About this file

This document contains an evaluation sheet and federal contract opportunity notice. The evaluation sheet provides a template for offerors to self-assess their qualifications across criteria including public health experience, policy experience, deployment readiness, clinical support, and public speaking skills. Technical ratings and overall scores are automatically calculated.

The federal contract opportunity is a small business set-aside notice seeking a physician to provide up to 600 hours of medical support services annually to the National Science Foundation Office of Polar Programs. Duties include medical waiver reviews, consultation on issues like COVID-19 response plans, liaising with a medical review panel, and other advisory services. The requirement encompasses a one-year base period and four one-year options, to be awarded as a firm-fixed-price contract by late June or early July 2021. The period of performance will begin on August 2, 2021. Questions are due by June 7, 2021. The incumbent contractor was awarded sole-source authority in 2020.

Preamble

The purpose of this excel sheet is to provide an evaluation of the different skillsets this position requires in a manner that is as objective as possible, thus allowing comparisons of offerors.

Offerors are required to fill out this form to the best of their ability. As a result, upon completion offerors will know their technical rating.

As a reminder, while technical is weighted greater than price, price still influences award determination. Offerors should not be discouraged based on the technical calculations.
