1. DESCRIPTION OF SERVICES: This Performance-Based Work Statement (PWS) is for fixed fee calibration on the Material Test System (MTS), High Rate Propellant Test System (HRPTS), Identified in Appendix E for the 582 Missile Maintenance Squadron (582 MMXS), Propellant Laboratory (582 MMXS/MXDTA) at 6357 Maple Lane, Hill Air Force Base (HAFB), Utah 84056-5966. This system is a key element of the propellant testing support for the Air Force Nuclear Weapons Center (AFNWC) for the Aging and Surveillance Program (ASP) in conducting service life estimates for fielded Intercontinental Ballistic Missiles (ICBMs).

1.1. CONTRACTOR FURNISHED PROPERTY AND SERVICES: The Contractor shall perform, at a minimum; one (1) fixed fee calibration per year. The service shall be performed during normal working hours, with future calibrations shall be completed prior to expiration.

During each visit, the Contractor shall ensure that the MTS HRPTS operates within design specifications by providing calibration support equivalent to the original equipment manufacturers (OEMs) specifications and shall ensure that the system performs properly before leaving. The Contractor shall provide calibration certificate within 5 business day after completion of calibration. (CDRL A001)

1.1.1. Fixed Fee Calibrations. The Contractor shall schedule annual fixed fee calibration visits prior to current calibration expiration.

1.1.2. Load Cells. The contractor shall calibrate the load cells identified in Appendix E as part of the overall MTS HRPTS system calibration. These load cells are not calibrated separately, but included as part of the overall system. Load Cells shall be calibrated annually.

1.1.3. Linear Variable Differential Transducer (LVDT). The contractor shall calibrate the LVDT identified in Appendix E as part of the overall MTS HRPTS system calibration. The LVDT is not calibrated separately, but included as part of the overall system. LVDT calibrations shall be performed annually.

1.1.4. Clip-on Extensometer. The contractor shall calibrate the Clip-on

Extensometers identified in Appendix E as part of the overall MTS HRPTS system calibration. Clip-on Extensometers are not calibrated separately, but included as part of the overall system. Clip-on Extensometers shall be calibrated annually.

1.1.5. Pressure Transducer. The contractor shall calibrate the Pressure Transducer identified in Appndix E as part of the overall MTS HRPTS system calibration.

The Pressure Transducer shall be calibrated annually.

1.1.6. Temperature controller. The contractor shall calibrate the Thermocouple /

Temperature controller Identified in Appendix E as part of the overall MTS HRPTS system calibration. Temperature controllers shall be calibrated annually.

1.1.7. Equipment Item Calibration List. A list of all equipment calibration items are in Appendix E – Equipment Item Calibration List.

2. SERVICE SUMMARY (SS)

3.0 GENERAL INFORMATION:

3.1 Government Furnished Property:

No GFP will be provided under this contract.

Access to Government owned equipment shall be provided to the contractor including engineering workstations, test stations, and assets. All government owned equipment must be safeguarded by the contractor. Government owned equipment will remain in possession of the Government and is only available for contractor use during normal working hours.

3.2 Security:

3.2.1 HAFB and Missile and Ammunition Maintenance and Storage (MAMS) Access:

3.2.2 The contracted company representative shall provide a Visit Pass Request to Chemical Properties Laboratory Lead or Supervisor which shall include: First and Last Name, Driver’s License Number, Driver’s License State in which it was issued, dates of visit and shall be on corporate letterhead signed by contractor’s Security Manager or management.

Visit Pass Request shall be the completed No Later Than (NLT) 7 days prior Scheduled Preventative Maintenance visit IAW.

3.2.2.1 The contracted company representative shall check in at the South Gate Visitor Center. They shall have a valid vehicle registration, driver’s license, and proof of insurance.

3.2.3 The Contracted Company Representative will be accessing Hill AFB to calibrate/repair test equipment as required. They will not be accessing classified or sensitive information. They do not require computer access. They will be escorted into munitions storage areas.

3.2.4 The GCMS is located in the MAMS II secured area. The calibration/service technician shall coordinate access with the Chemical Properties Laboratory Lead at (801) 777-5200 or (801) 777-5552 or Supervisor at (801) 777- 7420. This area requires an escort and an in-briefing upon arrival.

3.3 Special Requirements:

3.3.1 Contractor shall provide a manufacturer certified representative to accomplish all calibration and maintenance. Shall have expertise and knowledge associated with the MTS HRPTS that was specifically built for the Propellant Test Center, HAFB operation. Air Force Metrology and Calibration Program Office (AFMETCAL’s) requirements, as outlined in Technical Order (TO) 00-20-14 attached appendix D, shall be met.

