool will be used every day, all day that the clinic is open. The seat must hold up to wear and tear as well as be easily cleaned/disinfected per infection control standards.

Literature Review

7
Characteristic: Seat adjustment settings are preferred, but not required, to be intuitive and easy to use/learn Justification: The intent of the stool is to facilitate the seated mobility of the clinicians during the exam. Stools with complex to use, or complex to understand controls impede this purpose.

Some lens sets come in adjustment intervals greater than 0.12, such as 0.25. Such systems do not allow for the more precise and accurate measurements which in turn influences the diagnosis and prescription.

Characteristic: Accessory lenses must include red, occlude, pinhole, and slit options Justification: These are necessary to facilitate the complete eye exam among a very broad and diverse patient population. Often, these accessories are purchased separately. Including them in the set ensures their compatibility.
Literature Review
3
Characteristic: Lenses must be compatible with frame requirements listed in LIN 12 Justification: Lenses cannot be used without a frame and may or may not fit frames that are not designed to hold them. It does not make sense to purchase lenses and frames that do not fit together.

Characteristic: Trial frame must fit with and be compatible with Lense requirements described on LIN 11. Justification: Lenses cannot be used without a frame and may or may not fit frames that are not designed to hold them.
Literature Review
2
Characteristic: Must provide smooth controls for quick, easy lens fitting Justification: a clinician must switch between various lenses frequently during an examination. Often this means switching back and forth between one or more lenses to facilitate their observation of the patient's eye. If it is cumbersome or difficult to move between lenses, it will dramatically impede the clinicians ability to conduct the exam and directly impact clinic efficiency.
Literature Review
3
Characteristic: Must have multiple lens seating options and adjustability factors such as pupillary distance, tilt, nasal adjustment, and adjustable temple pieces Justification: It is essential that patients remain comfortably and perfectly still during exam. For this, it is important for the frame to have a broad variety of adjustments to accommodate patients of all sizes and shapes comfortably. Patient movement directly impacts the accuracy of the exam.

1
Characteristic: Must have a digital monitor (LED or LCD) Justification: The monitor must be absolutely clear and easy to see and read. As the clinician uses the machine to determine the power of a given Lense, it’s important that they can see the reading quickly for the sake of efficiency and clearly to avoid mistakes.
Literature Review
2
Characteristic: Must have automatic Mono & Multi-focal detection Justification: Modern lenses come with a variety of coatings (for UV protection, etc.). A machine that can automatically account for this is essential for clinic efficiency and prevent errors.
Literature Review
3
Characteristic: Must have a green measurement light Justification: The green light eliminates errors due to varying ABBE values (a lenses value of disbursement of light). This enables the clinician to examine the lens and the reading is not influenced by aberrations or distortions. Without a green light the lens may end up being incorrect for the patient.
Literature Review
4
Characteristic: Must measure lenses for glasses and both hard and soft contact lenses Justification: Necessary to ensure the proper fit of a Lense of either type. Otherwise, multiple machines are necessary (one for each type).
Literature Review
5
Characteristic: Must provide means to measure Pupillary Distance as well as Ultraviolet (UV) Measurement Function information on the UV transmittance in the range from 0% to 100% Justification: Necessary to ensure the proper fit of a Lense. This ensures the center of the power of the Lense is precisely where it should be for the patient.

Literature Review

6
Characteristic: Must have a minimum of a 5" tilt monitor Justification: Because clinicians of all sizes may use this device tilting the monitor (screen) so they can properly see it is essential. If the monitor is at an improper angle, it may distort what the clinician sees. For example, one cannot see all televisions clearly if the viewer is too far off to one side or another.
Literature Review
7
Characteristic: Must have a printer Justification: Printing results in the exam room is necessary to maintain efficiency. If the clinician must go find a printed document down the hall it will slow the clinic significantly.
Literature Review
8
Characteristic: Must have an interface to send data to a patient's electronic medical record (specifically, it must be capable of interfacing with the RHJ VA HCS system which is VistA/CPRS Justification: Necessary for documentation of care.
Literature Review
9
Characteristic: Must have capability to provide measurement of Pupillary Distance (PD) Justification: Measurements of PD ensures the proper placement of the center of power for the Lense. Without this, a Lense cannot be properly fitted.
Literature Review
10
Characteristic: Touch screen controls are preferred, but not required Justification: ease of use for clinicians who must manage multiple devices simultaneously and touch screens are more able to be cleaned and kept in sanitary conditions than other controls.
