PERFORMANCE WORK STATEMENT
3/160th SOAR FY23 Medical Training Exercise Support
AT
Hunter Army Airfield, GA and Fort Stewart, GA
27-31 March 2023

1 DESCRIPTION OF SERVICES: The contractor shall provide cinematic quality makeup artists, Role Players with acting experience while in an intense trauma scenario. The contractor shall provide Role Players that are specifically veterans and are able to pass background checks to gain access to DOD installations. The contractor shall provide a not for harm clause to fly in Army Helicopters accepting seats out risk within their contract in accordance with USSOCOM 350-6 Table 7-1. Role Players must be properly equipped by the contractor with the required equipment to fly IAW AR 95-1. Role Players cannot have a fear of heights, dark, or enclosed spaces. Additionally, Role Players must be equipped with body armor, helmet, weapon, Individual First Air Kit (IFAK), and additional tourniquet provided by the contractor. The contractor shall provide: Five Role Player contractors, Four of which being amputees, subject matter experts to assist in patient set up and wound pattern capable of split operations, a Moulage Artist, a Training aid (MQ-9 and AH-1) with replicated sensitive items that are able to be recovered, and all capable of being reset within 18 hours. Role Players must be able to receive intravenous injections of medications through multiple intravenous access ports. All training will take place at Hunter Army Airfield, GA; Fort Stewart, GA; and surrounding areas.
Period of Performance: 27 March 2023 – 31 March 2023
1 GENERAL INFORMATION
1.1 Removal of items or any type of objects from government refuse for personal use is unauthorized. Contractor personnel may submit a formal request to obtain items through unit designee or security personnel. Violating this regulation may be subject to termination of this contract.
1.2 At no time, without prior authorization, shall contractor personnel purchase any items from the local economy for any U.S. personnel.
1.3 Contractor Housing: Service will be completed at requestor location (Hunter Army Airfield, GA; Fort Stewart, GA; and surrounding areas).
1.4 The contractor shall provide contact information on all the points of contact within the company to interface with the Contracting Officer Representative (COR) on issues concerning contract requirements. Names, business addresses, work and cell phone numbers and hours of operation are the minimum information requirements. Arrangements for contacting the contractor’s points of contact after normal business hours are also required information for the Contracting Officer Representative (COR).
1.4.1 The Contracting Officer Representative (COR) is the individual nominated by their organization/unit and is appointed by the Contracting Officer to ensure contract compliance with the timely delivery of goods to be delivered and/or services to be performed as specified within this contract.
1.5 The contractor is required to establish and maintain a quality control program for the performance requirements of this contract. The contractor is required to provide a copy of the plan and to conduct a meeting with detailed information contained within the plan to the Contracting Officer Representative (COR). At a minimum, the plan is required to meet the quality standards set forth in this contract.
1.5.1 The contractor shall be responsible for the total management and quality control actions necessary to meet the quality standards set forth in this contract. Changes to the actions must be approved and documented by the Contracting Officer only.
1.5.2 The contractor shall notify the Contracting Officer or the Contracting Officer Representative (COR) of any issues regarding delays in services upon identification
1.6 Security Requirements:
1.6.1 The contractor shall comply with all prescribed security requirements in effect during the work period. Failure to comply with security requirements will lead to removal of personnel and/or cancellation of contract as deemed necessary.
1.6.2 Physical Security: Responsible for safeguarding all government property provided for contractor use. Secure all government facilities, property, and materials at the close of each work period.
1.6.3 Vehicles: Contractors will operate and repair equipment at their own site.

4.1.4 Equipment: The following minimum criteria shall be met at the time of the delivery of requested equipment: training aids, apparatuses, role-player uniforms, and consumable training aids that enable realistic medical training.