3.4 Publications:

The contractor shall follow, troubleshoot, repair and calibrate according to procedures recommended by the OEM.

3.5 Quality Control:

3.5.1 The Chemical Properties Laboratory Lead or Supervisor are the authorized individual(s) who will perform assessment of the contractor’s performance.

3.5.2 The Chemical Properties Laboratory Lead or Supervisor will inform the Government

Contracting Officer (CO) when discrepancies occur and will request corrective action.

3.5.3 Any matter concerning a change to the scope, prices, terms or conditions of this contract shall be referred to the Contracting Officer (CO).

3.5.4 The services to be performed by the contractor during the period of this contract shall at all times and places be subject to review by the CO and/or authorized Chemical Properties Laboratory Lead or Supervisor.

3.6 Tool Control

3.6.1 Tool Control and Accountability Procedures – The Contractor shall comply with Tool

Control and Accountability Procedures as listed herein when contractor owned tools or equipment are brought into any maintenance area within the Complex:

3.6.2 The Contractor shall maintain 100% accountability of contractor-owned tools at all times.

The contractor shall inventory all contractor-owned tools located in the work center at the beginning and end of each job task location change and at the end of each day/shift.

Inventories must be documented and are subject to inspection by the Chemical Properties Laboratory Lead or Supervisor. The Contractor shall secure tools at all times when the contractor is not present at the immediate worksite.

3.6.3 Inventory discrepancies will immediately be researched and reported to the work center supervisor, Chemical Properties Laboratory Lead or Supervisor. If a tool is identified as being lost, the Contractor shall comply with AF lost tool procedures outlined within AFI 21- 101 AFMCSUP AFMCGM2022-01 CH 8. The Contractor shall ensure contractor personnel working with in the Complex Maintenance area are familiar with the 100% accountability of tools and the proper reporting for lost tools.

3.7 Government Observatins:

3.7.1 During the period that the company representative is working on the system, government personnel from the Missile Maintenance Group, Missile Maintenance Squadron, shall oversee the trouble shooting, repair and calibration of the GCMS.

3.8 Safety:

3.8.1 In performing work under this contract, the contractor shall:

3.8.1.1 Conform to the safety requirements contained in this contract for all activities related to the accomplishment of the work.

3.8.1.2 Comply with additional, immediate precautions that the CO may require for safety and mishap prevention purposes.

3.8.1.3 Provide protection to government property to prevent damage during the period of time the property is under the control, or in possession of, the contractor.

3.8.1.4 Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.

3.8.1.5 In the event of an accident or mishap, the contractor shall take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO.

3.8.1.6 Record and report promptly (within one hour) to the CO and/or authorized

Chemical Properties Laboratory Lead or Supervisor, all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

3.8.1.7 If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

3.9 Hours of Operations:

3.9.1 Normal Hours of Operation:

The contractor shall perform the services required under this contract during the following hours:

Monday through Thursday, 0600-1530. Work will not be scheduled on Federal holidays. With prior approval from the CO, the Contractor may work extended hours to ensure timely completion of work at no additional cost to the government.

3.9.2 Recognized Federal Holidays:

The contractor shall not provide service on the following Federal Holidays: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day and Christmas Day. If the holiday falls on a Saturday, it is observed on the prior Friday. If the holiday falls on a Sunday, it is observed on the following Monday.

Requirements:

f. A description of, the condition of, and the unambiguous identification of the item(s) tested or calibrated;

g. The date of receipt of the test or calibration item(s) where this is critical to the validity and application of the results, and the date(s) of performance of the test or calibration;

SECTION I - GENERAL REQUIREMENTS

A. Safety Program Requirements

The contractor shall implement a safety program plan that ensures protection of Government personnel and property. The program shall consist of, as a minimum:

1. Mishap reporting, as defined in paragraph B1 below.

2. A Safety Plan that addresses, as a minimum, the subjects listed in Section II –Specific Requirements, and shall be used during the performance of the work described in the contract. The Safety Plan shall be approved by OO-ALC/SE (Safety Office) prior to commencement of any work described in this contract.

3. Routine and recurring surveillance to ensure the safety requirements of this contract are enforced.

4. Competent personnel to provide surveillance of the Safety Plan.

5. Identification of segregated work site locations for operations that cannot be co-mingled with general industrial operations and the process for ACO approval of operations and changes at these specific sites.

