About this file

The document is a Living Wage Certification Form issued by the Town of Manchester, Connecticut, pertaining to a contract opportunity for the purchase of athletic field marking paint. The form requires bidders to certify whether they are a "Covered Employer" under the Manchester Living Wage Ordinance (Chapter 212, Sections 212-1 through 212-11) and must either commit to paying the required living wage to eligible employees or qualify for an exemption based on specific criteria such as being a nonprofit organization, employing fewer than 25 eligible employees, or having an annual contract value less than $25,000.

Bidders must complete and return this certification form with their bid or RFP submission, indicating their status as either a covered employer or an exempt entity. The form provides three potential exemption categories: charitable foundations or nonprofits with 501(c) tax-exempt status, employers with fewer than 25 eligible employees, or contracts with an annual value under $25,000. Failure to properly complete and indicate the appropriate status may result in bid rejection. The form requires the signature of an officer, owner, or authorized representative, who certifies the accuracy of the representations made.
