About this file

This is a Past Performance Evaluation Questionnaire form issued by the U.S. Army Contracting Command - Aberdeen Proving Ground (ACC-APG) for RFP W56ZTN-24-R-0013 related to Office Administrative Services (NAICS 561110). The questionnaire requests performance feedback from references identified by offerors regarding their past performance on similar federal, state, local government or commercial contracts within the past three years.

The form includes two rating categories (Acceptable and Unacceptable) and evaluates five key areas: compliance with specifications and workmanship standards, effectiveness of project management, timeliness of performance, cost control effectiveness (for cost reimbursement contracts only), and commitment to customer satisfaction. Responses must be submitted electronically within 15 days to the contracting officer POCs Amy Bowen and Nicole Sutton. The form includes sections for contract identification details, offeror representative information, evaluator comments, and respondent identification. This questionnaire will be used as part of the competitive source selection process for a Military Personnel Office Support contract at Aberdeen Proving Ground.