6. All contractor personnel shall be trained and qualified to perform their duties safely.

7. The contractor shall include a clause in all subcontracts requiring the subcontractors to comply with the safety provisions of this contract, as applicable.

B. Mishap Notification

1. The contractor shall notify the OO-ALC/SE Workflow (OO- ALC.SE.Workflow@us.af.mil) or the Hill AFB Command Post (801-777-3007) after normal duty hours, and the designated Government Representative (GR), i.e., the ACO, PCO, or DCMA QAR (Quality Assurance Representative) within one (1) hour of all mishaps or incidents at or exceeding $2,000 (material + labor) in damage to DOD property entrusted by this contract, even if the government is wholly or partially reimbursed. This notification requirement shall also include physiological mishaps/incidents. A written or email copy of ALL the mishap/incident notification shall be sent within three calendar days to the GR, who will forward it to OO-ALC/SE. For information not available at the time of initial notification, the contractor shall provide the remaining information no later than 20 calendar days after the mishap, unless extended by the ACO.

Mishap notifications shall contain, as a minimum, the following information:

(a) Contract, Contract Number, Name and Title of Person(s) Reporting

(b) Date, Time, and exact location of accident/incident

(c) Brief Narrative of accident/incident (Events leading to accident/incident)

(d) Cause of accident/incident, if known

(e) Estimated cost of accident/incident (material and labor to repair/replace)

(f) Nomenclature of equipment and personnel involved in accident/incident

(g) Corrective actions (taken or proposed)

(h) Other pertinent information

2. The contractor shall cooperate with all government mishap investigations. Additionally, if requested by government personnel or designated government representative (GR), i.e., the ACO, PCO, or DCMA QAR (Quality Assurance Representative), the contractor shall immediately secure the mishap scene/damaged property and impound pertinent maintenance and training records, until released by safety investigators.

3. The contractor shall provide copies of contractor data related to mishaps, such as contractor analyses, test reports, summaries of investigations, etc. as necessary to support the government investigation.

4. The contractor shall support and comply with the safety investigation and reporting requirements of DAFI 91-204, Chapters 1 – 5.

C. General Safety Requirements:

If the safety plan is modified, the contractor shall submit the proposed modification, in writing, to the Contract Administration Office safety representative.

The contractor is solely responsible for compliance with all federal, state, and local laws, the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standards 29 CFR 1910 and 1926, as applicable, Air Force Occupational Safety and Health Standards (AFOSH STD), to include DAFMAN 91-203, Air Force Occupational Safety, Fire, and Health Standards, DAFI 91-204, Safety Investigations and Reports, Contract Safety, and the protection of their employees.

Additionally, the contractor is responsible for the safety and health of all subcontractor employees.

The contractor shall ensure assigned personnel are adequately trained and qualified for the task being performed. Brief all personnel on the hazards involved with operations and applicable precautions to be taken. Should unidentified hazards arise, cease operations until actions are taken to eliminate or mitigate hazards to safe levels.

Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor. Note:

AFOSH STDs are annotated because many of the Air Force Standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence.

The government shall assume no liability or responsibility for the contractor's compliance or non-compliance with such requirements. The contractor shall furnish to each of his/her employees a place of employment which is free from recognized hazards. The contractor shall brief his/her employees on the safety requirements of this contract and on hazards associated with prescribed tasks. The contractor is responsible for compliance with OSHA Public Law and the resultant standards identified within. In addition, the contractor is required to flow down the safety requirements/specification to all subcontractors. This applies to Federal Acquisition Regulation (FAR) 12 commercial acquisitions as well. This contract shall in no way require persons to work in surroundings or under working conditions which are unsafe or dangerous to their health. The contractor must coordinate and perform work so as not to impact the safety of government employees or cause damage to government property. This requires providing personnel with protective equipment and associated safety equipment as may be necessary. The contractor must also protect personnel from hazards generated by the work. If the contractor employs BILINGUAL speaking employees, they must post bilingual signs and have written procedures for specific tasks in applicable languages.

SECTION II – SPECIFIC REQUIREMENTS

The contractor’s prepared Safety Plan shall:

Provide contact information for on-site personnel Include a work hazard analysis of the worksite and operations to be performed to include baseline hazard identification and required control measures Identify employee safety and health training requirements and the documentation process

The contractor shall ensure that each element identified below is adequately addressed in detail in the safety and health plan:

PEDESTRIAN CROSSWALKS: All contractor personnel are required to use the closest crosswalk or traffic-controlled intersection when crossing the road. Pedestrians must look both ways to ensure the coast is clear before stepping out into the crosswalk. Pedestrians DO NOT have the right of way unless they are already in the crosswalk. Contractor vehicle operators have the same responsibilities as pedestrians to share the road and mutually observe and yield to pedestrians.

