About this file

The document is a Living Wage Certification Form issued by the Town of Manchester, Connecticut, specifically designed for bidders participating in municipal contract opportunities. The form requires bidders to indicate whether they are a "Covered Employer" under the Manchester Living Wage Ordinance (Chapter 212 of the Manchester Code of Ordinances) and comply with wage requirements, or certify an exemption based on specific conditions such as being a nonprofit organization, employing fewer than 25 eligible employees, or having an annual contract value less than $25,000.

The certification form is a mandatory submission document for bid or Request for Proposal (RFP) responses, with potential bid rejection for failure to complete the form accurately. Bidders must either affirm they will pay the required living wage to eligible employees throughout the contract term or select an appropriate exemption category. The form requires the signatory (an officer, owner, or authorized representative) to certify the accuracy of their representations, and must be signed and dated as part of the bid or RFP submission process.