MOTOR VEHICLES: Contractor shall comply with the standards in: DoD Directive 5525.4, Enforcement of State Traffic Laws on DoD Installations", Nov 2, 1981, Para 3-4;

DODI 6055.4, DoD Traffic Safety Program, 20 Jul 99; AFI 91-207, USAF Traffic Safety Program, 22 May 07; Each applies to all persons at any time on an Air Force Installation and includes all leased, owned, or privatized property including housing areas. In addition:

AFI 13-213, Airfield Management, 29 Jan 08, Paragraphs 1.3.6 and 4.4.2.1 applies to all contractors, sub-contractors, vendors, commercial delivery companies, and all other private business vehicles who operate anywhere on Hill Air Force Base, including the airfield (to include the industrial areas and any buildings or hangars located upon the airfield) in support of their mission.

HOUSEKEEPING: Housekeeping shall be conducted according to the requirements in OSHA Standard 29 CFR 1910.141. CLEAN AS YOU GO will be enforced.

APPENDIX D

AFMETCAL TO 00-20-14 STATED REQUIREMENTS FOR CONTRACT

CALIBRATIONS

4.5.1.1 General. The 562 CBSG is the only authority for approving commercial calibration service, unless authorized in paragraph 4.5.1.2.2 or 4.5.1.4. An Air Force PMEL shall calibrate TMDE in accordance with the applicable CMS or T.O. 33K-1-100-2. When organic (Air Force) capability exists, commercial calibration shall not be purchased and support shall be obtained from another PMEL IAW Section 4.1 of this TO. If organic capability does not exist or calibration requirements exceed the AFMETCAL program capabilities, then supporting PMELs may submit a Request for Authorization of Commercial Calibration Services. When authorized to contract any part of the calibration, the work shall be placed with a calibration source that provides a calibration certificate/report containing the information required by Data Item Description DI-QCIC-80798B as a minimum (see paragraph 4.5.1.4). Also, the calibration should meet the same performance specifications of the Air Force calibration authority; if not, any deviation shall be documented and coordinated with the owner.

4.5.1.2 Requests for Commercial Calibration Service. OWCs and PWCs shall obtain 562 CBSG approval prior to procuring any calibration services from commercial sources except as noted below in paragraphs 4.5.1.2.1 and 4.5.1.2.2. The OWC and PWC shall accomplish each of the following:

a. Verify that the part number is listed in TO 33K-1-100-2 or an applicable CMS, and:

(1) If not listed, assist the supporting PMEL in preparing and submitting an AFTO Form 45 in accordance with Section 5 of this TO.

(2) If listed, deliver the TMDE to the supporting Air Force PMEL for calibration. If the supporting PMEL lacks the capability (e.g., trained personnel or equipment) to accomplish the calibration, then the supporting PMEL shall seek lateral support from another PMEL IAW paragraph 4.1 of this TO. If no lateral support source can be located, the supporting PMEL shall contact the MAJCOM PMEL MFM for approval to submit the request.

b. When approved to submit a request for commercial calibration services:

(1) Prepare a Request for Authorization of Commercial Calibration Services; a form letter is available on the AF Portal, Metrology & Calibration, Products and Services, Customer Support, Commercial Cals, Commercial Calibration Support, Request for Authorization of Commercial Calibration Services.

FAX or EMAIL the request to the 562 CBSG office indicated on the Request Form.

(2) The requestor shall ensure and be able to provide documentation that the contractor is able to perform a calibration that meets the same performance specifications of the Air Force calibration authority, or that any deviation from the calibration authority was coordinated with the owner.

The 562 CBSG shall, at its discretion, determine if additional data or a site visit is required. The calibration results shall be reported in a calibration certificate/report containing the information required by Data Item Description DIQCIC- 80798B as a minimum (see paragraph 4.5.1.4)

4.5.1.2.1 Exception for Items Designated CONTRACT. Items listed in TO 33K-1-100-2 or an applicable CMS with a calibration responsibility of “CONTRACT” do not require a Request for Authorization of Commercial Calibration Services (see CONTRACT definition in TO 33K-1-100- 1).

APPENDIX E
